BACKGROUND: Hemipelvectomy is a major operation with significant risks including infection, prosthesis failure and fracture. This systematic review was designed to review the functional outcomes, oncologic outcomes and complications in patients who received internal hemipelvectomy and pelvic reconstruction for primary pelvic tumour.
METHODS: Searches on MEDLINE, the Cochrane Library, Embase and Google Scholar were performed to locate studies involving patients receiving internal hemipelvectomy and pelvic reconstruction using a prosthesis, implant or bone graft. All studies were either prospec

Traumatic hemipelvectomy through the sacroiliac joint is a devastating injury, mainly because of motor vehicle accidents. Recent improvements in prehospital trauma care have increased the chances of survival for victims. Besides amputation of the lower limb, associated complications usually involve digestive and urological systems. We report on 2 pediatric patients from 2 different European countries. PATIENT 1: A 9-year-old boy suffered uprooting of his left lower limb, laceration of the rectum and anal sphincter, as well as an injury to distal urethra with partial loss of cavernous bodies. I

Near-complete traumatic hemipelvectomy probably carries an extremely high mortality rate. The usual techniques which have been used to control major hemorrhage associated with pelvic fractures such as transperitoneal vascular ligation, intra-arterial embolization, and packing are not applicable (14). Successful management requires prompt recognition of the nature of this injury so that surgical efforts may be directed at resuscitation and expeditious operative completion of the traumatic amputation. When this decision is made appropriately, the dual goals of control of hemorrhage and preventio

This tenth survivor of traumatic hemipelvectomy was a young male involved in a collision between a car and his bicycle. As in previous cases, he sustained severe associated injuries to the genitourinary tract and rectum. Atypically he required secondary closure with autogenous skin taken from the severed lower limb, and in addition, he developed a compartment syndrome in the opposite leg.

AIMS: A type III internal hemipelvectomy includes resection of the pubic rami. Although bony reconstruction is not necessary, the integrity of the abdominal wall is deficient following resection. The aim of this study was to report the outcome of soft-tissue reconstruction following these resections.
METHODS: We retrospectively reviewed all isolated type III internal hemipelvectomies which were undertaken at the Mayo Clinic (Rochester, USA) between January 2002 and December 2022. The group included 25 patients, of whom 14 (56%) were male. Their median age was 45 years (IQR 61 to 31) at the ti

Phlegmasia cerulea dolens (PCD) results from near complete venous thrombosis and occlusion in an extremity resulting in pain, cyanosis, and edema and potentially death. While PCD can result from a variety of mechanisms including trauma, iatrogenic instrumentation, the majority of cases occur in the setting of malignancy. PCD is often managed medically with anticoagulation. However, in certain circumstances, surgical intervention may be warranted. Here, we present a unique case of phlegmasia cerulea dolens occurring following an internal hemipelvectomy requiring emergent surgical intervention t

OBJECTIVE: Treatment of tumors of the pelvic girdle by resection of part or all of the innominate bone with preservation of the extremity. Implantation and stable fixation using a custom-made megaprosthesis to restore painless joint function and loading capacity. The surgical goal is to obtain a wide surgical margin and local tumor control.
INDICATIONS: Primary bone and soft tissue sarcomas, benign or semi-malignant aggressive lesions, metastatic disease (radiation resistance and/or good prognosis).
CONTRAINDICATIONS: Limited life expectancy and poor physical status, extensive metastatic dis

We report on our experiences with internal hemipelvectomy followed by an implantation of a endoprosthetic pelvic replacement (EPR) performed in 15 patients over a period of 15 years. Our primary aim was to determine the implications for this procedure because of its high rate of complications. Due to the malignant character of the disease and the biomechanical stress on the anatomical region, the demands on the surgeon are high. The most important factor is a sufficiently wide resection of the primary tumour because most are chondrosarcomata which do not respond to other therapies. In addition

Pelvic resection for a sarcoma in young patients is challenging, with significant functional, psychological, and social implications. We report the case of a 26-year-old former Ewing's sarcoma patient in whom the Ilizarov technique was applied to address 6 cm of leg length discrepancy secondary to internal hemipelvectomy with superomedial dislocation of the right femoral head. Fifteen years after the internal hemipelvectomy, the patient underwent distal femoral and bifocal tibial lengthening using circular frames. The results obtained in this patient underline the usefulness of the Ilizarov tr

INTRODUCTION: Amputations at the hip and pelvic level are often performed secondary to high-energy trauma or pelvic neoplasms and are frequently associated with a prolonged postoperative rehabilitation course that involves a multitude of health care providers. The purpose of this study was to examine the health care utilization of patients with hip- and pelvic-level amputations that received care in the U.S. Military Health System.
MATERIALS AND METHODS: We performed a retrospective review of all patients who underwent a hip- or pelvic-level amputation in the Military Health System between 20

BACKGROUND AND OBJECTIVES: Recent studies have reported acceptable outcomes after arthrodesis takedown and conversion to total hip arthroplasty (THA); however, there are no reports on outcomes after oncologic resection, which are inherently complex and may portend poorer outcomes. The purpose of this study was to examine the surgical and functional outcomes of patients who underwent prior hemipelvectomy for tumor resection and were later converted to THA.
METHODS: All patients who had prior iliofemoral arthrodesis after oncologic resection that were later converted to THA at a single institut

In this case report, a 67-year-old woman presented with increasing pain in the left side of the thigh and pelvis. A PET scan revealed a large malignant tumour in the left side of the pelvis and proximal left femur, and an ultrasound-guided biopsy diagnosed an undifferentiated pleomorphic sarcoma. Combined orthopaedic and plastic surgery resulted in a left-sided hemipelvectomy and amputation of the left leg. The amputated left leg was used as a free fillet flap for reconstruction of the pelvis. The patient healed without complications, and seven months post-operatively she was able to use a pro

Pelvic fractures are a common injury in cats, and both surgical and conservative management approaches have been described. One of the major complications of pelvic fractures managed conservatively is narrowing of the pelvic canal. Severe pelvic canal narrowing can result in constipation and subsequent megacolon. The purpose of this case series is to describe the long-term outcome for 3 cats with obstipation treated with internal hemipelvectomy because of megacolon secondary to pelvic canal narrowing after conservative management. All cats had a good functional outcome of the affected limb. Tw

Primary bone tumours of the pelvis usually present late, with large tumour size, in proximity to major neurovascular structures and visceral organs. Due to its operative morbidity, prolonged time of surgery, associated blood loss, and inherent anatomical challenges, internal hemipelvectomy is practiced in specialised centers only. This study was designed to evaluate the surgical complications and oncological outcomes in patients undergoing internal hemipelvectomy for primary pelvic bone tumours. A prospectively collected database at our Institute was retrospectively reviewed, and 51 consecutiv

INTRODUCTION: Traumatic hemipelvectomy is a rare operation, often performed for injuries caused by a high force mechanism such as a traffic or industrial accident, and often fatal. Most often performed emergently, here we report a traumatic hemipelvectomy performed on a pediatric patient one month from initial injury.
CASE PRESENTATION: A 7-year-old healthy male was crushed between two motor vehicles while riding his bicycle. He was taken emergently to the nearest trauma center and underwent external fixation of his pelvis, exploratory laparotomy, abdominal packing, diverting colostomy, vesic

CASE: A 46-year-old man was involved in a high-energy collision resulting in a left traumatic hemipelvectomy with contralateral pelvic fractures and regional soft-tissue injuries. He had a pulseless left lower extremity and multiple life-threatening injuries. Emergent hemipelvectomy was performed, with intraoperative findings confirming external iliac artery transection. Remaining orthopaedic injuries were repaired sequentially, including percutaneous fixation of contralateral acetabular fracture.
CONCLUSION: Traumatic hemipelvectomy requires rapid intervention. Early amputation is necessary

BACKGROUND: External hemipelvectomy (EH) is a complex amputation that removes the hemipelvis and leg, most commonly performed to resect a locally advanced pelvic malignancy. This destabilizes the spinopelvic anatomy and may result in a compensatory scoliosis (CS) to maintain an upright posture. The authors hypothesized that pelvic ring reconstruction with vascularized bone (VB) will reduce CS and improve functional outcomes.
METHODS: The authors conducted a retrospective cohort study of patients who underwent an EH between May of 2023 and January of 2004. Patients were categorized by use of V

A 15-year-old male domestic shorthair cat was presented with a mass over the left ilium. Computed tomography showed an osteolytic lesion originating from the left ilial wing with soft tissue invasion but no pulmonary metastasis. Cytology was consistent with a sarcoma, with axial osteosarcoma most likely. A partial cranial internal hemipelvectomy was performed, preserving the left pelvic limb and the caudal sacroiliac joint. Histopathology confirmed a poorly differentiated chondroblastic osteosarcoma with a mitotic count of 61 per 2.37 mm2. Surgical margins were clear, with a minimum 9-mm tumou
