Psychiatric comorbidities in hoarding disorder: a structured clinical interview study.

OBJECTIVE: Hoarding disorder (HD) is a psychiatric condition characterized by persistent difficulty in discarding possessions, leading to significant clutter and functional impairment. HD is often comorbid with other psychiatric disorders, but previous findings of these comorbidities are inconsistent due to limited structured assessments. This study investigates the psychiatric comorbidities in individuals with HD using structured, clinician-administered interviews. METHODS: Between 2016 and 2022, 68 individuals diagnosed with HD were recruited from an outpatient clinic at Kyushu University H

Hoarding Disorder: The Current Evidence in Conceptualization, Intervention, and Evaluation.

Hoarding disorder is characterized by difficulty parting with possessions due to strong urges to save the items, leading to the excessive accumulation of items. High clutter levels result in varied personal, social, and legal consequences. Specialized treatments, including individual, virtual, and group cognitive and behavioral therapies, community-based interventions, and peer support approaches have shown preliminary effectiveness. Animal, attachment, and neurobiological models are expanding our understanding of the etiological bases of the disorder. Specialized populations such as children,

Cognitive behavioral therapy for hoarding disorder: An updated meta-analysis.

BACKGROUND: Hoarding disorder (HD) is a new disorder in DSM-5. While cognitive-behavioral therapy (CBT) is seen as the gold standard approach to treatment, the literature in this field is still emerging. METHODS: The aim of the present study is to synthesize the current treatment outcome literature on CBT for HD, as well as secondary depressive symptoms, using a meta-analytic approach. Due to a lack of controlled trials only within-group effect sizes were calculated. RESULTS: Sixteen studies were included in the meta-analysis (n = 505; mean age = 56 years; mean percentage female participants

Prevalence of Hoarding Disorder: A systematic review and meta-analysis.

Currently there is uncertainty concerning the prevalence of Hoarding Disorder (HD) due to methodological issues in the evidence base. Estimates have widely ranged from between 1.5% and 6% of the general population. This systematic review and meta-analysis therefore aimed to summarise and reliably estimate the prevalence of HD by employing strict inclusion criteria and using studies with sufficiently large samples. A systematic literature search was conducted to identify all relevant prevalence studies. Inclusion criteria were studies that reported working age adult HD prevalence rates and had

Sex-specific clinical correlates of hoarding in obsessive-compulsive disorder.

Little is known about whether the clinical correlates of hoarding behavior are different in men and women with obsessive-compulsive disorder (OCD). In the current study, we evaluated the association of hoarding with categories of obsessions and compulsions, psychiatric disorders, personality dimensions, and other clinical characteristics separately in 151 men and 358 women with OCD who were examined during the OCD Collaborative Genetics Study. We found that, among men but not women, hoarding was associated with aggressive, sexual, and religious obsessions and checking compulsions. In men, hoar

Who really hoards? Hoarding symptoms in adults with attention-deficit hyperactivity disorder (ADHD), obsessive-compulsive disorder (OCD) and healthy controls.

UNLABELLED: Although hoarding disorder (HD) is included in the DSM-5 in the obsessive-compulsive and related disorders chapter, in the last few years, HD has been consistently associated with attention-deficit hyperactivity disorder (ADHD). Some studies on HD patients show higher comorbidity with ADHD than with OCD and some studies on ADHD patients found significant higher rates of HD symptoms compared to the general population. However, the aim of the present study was to be the first direct comparison of the prevalence of HD and HD symptoms across adults with a primary diagnosis of ADHD, OCD

An exploration of exaggerated fears of identity theft in hoarding disorder.

INTRODUCTION: Individuals with hoarding symptoms often report laborious routines of reviewing and destroying paperwork prior to discarding, citing fears of identity theft if personal information is discarded. While fears of identity theft seem likely to be elevated in hoarding disorder, it is unclear if these fears are elevated beyond what is normative for older adults, and whether such fears may be related to third factors. METHOD: In the current study we sought to explore whether fears of identity theft were higher among 103 individuals high in hoarding symptoms as compared with 59 non-hoar

A comparison between community and treatment-seeking samples of hoarding disorder.

OBJECTIVE: Hoarding disorder studies are primarily based on persons who seek treatment and demonstrate good insight. The aim of the present study is to evaluate whether there are differences between community and treatment-seeking samples of individuals with hoarding disorder (HD). METHODS: Fourteen people with HD from the community and twenty treatment-seeking people with HD were assessed by a battery of instruments to evaluate HD features and other associated characteristics. RESULTS: Compared to the treatment-seeking sample, the HD community sample was older, had poorer insight, and had a

Rates of diagnosis and service utilization in veterans with hoarding disorder.

Hoarding Disorder (HD) is a prominent and disabling neuropsychiatric condition defined by the inability to discard objects resulting in impairing levels of clutter. The prevalence rate is 2-6 % and increases with age. The aging Veteran population is a high risk group for impairment associated with HD. Medical and psychiatric comorbidities as well as associated rates of disability and poor quality of life are very common in both HD and the related disorder of OCD. We examined rates of HD and OCD diagnoses at the VA San Diego Healthcare System. Data were obtained from medical records for all Vet

Discarding personal possessions increases psychophysiological activation in patients with hoarding disorder.

Difficulty discarding is the core behavioral symptom of hoarding disorder (HD). Patients with HD report greater subjective distress when discarding their own possessions as compared to others' possessions. To date, no prior studies have examined psychophysiological activation, an objective measure of anxious arousal, during discarding among individuals with HD. The current study assessed psychophysiological responses during a baseline resting period and two discarding tasks, one involving personal possessions and the other involving matched control ("experimenter-owned") items in 52 patients w

Hoarding Symptoms Respond to Treatment for Rapid Cycling Bipolar II Disorder.

Although some studies have reported a relationship between hoarding and bipolar disorder, we are unaware of any previous description of how they may interact with each other and how they should be managed appropriately. A 48-year-old male depressed patient with hoarding symptoms and obsessive-compulsive disorder (OCD) was diagnosed with bipolar II disorder after 2 hypomanic episodes. The patient was treated unsuccessfully with different high-dose serotonin reuptake inhibitors and atypical antipsychotics, maintaining a pattern of 6 to 8 discrete, but severe, depressive episodes each year, alway

Diogenes syndrome in a patient with obsessive-compulsive disorder without hoarding.

Diogenes syndrome (DS) is characterized by self-neglect, classically portrayed by a filthy personal appearance, dirty home, and hoarding of rubbish. We report a patient with DS who presented obsessive-compulsive disorder and Tourette syndrome in the absence of hoarding. We suggest that hoarding may be a symptom of the conditions that are frequently comorbid with DS, but is not one of its fundamental features.

Changes in hoarding-related beliefs and associated neural changes during a simulated discarding task after cognitive-behavioral treatment for hoarding disorder.

Hoarding disorder (HD) is maintained by maladaptive beliefs about possessions, and recent research has demonstrated that changes in these beliefs partially mediate improvement in cognitive-behavioral therapy (CBT) for hoarding. It is not yet known whether changes in neural activity, particularly when discarding possessions, are associated with cognitive change during CBT for HD. Adults who completed group CBT for HD (N = 58) participated in a simulated discarding task before and after CBT. Neural activity was measured using functional magnetic resonance imaging (fMRI) as participants made simu

Identifying predictors of treatment response in Hoarding Disorder using machine learning.

The current gold standard treatment for Hoarding Disorder (HD) is cognitive behavioral therapy (CBT). However, typical symptom reduction is 25% and only about one third of individuals achieve a clinically meaningful response. We used machine learning (ML) to identify predictors of treatment response using data from a non-inferiority trial of clinician vs peer-led group CBT for HD. Assessments were completed for 323 individuals with HD at post-treatment (N = 323) and at one-year follow-up (N = 183). Assessments included neuropsychological testing, symptom severity and psychiatric diagnoses, bel

Central and peripheral physiological responses to decision making in hoarding disorder.

Individuals with hoarding disorder (HD) have difficulty parting with personal possessions, which leads to the accumulation of excessive clutter. According to a proposed biphasic neurobiological model, HD is characterized by blunted central and peripheral nervous system activity at rest and during neutral (non-discarding) decisions, and exaggerated activity during decision-making about discarding personal possessions. Here, we compared the error-related negativity (ERN) and psychophysiological responses (skin conductance, heart rate and heart rate variability, and end tidal CO2) during neutral

Functional Neuroimaging Test of an Emerging Neurobiological Model of Hoarding Disorder.

BACKGROUND: Over the past decade, functional neuroimaging studies have found abnormal brain function in several cortical systems when patients with compulsive hoarding behaviors make decisions about personal possessions. The purpose of this study was to use functional magnetic resonance imaging to test a neurobiological model of hoarding disorder (HD) that has begun to emerge from these small studies by confirming HD-related brain dysfunction in previously implicated brain regions in the largest sample of HD patients examined to date. METHODS: We compared 79 adults diagnosed with DSM-5 HD wit
