Ischemia of the lower extremity after total hip replacement.

Following total hip replacement, three patients had early and one had late ischemia of the ipsilateral extremity. Three required vascular surgery and one, a lumbar sympathectomy for relief of pain at rest. In each instance there had been multiple previous procedures on the same hip resulting in extensive scarring, shortening, flexion contracture, or fusion. The ischemia after total hip replacement was probably the result of interruption of critical collateral circulation about the hip or of traction on the femoral vessels tethered by scar when the short limb was lengthened or when the hip cont

A new test for estimating iliotibial band contracture.

Contracture of the iliotibial band leading to flexion and abduction deformity at the hip is common in residual paralysis after polio. Ober's test has been used to detect this, but it is unreliable and cannot determine the degree of contracture. We describe a new test which quantifies this contracture and can be used for comparative purposes.

Hip range of motion and association with injury in female professional tennis players.

BACKGROUND: Adequate hip range of motion is required for the transfer of energy from the lower to the upper extremity along the kinetic chain. Repetitive rotational stresses in the lower extremities during tennis may lead to sport-specific range of motion adaptations, which may increase the risk of injury to other joints along the kinetic chain. PURPOSE: To assess whether such range of motion adaptations occur in the hip, and if so, to identify whether they are associated with injury. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A total of 125 female professional tenn

Walking ability in patients with arthrogryposis multiplex congenita.

BACKGROUND: Arthrogryposis multiplex congenita (AMC) is a multiple joint condition which affects both lower and upper extremities and thus affects ambulation. Multiple surgeries are needed to correct limb deformity in order to promote walking. The objective of this study is to identify the most critical residual deformity that diminishes the ambulatory status. MATERIALS AND METHODS: 51 patients were included in this study, 14 patients were nonambulatory. The mean age at first surgery was 4.1 years (range 2-16 years). The mean length of followup was 44.0 months (range 22-168 months). Type of p

Natural history of the dislocated hip in spastic cerebral palsy.

A review was made of 88 adult institutionalized patients with spastic cerebral palsy and contractural deformity of the hips. 21 were untreated for dislocated hip, and 11 of these suffered from hip pain. The degree of pain was directly related to neurological maturity and to the coexistence of athetosis and spasticity. Decubitus ulcers and perineal care problems were more associated with contractures than with dislocation alone. It is concluded that dislocation and subluxation should be prevented by surgical means, but that surgical treatment of the already dislocated hip should be reserved for

Lower extremity joint contracture according to ambulatory status in children with Duchenne muscular dystrophy.

BACKGROUND: Lower extremity joint contractures have negative effects on gait in children with Duchenne muscular dystrophy (DMD). Thus, contracture prevention is essential for maintaining a patient's functional ability and an acceptable quality of life. This study investigated hip flexion (HF), knee flexion (KF), and ankle joint plantar flexion (APF) contractures among male patients with DMD, based on the patients' ambulatory status. Differences in major joint contractures, based on passive stretching exercise participation, were also investigated. METHODS: A total of 128 boys with DMD, follow

Factors associated with internal hip rotation gait in patients with cerebral palsy.

Internal rotation gait is common in children with cerebral palsy. Factors thought to contribute include femoral anteversion, hip flexor tightness, imbalance of hip rotators, and hamstring and adductor tightness. The exact cause of internal rotation must be defined before contemplating surgery. We investigated the prevalence of internal hip rotation and associated factors, which are considered to influence this walking pattern, in patients with cerebral palsy. Gait laboratory data of 222 patients with cerebral palsy were studied retrospectively. Two groups were selected; those with maximum dyna

Fitting transtibial and transfemoral prostheses in persons with a severe flexion contracture: problems and solutions - a systematic review.

PURPOSE: In persons with a hip or knee flexion contracture ≥25°, fitting a prosthesis is said to be difficult. This systematic review aims to assess the evidence for fitting of a prosthesis in persons with a severe contracture (≥25°) after a lower limb amputation. METHOD: PubMed, Embase, Scopus, CINAHL, and Orthotics & Prosthetics Virtual Library databases were searched from inception to December 2019, using database specific search terms related to amputation, prosthesis, and contracture. Reference lists of included studies were checked for relevant studies. Quality of the included studies w

Twenty-year follow-up of hip problems in arthrogryposis multiplex congenita.

Nineteen patients with arthrogryposis (38 hips) with significant hip problems were reviewed (13 dislocations, 9 subluxations, 16 contractures). The average follow-up was 20 years, with 15 patients reaching skeletal maturity. All patients had minimal or no pain. Thirteen of the 19 patients were community walkers. The dislocation group in general had more stiffness of the hip joint than the subluxation and the contracture groups. However, the long-term functional results were comparable among these three groups. Closed treatment always failed in treating dislocation of hips in arthrogryposis. Op

[Evaluation of treatment of the hip in arthrogryposis multiplex congenita].

In a retrospective review of 77 patients with arthrogryposis multiplex congenita, 21 showed involvement of the hip (28%). The authors studied 18 cases with hip dislocation (11 bilateral and seven unilateral) and three with hip contracture. There were 12 female and nine male patients ranging their age from one month to 14 years. The follow-up in this series lasted from two to 23 years. The treatment was conservative or surgical. The contractures were treated by manipulation and abduction orthosis in one patient and surgically by multiple hip tenotomy in two. The dislocation was treated by abduc

Recent observations in the biomechanical etiology of so-called idiopathic scoliosis. New classification of spinal deformity--I-st, II-nd and III-rd etiopathological groups.

The article examines the biomechanical etiology of so-called idiopathic scoliosis (AIS). It describes I-st, II-nd and III-rd etiopathological groups (epg) of spine deformity which were developed during the years 2001/2004/2005. All children with so-called idiopathic scoliosis had an abduction contracture of the right hip, often connected with a flexion and external rotation contracture. In other cases we found only limited range of adduction of the right hip in comparison to the left hip. We maintain that children with this real abduction contracture of the right hip constitute the first etiop

[Lower extremity deformities as an obstacle in rehabilitation of meningomyelocele patients--pathogenesis and principles of treatment].

The fate of 89 patients with meningomyelocele operated at the Institute of Orthopedics and Rehabilitation in Poznań between 1970 and 1989 due to paretic deformities of lower extremities has been traced by the authors. Deformities prevented nursing, standing or ambulating; their type and results of treatment have been related to the level of neurosegmental lesion. Modified Sharrard's classification served to group the patients. The level of lesion established during lower extremities muscles testing has been verified after neurological examination supplemented with electrophysiological tests: s

Combination of bexarotene and psoralen-UVA therapy in a patient with Mycosis fungoides.

In congenital pelvic obliquity, there is a restricted abduction in one hip and an abduction contracture in the other. This association has been considered as a cause of acetabular dysplasia with progressive subluxation of the hip opposite to the abduction contracture. The clinical and radiological study of 120 children with an average follow-up of 3 years and 3 months permitted to demonstrate that two different types exist: the simple types (93 cases) without dysplasia, which never developed progressive subluxation and must not be treated, and the pelvic obliquities with dysplasia-subluxation-

Influence of different degrees of bilateral emulated contractures at the triceps surae on gait kinematics: The difference between gastrocnemius and soleus.

INTRODUCTION: Ankle plantarflexion contracture results from a permanent shortening of the muscle-tendon complex. It often leads to gait alterations. The objective of this study was to compare the kinematic adaptations of different degrees of contractures and between isolated bilateral gastrocnemius and soleus emulated contractures using an exoskeleton. METHODS: Eight combinations of contractures were emulated bilaterally on 10 asymptomatic participants using an exoskeleton that was able to emulate different degrees of contracture of gastrocnemius (biarticular muscle) and soleus (monoarticular

The bent spine syndrome: myopathy + biomechanics = symptoms.

BACKGROUND CONTEXT: The bent spine syndrome, which mimics spinal stenosis, is thought to be a focal paraspinal myopathy, but because paraspinal fatigue with ambulation is not a feature of more severe myopathies, the cause of symptoms is not clear. PURPOSE: To evaluate electromyographic and biomechanical aspects of the bent spine syndrome. STUDY DESIGN/SETTING: University spine clinic. METHODS: A patient with severe disability from the bent spine syndrome was compared with a fortuitously discovered asymptomatic research subject with the syndrome, in terms of physical examination, magnetic re

Changes in the range of motion of the lower limb joints during extensive tibial lengthening in achondroplasia.

Increase in the magnitude of lengthening during the limb lengthening procedure involves a high risk of decreased range of motion (ROM) in adjacent joints. Even though patients with achondroplasia can tolerate a relatively larger amount of lengthening owing to its inherent soft-tissue laxity, they often exhibit significant joint contractures during extensive lengthening. In the present study, we evaluated temporal changes in the ROM of the hip, knee, and ankle joints throughout the treatment period in 12 limbs of six patients with achondroplasia who had undergone extensive tibial lengthening. Feasibility and reliability of using an exoskeleton to emulate muscle contractures during walking.

Contracture is a permanent shortening of the muscle-tendon-ligament complex that limits joint mobility. Contracture is involved in many diseases (cerebral palsy, stroke, etc.) and can impair walking and other activities of daily living. The purpose of this study was to quantify the reliability of an exoskeleton designed to emulate lower limb muscle contractures unilaterally and bilaterally during walking. An exoskeleton was built according to the following design criteria: adjustable to different morphologies; respect of the principal lines of muscular actions; placement of reflective markers

[Preliminary treatment of congenital hip dislocation using physical therapy based on neurophysiology].

Adductor tightness is a typical symptom in CDH. In cases of subluxation and complete dislocation even a rigid abduction contracture can occur, preventing reduction and favouring redislocation. The contracture of the hip joint is the result of neurophysiological disorders, which can be treated by an exercise program on a neurophysiological basis. 62 children with 73 severely contracted and 5 hypotonic hip dislocations have been treated by the so-called "Vojta-program" before reduction. In a prospective study over 5 years this kind of treatment has been very successful: the incidence of necrosis

Type IIB Spino-Pelvic Imbalance Is an Independent Risk Factor for Flexion Contracture after Total Knee Arthroplasty.

BACKGROUND: Sagittal imbalance leads to adjustments in the pelvis, hips, and knees to keep a level gaze with minimal energy use. People who have a stiff spine adjust their posture by extending their hips, bending their knees, and tilting their pelvis back. This study hypothesized that type IIB spino-pelvic imbalance may influence the occurrence of flexion contracture after total knee arthroplasty (TKA). METHODS: We retrospectively evaluated all patients who received a robotic-assisted TKA between 2019 and 2024 at a single center. INCLUSION CRITERIA: patients who had a postoperative range of
