Hepatic subsegmentectomy with segmental hepatic vein sacrifice.

There is no clinical disorder in partial Budd-Chiari syndrome or in a major hepatic vein ligation in hepatic trauma. When considering these findings, it is significant to investigate hepatic subsegmentectomies in which a major hepatic vein is sacrificed. We performed such hepatic subsegmentectomies in nine cases of hepatocellular carcinoma. With the sacrifice of the right hepatic vein, S7, S8 resection was done in three patients, S7 resection in two patients, S8 resection in one patient, and S5 resection in one patient. With the sacrifice of the middle hepatic vein, S8 resection was done in tw

Hepatic vein reconstruction for resection of hepatic tumors.

SUMMARY BACKGROUND DATA: Involvement of the hepatic veins requiring reconstruction has traditionally been considered a contraindication to resection for advanced tumors of the liver because the surgical risks are high and the long-term prognosis poor. Recent advances in liver surgery gleaned from split and live donor liver transplantation that necessitate hepatic vein reconstruction can be applied to hepatic resection in some cases. METHODS: Sixteen patients who underwent hepatic resection requiring hepatic vein reconstruction from 1996-2001 were reviewed. The mean age was 43 years (range 2-6…

A computational model-based study on the exchangeability of hepatic venous pressure gradients measured in multiple hepatic veins.

Hepatic venous pressure gradient (HVPG) is a hemodynamic index widely used for evaluating the severity of portal hypertension. Theoretically, HVPG can be measured in any of the three major (i.e., right, middle and left) hepatic veins (HVs); however, it remains unclear whether HVPGs measured in different HVs are exchangeable, and if not, what factors cause inter-HV HVPG differences? In consideration of the potential limitations of invasive in vivo measurements, we employed a computational model implemented in conjunction with stochastic parameter sampling to simulate and compare HVPG measuremen…

Accessory hepatic vein recanalization: a viable approach in hepatic vein outflow tract obstruction.

OBJECTIVE: To evaluate the mid- and long-term outcome of recanalization of accessory hepatic vein (AHV) secondary to hepatic venous outflow obstruction and to compare the mid- and long-term outcome and technical efficacy and safety of AHV recanalization with native hepatic vein (HV) recanalization in hepatic vein outflow tract obstruction (HVOTO) patients. METHODS: This retrospective observational study, conducted at a single-centre, evaluated the mid- and long-term outcomes of AHV recanalization compared to native HV recanalization in 170 HVOTO patients treated between January 2013 and Octob

Urgent hepatic to azygous vein connection for thrombosis of hepatic vein to pulmonary artery conduit.

We report on an emergent use of hepatic-to-azygous vein connection using a 10-mm polytetrafluoroethylene graft in a 6-year-old girl who developed acute thrombosis of an extracardiac graft conduit from the hepatic veins to the pulmonary arteries. She had left atrial isomerism, interrupted inferior vena cava with azygous continuation, and had undergone surgical creation of bilateral superior cavopulmonary connections 5 years earlier. The emergency procedure to create hepatic-to-azygous connection resulted in successful salvage of the patient who was subsequently able to maintain stable oxygen sa…

Ultrasonographic Visualization of Accessory Hepatic Veins and Their Lesions in Budd-Chiari Syndrome.

The aim of this study was to investigate the ultrasonographic features of accessory hepatic veins (AHVs) and their lesions in Budd-Chiari syndrome (BCS). Three hundred patients with BCS were examined by ultrasonography with multifrequency (3-6 MHz) convex transducers. Sonography was performed 1 to 2 wk before digital subtraction angiography and computed tomography angiography or magnetic resonance imaging. Using sonograms, we evaluated the number, course, diameter, orifice, lesions and hemodynamics of patent and obstructed AHVs. Ultrasonography was superior to digital subtraction angiography, …

Sonographic classification of draining pathways of obstructed hepatic veins in Budd-Chiari syndrome.

PURPOSE: To describe and propose a sonographic classification of the blood-draining pathways of obstructed hepatic veins in Budd-Chiari syndrome. METHODS: This retrospective study included 206 patients with hepatic vein obstructions who underwent sonographic examination. We evaluated the afflicted hepatic veins, as well as the course, orifice, blood flow direction of draining veins, and communicating branches. Results were classified and compared with digital subtraction angiography and computed tomography angiography. RESULTS: Of 618 hepatic veins in 206 patients, 542 were obstructed. The b…

On the History of Hepatic Vein Catheterization.

Hepatic vein catheterization (HVC) in man was first performed 80 years ago in the wake of right-sided heart catheterization. Several new methods, recognitions and concepts followed, especially the indirect Fick-method for determination of splanchnic blood flow. Five years passed before pressure measurements were performed and routine HVCs were done in liver patients. Later came splanchnic kinetics of test substances, dyes, metabolites, hormones and bioactive material and counter current wedged and balloon occlusion pressure. HVC has been a prerequisite for our understanding of the pathogenesis

Simultaneous embolization of the right portal and hepatic veins before intrahepatic cholangiocarcinoma resection.

Major liver resections require extensive margins. Occasionally, insufficient parenchyma is available after surgery to maintain liver function. In such cases, vascular embolization in the affected lobe is necessary to induce contralateral lobe hypertrophy. We present a case of embolization of the right portal and hepatic veins prior to intrahepatic cholangiocarcinoma resection. Embolization was performed because of insufficient residual parenchyma on imaging studies. The patient recovered well with no signs of liver failure, and remains in remission at 3 years postoperatively. Knowledge of the …

[Dopplerography of the large hepatic veins in the diagnosis of tricuspid valve insufficiency].

During pulse dopplerography of the large hepatic veins in patients with tricuspid valve failure, the differences in the shape of the spectrum of Doppler's frequencies were revealed as dependent on cardiac rhythm. In sinus rhythm, the curve of the systolic flow is recordable beneath the baseline, in atrial fibrillation, over the baseline. In scanning of the large hepatic veins in patients with tricuspid valve failure, the shape of the curves of the spectrum of Doppler's frequencies coincides with the shape of the curves of liver pulsation. Tricuspid valve failure in sinus rhythm leads to a decr…

Evaluation of inferior right hepatic vein-preserving hepatectomy with resection of the superior right hepatic vein.

BACKGROUND/AIMS: If the superior right hepatic vein (SRHV) is resected, segment 6 will be drained by the inferior right hepatic vein (IRHV) and congestion will not occur. However, early postoperative liver function and long-term regeneration of segment 6 after IRHV-preserving hepatectomy remains unclear. METHODOLOGY: Three patients undergoing IRHV-preserving hepatectomy were examined. One (case 1) had liver metastasis from sigmoid colon cancer and two (cases 2 and 3) had hepatocellular carcinoma. Resection of segments 7 and 8 along with the SRHV was performed in cases 1 and 2. Anterior sectio

Reconstruction of the hepatic vein in reduced size hepatic transplantation.

Reconstruction of the hepatic vein (HV) is not required in size-matched orthotopic liver transplantation (OLT) because the vena cava (VC) is replaced. In reduced size OLT, used for providing small livers for children, the HV is often implanted directly. Grafts obtained from a split liver in which the right lobe is used for a second recipient or from a live donor must be implanted without the VC. To evaluate the occurrence of outflow complications and their prevention, we have reviewed our experience with 72 left sided reduced grafts in children. Between July 1985 and November 1990, 93 reduced

Correlation between hepatic venous pressure gradient and portal venous pressure gradient in hepatitis B cirrhosis with different hepatic veins anatomy.

PURPOSE: The hepatic venous pressure gradient (HVPG) has been employed as the gold standard for indicating the portal venous pressure gradient (PPG) in the diagnosis of portal hypertension (PHT). However, little has been reported on whether the HVPG can accurately estimate the PPG in patients with hepatic vein collateral shunts. We aimed to explore the correlation between the HVPG and the PPG in hepatitis B cirrhosis patients with different hepatic vein anatomies. METHODS: A total of 461 hepatitis B cirrhosis patients with portal hypertension (PHT) who were treated with a transjugular intrahe

Transjugular intrahepatic portosystemic shunt for Budd-Chiari syndrome with diffuse occlusion of hepatic veins.

Either acute or sub-acute Budd-Chiari syndrome (BCS) with diffuse occlusion of hepatic veins has a high mortality rate and remains challenging for clinical treatment. We aimed to evaluate the feasibility and safety of transjugular intrahepatic portosystemic shunt (TIPS) as a treatment for BCS with diffuse occlusion of hepatic veins. From January 2007 to December 2010, 100 patients were randomly recruited onto this study and 91 patients were treated with TIPS. 14 patients were defined as acute BCS group and 86 patients as sub-acute group. Patients with acute BCS had a significantly higher rate …
