OBJECTIVE: To evaluate adherence to JNC VII guidelines in nursing home patients.

Orthostatic hypotension is a common condition among nursing home (NH) residents. NH residents tend to have multiple disease processes and tend to be on multiple medications associated with orthostatic hypotension and are predisposed to a myriad of negative clinical consequences, most notably falls. This article discusses a commonsense approach to diagnosis, evaluation, and treatment of patients with this disorder, with an emphasis on nonpharmacological interventions, such as patient and staff education.

Pain is an understudied problem in frail elderly patients, especially those with cognitive impairment, delirium, or dementia. The objective of this study was to identify differences between nursing home (NH) and nonnursing home (non-NH) patients with bacteremia regarding host risk factors, exposures, microbiology, and outcome. Between October 1995 and February 1998, 134 episodes of true bacteremia were identified at the North Chicago Veterans Administration Medical Center (NCVAMC). Clinical practice guidelines are an important tool for improving quality of care. This study determined whether and how guidelines are being used in nursing homes. We surveyed staff at 36 Department of Veterans Affairs (VA) nursing homes. Employees were asked whether they were familiar with guidelines as well as whether 5 specific guidelines had been read, were available, and had been adopted. A Neonatal Integrated Home Care Program was developed to cross-train NICU staff nurses to provide follow-up care for high-risk neonates in the home. Implementation required collaboration of the NICU and the Center for Home Care and Hospice.
