Pain and convalescence following laparoscopic ventral hernia repair.

Severe pain is usual after laparoscopic ventral hernia repair (LVHR). Mesh fixation with titanium tacks may play a key role in the development of acute and chronic pain and alternative fixation methods should therefore be investigated. This PhD thesis was based on three studies and aimed too: 1) assess the intensity and impact of postoperative pain by detailed patient-reported description of pain and convalescence after LVHR (Study I), 2) evaluate the feasibility of fibrin sealant (FS) for mesh fixation in an experimental pig model (Study II), and 3) investigate FS vs. tacks for mesh fixation

Laparoscopic ventral hernia repair: an initial institutional experience.

BACKGROUND: Ventral and incisional hernias remain a problem for surgeons with reported recurrence rates of 25-50% for open repairs. Laparoscopic approaches offer several theoretical advantages over open repairs. MATERIALS AND METHODS: All patients undergoing a laparoscopic ventral hernia repair from April to December 2000 were prospectively entered in a database. Patients underwent repair with expanded polytetrafluoroethylene dual mesh. Full-thickness abdominal wall nonabsorbable sutures and 5-mm tacks were placed circumferentially. RESULTS: Of 32 patients, 15 underwent incisional repair, 13

[A watchful waiting strategy in ventral and inguinal hernias].

In male patients with asymptomatic or minimally symptomatic ventral and inguinal hernias, a watchful waiting strategy should be considered. Even though one third to two thirds of these patients will eventually undergo hernia repair, they may still benefit from a watchful waiting strategy, as hernia repair is associated with a range of complications, e.g. wound infection, haematoma, seroma, fistulas and chronic pain. Delay of hernia repair in these patients is not associated with any significant increase in morbidity or mortality, and the risk of incarceration is very low.

Repair of massive ventral hernias with "quilted" mesh.

INTRODUCTION: Prosthetic reinforcement is a critical component of hernia repair. For massive defects, mesh overlap is often limited by the dimensions of commercially available implants. In scenarios where larger mesh prosthetics are required for adequate reinforcement, it may be necessary to join several pieces of mesh together using non-absorbable suture. Here, we report our outcomes for abdominal wall reconstructions in which "quilted" mesh was utilized for fascial reinforcement. METHODS: Patients undergoing open incisional hernia repair utilizing posterior component separation and transver

Laparoscopic treatment of ventral hernias: prospective evaluation.

We describe 50 patients who recently underwent laparoscopic surgery. Early results, complications, and follow-up data were collected prospectively. Of 50 patients, 34 had an incisional hernia, whereas 16 had a primary defect. Three trocars were inserted. EndoShears or Ultracision was used for tissue manipulation. The prosthetic mesh used was an expanded polytetrafluoroethylene (ePTFE) mesh, inserted through the first trocar and fixed with a helicoidal stapler. Patients were followed-up in the outpatient clinic (mean, 14 months). Every operation was successfully completed, and mean operative ti

Laparoscopic Extraperitoneal Repair with Upfront Coring out of Hernia Defect for Ventral Hernia.

BACKGROUNDS AND OBJECTIVES: The enhanced-view totally extraperitoneal technique (eTEP) has gained popularity as a novel minimally invasive ventral hernia repair approach. However, this procedure becomes technically demanding once the view is no longer maintained, due to incidental pneumoperitoneum caused by peritoneal injury during the surgery. In an attempt to overcome this technical issue, we report laparoscopic extraperitoneal repair with upfront coring out of hernia defect (LERCO) where the intraperitoneal coring out of the hernia defect precedes the regular eTEP for the treatment of midli

Report of novel application of T-line hernia mesh in ventral hernia repair.

INTRODUCTION: Ventral hernia repair is one of the most common surgeries performed in the United States. Failure of hernia repairs can be attributed to sutures pulling through tissue or mesh (anchor point failure). T-Line Hernia Mesh is the first mesh designed to specifically prevent anchor point failure by distributing tension. This case study of two patients is the first clinical application of the novel T-Line Hernia Mesh. PRESENTATION OF CASE: Two separate patients presented with symptomatic ventral hernia secondary to previous laparotomy. Patient 1 is a fifty-five year-old male who underw

Ventral hernia with a skin-covered Silastic sheet for newborn infants with a diaphragmatic hernia.

The persistently high mortality rate for newborn infants with a congenital diaphragmatic hernia, which is symptomatic and treated in the first 24 hours of life, is due to multiple pulmonary and vascular factors. This demands the exclusion of any additional compromising elements. The effects of increased intra-abdominal pressure due to replacement of the intestine into the abdomen can be avoided by the formation of a ventral hernia. A large hernia can be produced best by suturing a sheet of Dacron reinforced Silastic to the fascia of a midline abdominal incision and covering it with skin flaps.

Negative Pressure Wound Therapy Prevents Hernia Recurrence in Simultaneous Ventral Hernia Repair and Panniculectomy.

UNLABELLED: Simultaneous ventral hernia repair with panniculectomy (VHR-PAN) is associated with a high rate of wound complications. Closed incision negative pressure wound therapy (ciNPWT) has been shown to lower complications in high-risk wounds. There is a debate in the literature as to whether ciNPWT is effective at preventing complications in VHR-PAN. The aim of our study was to evaluate if ciNPWT improves outcomes of VHR-PAN. METHODS: A retrospective review of patients who underwent VHR-PAN between 2009 and 2021 was conducted. Patients were divided into two groups: (1) those who received

Increasing hernia size requires higher GRIP values for a biomechanically stable ventral hernia repair.

BACKGROUND: Increasing hernia sizes lead to higher recurrence rates after ventral hernia repair. A better grip might reduce the failure rates. MATERIAL AND METHODS: A biomechanical model delivering dynamic intermittent strain (DIS) was used to assess grip values at various hernia orifices. The model consists of a water-filled aluminium cylinder covered with tissues derived from pig bellies which are punched with a central defect varying in diameter. DIS was applied mimicking coughs lasting for up to 2 s with peak pressures between 180 and 220 mmHg and a plateau phase of 0.1 s. Ventral hernia

Modified hernia grading scale to stratify surgical site occurrence after open ventral hernia repairs.

BACKGROUND: The lack of a universally accepted classification system for incisional hernia repair has led to inconsistent reporting of ventral hernia repairs, the inability to compare various series, and the lack of appropriate risk adjusted stratification systems to evaluate surgical outcomes. This study evaluates the Ventral Hernia Working Group's (VHWG) grading scale to accurately predict surgical site occurrence (SSO) after open ventral hernia repair. STUDY DESIGN: All open ventral hernia repairs performed at Case Medical Center were evaluated from a prospectively maintained database. Her

Biosynthetic Versus Synthetic Mesh in Oncologic Ventral Hernia Repair.

INTRODUCTION: Ventral hernia repair in oncologic patients is challenging due to poor tissue quality, immunosuppression, malnutrition, and prior operations. Limited data exists comparing the outcomes of biosynthetic (Poly-4-hydroxybutyrate) and synthetic mesh in this population. This study aims to assess the outcomes in patients who undergo hernia repair using one of two mesh types. METHODS: Patients undergoing ventral hernia repair by the Plastic and Reconstructive Surgery service and general surgery after oncologic resection from 2010 to 2023 were included, with patient records reviewed and

[Botox preceding ventral hernia repair: it's not about esthetics].

Ventral hernia is a common complication after laparotomy. The aim during ventral hernia repair is to close the abdomen through medialization of the rectus muscles. Particularly in patients with large ventral hernia, chronically retracted lateral muscles may preclude muscle medialization and therewith closure of the abdomen. A recent development in abdominal wall surgery is the injection of botulinum toxin (botox) in the lateral abdominal wall muscles a few weeks prior to surgery. These intramuscular injections cause a temporary partial paralysis resulting in elongation of the lateral muscles,

Robotic ventral hernia repair: a safe and durable approach.

BACKGROUND: Short-term success following robotic-assisted ventral hernia repair (RVHR) is well established; however, data describing outcomes after the first year are limited. In this study, we followed a cohort of patients with an average of 1.8 years of follow-up to demonstrate the durability of this technique and examine risk factors for recurrence. METHODS: A retrospective analysis of RVHR performed by a single surgeon from 2012 to 2016 was done. The technical approach for hernia repair consisted of tension-free primary fascial closure with placement of preperitoneal mesh when possible. T

Laparoscopic ventral hernia repair using only 5-mm ports.

BACKGROUND: The technique of laparoscopic ventral hernia repair has been evolving since it was first described over 20 years ago. We report a new technique where polyester mesh was back loaded through a 5-mm port site, coming into contact with the skin. This avoids the need for any 10-12-mm ports. METHODS: A prospective database of laparoscopic ventral hernia repairs was examined. A single surgeon performed 344 laparoscopic ventral hernia repairs using this technique over 60 months. Follow-up was conducted by both clinical and independent phone review. SURGICAL TECHNIQUE: Laparoscopic access

Laparoscopic ventral hernia repair is safe and cost effective.

BACKGROUND: Ventral hernia repair is increasingly performed by laparoscopic means since the introduction of dual-layer meshes. This study aimed to compare the early complications and cost effectiveness of open hernia repair with those associated with laparoscopic repair. METHODS: Open ventral hernia repair was performed for 92 consecutive patients using a Vypro mesh, followed by laparoscopic repair for 49 consecutive patients using a Parietene composite mesh. RESULTS: The rate of surgical-site infections was significantly higher with open ventral hernia repair (13 vs 1; p = 0.03). The median

Laparoscopic ventral and incisional hernia repair in 407 patients.

BACKGROUND: Recurrence rates after primary repair of ventral and incisional hernias range from 25% to 52%. Recurrence after open surgery is less likely if mesh is used, but the wide fascial dissection and required flap creation increase complication rates. Laparoscopic techniques offer an alternative. STUDY DESIGN: To assess the safety and efficacy of laparoscopic ventral and incisional herniorrhaphy, we reviewed the records of all our patients who underwent such a procedure from November 1993 to August 1999. A laparoscopic approach was attempted in all patients considered to require a mesh r
