UNLABELLED: In this case report we present a 2-year-old girl with the classical signs of the hyponatraemic hypertensive syndrome. She initially presented with a history of behavioural abnormalities and hyponatraemia (126 mmol/l) and her blood pressure was as high as 220/160 mmHg. After admission, somnolence developed. Intravenous anti-hypertensive therapy was started immediately. The hyponatraemia was treated with i.v. sodium supplementation. The cause of this syndrome proved to be fibromuscular dysplasia of the left renal artery. Finally, a left nephrectomy was performed. With this therapy, b

Posterior reversible encephalopathy syndrome (PRES) is a neurological syndrome characterized by acute encephalopathy due to different medical conditions. This syndrome may present with a wide spectrum of neurological symptoms including headache, disorders of consciousness, visual changes, seizures, and focal neurological deficits, in addition to nonspecific symptoms such as nausea and vomiting. Neuroimaging findings of bilateral cortical and subcortical brain edema involving the parieto-occipital regions are a hallmark of the disease. We present a case report of an eight-year-old boy who compl

A 14-year-old girl was admitted to the emergency department for repeated seizures with no fever. High blood pressure at admission was ascribed to the status epilepticus. Seizure recurrence promoted transfer to the paediatric intensive care unit. The MRI visualised bilateral, subcortical, white-matter lesions in the parietal and occipital lobes suggesting posterior reversible encephalopathy syndrome (PRES). Continuous nicardipine infusion corrected the hypertension and neurological manifestations within a few hours. The diagnostic workup showed right renal artery stenosis. Blood pressure return

BACKGROUND: Reversible posterior leukoencephalopathy syndrome (RPLS) is characterised by clinical neurological features of sudden onset and brain MRI findings such as T2/Flair white matter hyperintensities. RPLS can occur in autoimmune diseases, and rarely in systemic vasculitis. We report a case of RPLS in a woman presenting granulomatosis with polyangiitis (Wegener's granulomatosis).
PATIENTS AND METHODS: A 22-year-old female patient was treated with methylprednisolone pulses for granulomatosis with polyangiitis and neurological impairment. A few hours after the second pulse, the patient ha

Hyperbaric pressure induces hyperexcitability, tremor, and convulsions in intact animals by mechanisms that are not understood. High pressure induces spontaneous electrical discharges in the spinal ventral roots which we call "tremor-associated activity" (TAA). This study examined the nature of TAA, its likely origins, and its possible contribution in the cervical roots to respiratory difficulties. Activity was recorded extracellularly from the cut cervical ventral roots C1 and C5 in an in vitro brainstem-spinal cord preparation from newborn rats. Under hyperbaric conditions spontaneous TAA wa

Posterior reversible encephalopathy syndrome (PRES) is a clinico-radiological syndrome characterized by neurological disorder and distinctive neuroimaging findings of vasogenic edema, most commonly involving the parieto-occipital regions. Blood pressure fluctuation is a common complaint in patients with Parkinson's disease (PD), which might be explained by autonomic nervous system dysfunction, which impairs the body's ability to automatically regulate blood pressure. Additionally, PD medications can sometimes contribute to these fluctuations, leading to episodes of both low and, less commonly,

Helium pressure of more than 20 bar is a well known cause of neuroexcitatory changes, referred to as the high pressure neurological syndrome. In rodents, symptoms include myoclonia and locomotor activity. In the present study, we used the 5-HT2A receptor antagonist ketanserin and the 5-HT2C receptor agonist m-CPP to investigate the role of central 5-HT2 receptors in the occurrence of these symptoms. Results suggest that 5-HT2A receptors and 5-HT2C receptors would play a crucial role in the development of myoclonia and locomotor activity, respectively. The selectivity of the drugs used, and the

Nitrous oxide is used as an anaesthetic and analgesic agent in the medical setting and is known to cause raised intracranial pressure. The use of nitrous oxide recreationally for the drug's euphoric and relaxant properties has been linked to multiple neurological and psychiatric sequelae including neuropathy, myelopathy, and psychosis. We describe a case of a young person who declared heavy nitrous oxide use resulting in vision-threatening papilloedema secondary to raised intracranial pressure. He underwent emergency lumbar drainage alongside high-dose acetazolamide and parenteral vitamin B12

Changes in cerebral blood flow (CBF) and regulation of cerebral circulation occur at high altitude (HA). However, the changes in CBF and their associations with neurological syndrome sequelae and blood pressure after subjects return to the lowlands remain unclear. In this study, the subjects were 23 college students who were teaching at an altitude of 4300 m for 30 days. These subjects were studied before reaching the HA (Test 1), one week after returning to the lowlands (Test 2), and three months after returning to the lowlands (Test 3). Symptom scores for de-acclimatization syndrome were eva

Von Hippel-Lindau (VHL) disease is an autosomal, dominant inherited tumour syndrome with an estimated prevalence of 2-3 per 100,000 persons. A germline mutation in the VHL gene predisposes carriers to tumours in multiple organs. These tumours may include haemangioblastoma in the retina and central nervous system (CNS), renal cell carcinoma, phaeochromocytoma, islet cell tumours of the pancreas, and endolymphatic sac tumours, as well as cysts and cystadenoma in the kidney, pancreas, epididymis and broad ligament. Penetrance of VHL disease is high, most carriers of a VHL germline mutation develo

Low- or very-low-pressure hydrocephalus is a serious and rare phenomenon, which is becoming better known since it was first described in 1994 by Pang and Altschuler. Forced drainage at negative pressures can, in most cases, restore the ventricles to their original size, thus achieving neurological recovery. We present six new cases that suffered this syndrome from 2015 to 2020: two of them after medulloblastoma surgery; a third one as a consequence of a severe head trauma that required bifrontal craniectomy; another one after craniopharyngioma surgery; a fifth one with leptomeningeal glioneuro

INTRODUCTION: Reversible Posterior Leukoencephalopathy Syndrome was introduced into clinical practice in 1996 in order to describe unique syndrome, clinically expressed during hypertensive and uremic encephalopathy, eclampsia and during immunosuppressive therapy [1]. First clinical investigations showed that leucoencephalopathy is major characteristic of the syndrome, but further investigations showed no significant destruction in white cerebral tissue [2, 3, 4]. In majority of cases changes are localise in posterior irrigation area of the brain and in the most severe cases anterior region is

INTRODUCTION: "Man-in-the-barrel syndrome" is a neurological phenotype with brachial diplegia, normal sensation, and preserved motor function of the lower limb. It has been described in various neuropathological conditions affecting the cerebral hemispheres, pons, upper spinal cord, and peripheral neurons. Severe hypotension leading to watershed infarctions leading to this phenotype has been reported. We describe the first case of "man-in-the-barrel syndrome" in a patient with a precipitous drop in blood pressure following oral antihypertensive medications. Case Presentation. A 75-year-old Sri

Posterior reversible encephalopathy syndrome (PRES) is a potentially life-threatening but reversible neurological disorder. Commonly seen in the intensive care unit (ICU), PRES can arise from uncontrolled hypertension, renal failure, or the use of immunosuppressants and cytotoxic drugs. Erythropoietin (EPO) analogs help manage anemia in chronic kidney disease, but their hypertensive effects may lead to PRES. We report a patient with end-stage renal disease presenting with PRES that occurred four months after commencing darbepoetin alfa and closely correlated with darbepoetin use. Brain MRI con

Posterior reversible encephalopathy syndrome (PRES) is a neurologic condition defined by symptoms and imaging findings secondary to vasogenic edema in the brain. Even though not all hypertensive individuals will progress to PRES, high blood pressure is the most frequent risk factor associated with the condition. The pathophysiology of PRES is not clearly understood, but the most accepted proposed mechanism focuses on the brain's inability to regulate cerebral blood flow through constriction or dilation of vessels during extreme blood pressure. This case report is about a 38-year-old male patie

Chronic exertional compartment syndrome (CECS) is an exercise-related cause of lower extremity pain characterized by reproducible pain, tightness, cramping, and neurologic symptoms that improve with rest. Diagnosis may be delayed because chronic lower leg pain has a broad differential diagnosis, and currently used diagnostic strategies remain imperfect. We report the case of a young woman who began experiencing bilateral lower extremity pain during high school sports that progressed to pain with prolonged standing. Treadmill-provoked bilateral four-compartment pressure testing reproduced sever

BACKGROUND: Posterior reversible encephalopathy syndrome (PRES) is an acute or subacute neurological syndrome with clinical manifestations including headache, vision loss, altered mental status, and seizures. Magnetic resonance imaging(MRI) can assist in clarifying the diagnosis. Systemic lupus erythematosus (SLE) is an autoimmune disease, and the occurrence of PRES is infrequent, especially in children with SLE.
CASE DESCRIPTION: Our patient was initially diagnosed with juvenile systemic lupus erythematosus involving the skin, joints, and kidneys. The patient was treated with glucocorticoids

PRES is a neuroclinical and radiological syndrome that results from treatment with calcineurin inhibitor immunosuppressives. Severe hypertension is commonly present, but some patients may be normotensive. We report herein two children who received liver transplants, as treatment for biliary atresia in the first case and for Alagille's syndrome in the second one. In the early postoperative, both patients presented hypertension and seizures. In both cases, the image findings suggested the diagnosis of PRES. The CT scan showed alterations in the posterior area of the brain, and brain MRI demonstr

Valproic acid (VPA) and its salts (sodium calcium magnesium and orotic) are psychotropic drugs that are widely used in neurology and psychiatry. The long-term use of VPA increases the risk of developing adverse drug reactions (ADRs), among which metabolic syndrome (MetS) plays a special role. MetS belongs to a cluster of metabolic conditions such as abdominal obesity, high blood pressure, high blood glucose, high serum triglycerides, and low serum high-density lipoprotein. Valproate-induced MetS (VPA-MetS) is a common ADR that needs an updated multidisciplinary approach to its prevention and d
