Inflammatory and noninflammatory vascular disease causing hemobilia.

Typical angiographic finding of polyarteritis nodosa (PAN) shows aneurysms in small- and medium-size arteries, which can assist in diagnosis. Mimics of vasculitis share similar angiographic and clinical manifestations with PAN, making diagnosis confusing. We report 2 patients admitted with hemobilia, one of whom had vasculitis and case of the other mimicked vasculitis. The first patient was an 18-year-old man with epigastric pain. On serial workup, hemobilia with multiple hepatic artery aneurysms was diagnosed and embolized. Later, PAN was confirmed on pathologic examination of gallbladder afte

Atraumatic hemobilia arising from a cirrhotic liver.

Biliary hemorrhage may occur in a variety of clinical settings, but spontaneous hemobilia has not been reported from a cirrhotic liver. We describe a case of major hepatic hemobilia in a patient with cirrhosis and no history of trauma. A 50-year-old woman had abdominal pain, melena, and profound anemia. An extensive workup did not show the site of bleeding but did show a mass in the gallbladder. Cholecystectomy was performed, and at operation the patient was found to have cirrhosis and portal hypertension. The gallbladder "mass" was simply an organized clot, and hemorrhage recurred postoperati

Hemobilia-a rare complication after laparoscopic cholecystectomy.

BACKGROUND: Biliary bleeding is a condition reported by Sandblom as hemobilia. The most common cause of hemobilia is iatrogenicity. But it has also been reported as a rare complication after laparoscopic cholecystectomy (LC). CASE PRESENTATION: A man in his 60s underwent a LC. He was taking a direct Xa inhibitor for paroxysmal atrial fibrillation (pAf) and had a history of thrombectomy. There was variation in the bifurcation of the hepatic artery and cystic artery. The right hepatic artery branches from the common hepatic artery by itself, and the cystic artery is double. He complained of rig

Idiopathic cystic artery aneurysm complicated with hemobilia.

Aneurysm of the cystic artery is not common, and it is a rare cause of hemobilia. Most of reported cases are pseudoaneurysms resulting from either an inflammatory process in the abdomen or abdominal trauma. We report a healthy individual who developed hemobilia associated with cystic artery aneurysm. The patient was managed with cholecystectomy and concomitant aneurysm repair. Visceral artery aneurysms are rare and can rupture with potentially grave outcome due to excessive bleeding. Angiographic embolization is a common method of treatment for visceral artery aneurysms. Open cholecystectomy a

[Acute pancreatitis related to hemobilia complicating hepatocarcinoma].

Hemobilia is an rare cause of acute pancreatitis. The most frequent causes are iatrogenic trauma (percutaneous liver biopsy) and hepatic artery aneurysm. To our knowledge, this is the second published case of acute pancreatitis related to hemobilia secondary to hepatocarcinoma complicated cirrhosis in a patient treated with anticoagulants for a mechanical valvular aortic prosthesis. The clinical picture included acute epigastric pain, fever and jaundice. Increased amylase and lipase serum activities, and abdominal CT data confirmed the diagnosis of acute pancreatitis. Gallstone induced acute p

Hepatobiliary scanning in hemobilia-induced acute cholecystitis.

Radionuclide scanning of the hepatobiliary tree is highly accurate for the detection of patients with acute cholecystitis. Hemobilia, a rare complication of percutaneous liver biopsy, may result in blood clots within the biliary canaliculi. Such clots, like gallstones, may occlude the ducts and produce a clinical syndrome indistinguishable from acute cholecystitis. A patient with acute cholecystitis resulting from hemobilia following percutaneous biopsy of the liver is described. The diagnosis of acute cholecystitis secondary to blood clots was made with technetium 99mTc Iprofenin (Pipida [Sn]

Minor hemobilia. Clinical significance and pathophysiological background.

Profuse hemorrhage into the biliary tract--major hemobilia,--is an alarming condition which attracts much attention. Minor hemobilia, often caused by gallstones or operative injury, is much more frequent, yet often neglected. Clinical observations indicate that minor hemobilia is not always an innocent condition with the blood remaining fluid and unobtrusively flowing into the intestine. Examples are given where clots from occult hemobilia have caused diagnostic errors or obstructed the bile flow, thus imitating gallstones for which they may be mistaken. Experiments have been performed to eluc

Acute hemorrhagic cholecystitis as a rare cause of hemobilia.

Hemobilia is an unusual type of gastrointestinal bleeding most frequently due to iatrogenic injury, trauma, or neoplasia. Acute cholecystitis as a cause of hemobilia is rare. We present the case study of a patient with bleeding from eroded gallbladder mucosa in the setting of severe calculous cholecystitis. The hemorrhagic episode was preceded by acute ERCP due to obstructive icterus with extraction of the calculi, followed by the development of severe acute pancreatitis. These factors initially misled the diagnosis. The bleeding was not hemodynamically important and routine diagnostic methods

Upper Gastrointestinal Bleeding With Hemobilia Caused by Gallstones.

BACKGROUND: Hemobilia refers to bleeding in the biliary tract, commonly due to iatrogenic, traumatic, and neoplastic causes. It is a rare source of upper gastrointestinal hemorrhage, but it can be severe and fatal. However, gallstones account for 5%-15% of hemobilia cases. CASE REPORT: A 60-year-old woman with diabetes mellitus and chronic kidney disease visited the emergency department with complaints of epigastric pain and vomiting of coffee ground-like content for 2 days. Physical examination revealed epigastric tenderness and hyperactive bowel sounds. Laboratory tests showed anemia with a

Hemobilia due to a cystic artery pseudoaneurysm on ultrasound.

The present paper describes a case of hemobilia in a woman with a cystic artery pseudoaneurysm. The pseudoaneurysm could be seen with ultrasound, Doppler sonography and CT angiography. In our case, Doppler sonography was the most useful technique for diagnosis, revealing the turbulent forward and backwards flow within the gallbladder, representing the focally dilated artery. This was later confirmed by CT angiography. A recent bleeding site was found on the cholecystectomy specimen.

[Dieulafoy's lesion of the gallbladder as a cause of hemobilia with hemoperitoneum].

Dieulafoy's lesion is an uncommon vascular anomaly causing massive gastrointestinal hemorrhage, characterized by bleeding from an arteriole that protrudes through a tiny mucosal defect. Although Dieulafoy's lesion was initially described in the stomach and upper gastrointestinal tract, cases have also been reported in the colon, rectum and anal canal. We report the first case of Dieulafoy's lesion in the gallbladder, which presented as massive peritoneal bleeding. We describe the clinical and pathological characteristics of this unusual entity, as well as the approach to its diagnosis and trea

[Traumatic hemobilia: a case report and literature review].

Traumatic hemobilia consists of hemorrhage into the biliary tract as a result of abdominal trauma. The classical triad of biliary colic, jaundice and upper gastrointestinal bleeding is not a constant finding, and clinically silent hemobilia has been reported. The treatment of choice is selective embolization, but spontaneous cessation of bleeding can occur, especially in mild forms. We report a case of occult traumatic hemobilia in which the diagnosis was suggested by transitory changes in ultrasonography and hepatic biochemistry.

A patient with undifferentiated carcinoma of gallbladder presenting with hemobilia.

Hemobilia is relatively rare among hemorrhages in the digestive tract, and hemobilia caused by tumors of the biliary tract is particularly rare. We treated a 74-year-old-man with undifferentiated carcinoma of the gallbladder presenting with hemobilia. During hospitalization for neurogenic bladder at the Department of Urology, he showed progressive anemia. Since hemorrhage in the digestive tract was suspected, endoscopy of the upper gastrointestinal tract was performed, and bleeding from the papilla of Vater was observed. On ultrasound examination, findings were indicative of cholecystic cancer

Metastatic melanoma presenting as obstructive jaundice with hemobilia.

Metastasis of melanoma to gastrointestinal organs are common and have been well documented. Metastasis to the common bile duct with a clinical presentation of obstructive jaundice and hemobilia, as encountered in our patient, is a heretofore unreported clinical problem. This diagnosis was not suspected before operation but was established after a successful pancreaticoduodenectomy. The patient had an uneventful recovery and has shown no signs of recurrence. The causes of obstructive jaundice and hemobilia have been presented and discussed.

Traumatic hemobilia: a complication of percutaneous liver biopsy.

Two patients with hemobilia are presented. The first patient, with alcoholic liver disease, had a percutaneous liver biopsy. Subsequently he developed jaundice, with an enlarged tender gallbladder, biliary colic, and gastrointestinal bleeding. Hemobilia was demonstrated by superselective hepatic angiography and bleeding was stopped by intraarterial infusion of epinephrine and propranolol. The second patient, with primary biliary cirrhosis at an advanced stage, had a percutaneous liver biopsy followed by gastrointestinal bleeding, severe abdominal pain, and finally death. In both cases hemobili

[Hemobilia and hemoperitoneum (discussion of an unusual clinical case)].

A rare case of haemoperitoneum from rupture of haemocholecyst is described. The origin of the haemorrhage, on the basis of the case history, clinical, intra-operative and anatomo-pathological data, was in all probability intrahepatic. In addition to discussion of the incidence of this morbid event, the article refers to the most frequent causes of haemobilia, the physiopathological mechanisms supporting it, the symptomatological triad characterising it even when aspecific, and the available diagnostic possibilities.
