Home-based supportive and health care services based on functional ability in older adults in Iran.

BACKGROUND: Home-based care is affordable due to population aging, increased chronic disease, and higher hospitalization costs. The objective was to evaluate home-based supportive and health care services provided to older adults and identify possible associations between activities of daily living (ADLs), instrumental ADL (IADLs) classifications, sociodemographic variables, clinical characteristics, and perceived social support among older adults. MATERIALS AND METHODS: In this cross-sectional study, 700 people aged 60 years and older were selected by stratified cluster sampling. Areas of Ta

Nursing care needs and services utilised by home-dwelling elderly with complex health problems: observational study.

BACKGROUND: In Norway, as in many Western countries, a shift from institutional care to home care is taking place. Our knowledge is limited regarding which needs for nursing interventions patients being cared for in their home have, and how they are met. We aimed at assessing aspects of health and function in a representative sample of the most vulnerable home-dwelling elderly, to identify their needs for nursing interventions and how these needs were met. METHODS: In this observational study we included patients aged 75+ living in their own homes in Oslo, who received daily home care, had th

Inequity in maternal health-care services: evidence from home-based skilled-birth-attendant programmes in Bangladesh.

OBJECTIVE: To explore use-inequity in maternal health-care services in home-based skilled-birth-attendant (SBA) programme areas in Bangladesh. METHODS: Data from a community survey, conducted from February to May 2006, were analysed to examine inequities in use of SBAs, caesarean sections for deliveries and postnatal care services according to key socioeconomic factors. FINDINGS: Of 2164 deliveries, 35% had an SBA, 22.8% were in health facilities and 10.8% were by caesarean section. Rates of uptake of antenatal and postnatal care were 93% and 28%, respectively. There were substantial use-ine

Continence care quality from the perspective of older adults receiving home care services and residents of long-term care: a scoping review protocol.

INTRODUCTION: Urinary incontinence is associated with social isolation, deconditioning, depression, falls and early mortality. It impairs quality of life, even in residents of nursing homes, and, in the community, increases the risk of institutionalisation. Care focused on the preservation of dignity during intimate care is important in the care of older adults. Despite this, there are few data which seek to define the quality of care or desired outcomes of care from the older adult's perspective. This scoping review aims to assess the research literature addressing the question of what is kno

The Impact of a Psychiatric Home Care Program on Mental Health Services Utilization and Costs in Spain.

Crisis resolution and home treatment (CRHT) is a home-based alternative to acute psychiatric hospitalization. A retrospective cohort study in Spain measured the impact of CRHT on mental health resource utilization, comparing emergency department (ED) visits, acute admissions, and length of stay. The authors compared three CRHT catchment areas (N=298,935 adults) with three control areas without CRHT (N=157,334 adults). The mean cost of CRHT was compared with that of inpatient hospitalization. Results showed that CRHT models may be associated with a reduction in ED visits and potentially lower m

Effectiveness and cost-effectiveness of home palliative care services for adults with advanced illness and their caregivers.

General practitioners' views and experiences of providing services in Adelaide residential aged care homes.

AIM: To explore general practitioners' (GPs') views and experiences of providing services in Adelaide residential aged care homes (RACHs) and identify barriers, enablers, and suitable models of care (MoC). BACKGROUND: National reports describe persistent challenges with GP access in RACHs. Residents often have limited choice of visiting GPs, and fee-for-service payments incentivise episodic rather than proactive care. METHODS: A qualitative study comprising semi-structured interviews was conducted with Adelaide-based GPs. Interviews were audio-recorded, transcribed, and analysed using reflex

Sociodemographic Characteristics of Patients Registered with a Home Care Unit and an Evaluation of the Health Services Offered.

OBJECTIVES: The growing aging population and the difficulties they often face in accessing health services brought attention to home care units. Which need characteristics influence healthcare service utilization in home care arrangements in Germany?

BACKGROUND: We see a growing number of older adults receiving long-term care in industrialized countries. The Healthcare Utilization Model by Andersen suggests that individual need characteristics influence utilization. The purpose of this study is to analyze correlations between need characteristics and service utilization in home care arrangements. METHODS: 1,152 respondents answered the questionnaire regarding their integration of services in their current and future care arrangements. Care recipients with high long-term care needs answered the questionnaire on their own, the family caregi

Fall risk factors in community-dwelling elderly who receive Medicaid-supported home- and community-based care services.

OBJECTIVE: This study identifies fall risk factors in an understudied population of older people who receive community-based care services. METHOD: Data were collected from enrollees of Ohio's Medicaid home- and community-based waiver program (preadmission screening system providing options and resources today [PASSPORT]). A total of 23,182 participants receiving PASSPORT services in 2005/2006 was classified as fallers and nonfallers, and a variety of risk factors for falling was analyzed using logistic regressions. RESULTS: The following factors were identified as risk factors for falling:

Stroke rehabilitation services to accelerate hospital discharge and provide home-based care: an overview and cost analysis.

BACKGROUND: Limited information exists on the best way to organise stroke rehabilitation after hospital discharge and the relative costs of such services. OBJECTIVE: To review the evidence of the cost effectiveness of services that accelerate hospital discharge and provide home-based rehabilitation for patients with acute stroke. METHODS: A systematic review with economic analysis of published randomised clinical trials (available to March 2001) comparing early hospital discharge and domiciliary rehabilitation with usual care in patients with stroke was conducted. From included studies, data

What Women With Dementia Who Receive Home Care Services Consider Important in Daily Life: A Qualitative Study.

INTRODUCTION: Most people with dementia in Norway live at home, and maintaining physical, social and spiritual activity remains a basic need. Yet research has focused on women as caregivers rather than women living with dementia. Consequently, more knowledge is needed into what women receiving home care service consider important in daily life to tailor health care services and to understand how these women experience participating in activities. OBJECTIVE: The aim of this study was to describe what women with dementia who receive home care service consider important in their daily lives. ME

Development of 'Location of Dialysis Care in Kidney Life', a service delivery intervention to eliminate unwarranted variation in home dialysis uptake between kidney services and to optimise home dialysis use.

AIM: To introduce a new service delivery intervention to improve equity of access to home dialysis therapy and describe the process of intervention development. BACKGROUND: Despite strong evidence in favour of home dialysis, its uptake remains stubbornly low in England and elsewhere. Furthermore, uptake levels vary between kidney services and between population groups. The Inter-CEPt study identified several enablers of access to home dialysis, including: receptive organisational cultures; adoption of reflective practice; engagement in quality improvement, and shared belief in the benefit of
