Health professionals and hospital administrators in organ procurement: attitudes, reservations, and their resolutions.

The responses of hospital administrators, directors of nursing, intensive care unit nurses, and neurosurgeons are reported to a range of inquiries designed to measure their commitment to organ procurement and thereby identify impediments limiting their cooperation with organ procurement efforts. Descriptive and multivariate statistical techniques are used to analyze data collected from each group. We find general approval for organ procurement but serious hesitation about dealing with donor families, particularly among physicians. Physician support for donation, moreover, is the strongest pred

Inclusion of Veterans Health Administration hospitals in Centers for Medicare & Medicaid Services Overall Hospital Quality Star Ratings.

BACKGROUND/OBJECTIVE: The Centers for Medicare & Medicaid Services (CMS) Overall Hospital Quality Star Rating, established in 2016, is a summary of publicly available quality information for acute care hospitals. In July 2023, Veterans Health Administration (VHA) hospitals became eligible to receive a CMS Overall Hospital Quality Star Rating for the first time. Our objective was to compare performance in quality ratings among VHA and non-VHA hospitals. METHODS: We used the hospital quality measure scores posted to Care Compare on Medicare.gov as of January 2023 as our primary data set. We con

Identifying Contextual Factors affecting Hospital In Home Program Implementation within the Veterans Health Administration.

BACKGROUND AND OBJECTIVES: Understanding the factors that enable or hinder successful implementation is essential for improving patient outcomes. Hospital in Home (HiH) programs provide hospital-level care in patients' homes to reduce hospitalization, enhance satisfaction, and improve resource use. Within the Veterans Health Administration (VHA), 14 sites have implemented HiH programs, yet limited research has examined implementation within a large integrated system. This study identifies factors that can support or hinder HiH implementation. RESEARCH DESIGN AND METHODS: We conducted intervie

Improved pertussis vaccine uptake following in-hospital administration among pregnant women living with HIV.

BACKGROUND: Pregnant women living with HIV (PWLHIV) are at increased risk of infectious complications during pregnancy, and HIV-exposed but uninfected women are more susceptible to serious infectious diseases. Therefore, maternal immunization during pregnancy is an essential standard of care for this patient population. However, vaccination during pregnancy is suboptimal among these patients. METHODS: This is a single-center retrospective observational study, conducted at the University Hospital Federico II of Naples, Italy. We examined how our Center's vaccination uptake among PWLHIV changed

Early amiodarone or lidocaine administration during in-hospital cardiac arrest caused by shockable rhythms.

INTRODUCTION: Published data investigating a time-dependent effect of initiation of antiarrhythmic therapy for shockable in-hospital cardiac arrest (IHCA) is lacking. We aimed to evaluate the association between time of intravenous amiodarone or lidocaine administration and return of spontaneous circulation (ROSC) in patients with IHCA caused by ventricular fibrillation (VF) or pulseless ventricular tachycardia (pVT). METHODS: This was a retrospective, multi-center, single health system, observational cohort study of patients with an IHCA caused by VF/pVT and who received amiodarone or lidoca

Impact of domiciliary administration of NSAIDs on COVID-19 hospital outcomes: an unCoVer analysis.

Background: Effective domiciliary treatment can be useful in the early phase of COVID-19 to limit disease progression, and pressure on hospitals. There are discrepant data on the use of non-steroidal anti-inflammatory drugs (NSAIDs). Aim of this study is to evaluate whether the clinical outcome of patients who were hospitalized for COVID-19 is influenced by domiciliary treatment with NSAIDs. Secondary objective was to explore the association between other patient characteristics/therapies and outcome. Methods: A large dataset of COVID-19 patients was created in the context of a European Union-

Practice Patterns in Perioperative Nonopioid Analgesic Administration by Anesthesiologists in a Veterans Affairs Hospital.

OBJECTIVE: Although multimodal analgesia (MMA) is recommended for perioperative pain management, previous studies have found substantial variability in its utilization. To better understand the factors that influence anesthesiologists' choices, we assessed the associations between patient or surgical characteristics and number of nonopioid analgesic modes received intraoperatively across a variety of surgeries in a university-affiliated Veteran Affairs hospital. METHODS: We included elective inpatient surgeries (orthopedic, thoracic, spine, abdominal, and pelvic procedures) that used at least

A Comparison of Administrative Data Versus Surveillance Data for Hospital-Associated Methicillin-Resistant Staphylococcus aureus Infections in Canadian Hospitals.

BACKGROUND In Canadian hospitals, clinical information is coded according to national coding standards and is routinely collected as administrative data. Administrative data may complement active surveillance programs by providing in-hospital MRSA infection data in a standardized and efficient manner, but only if infections are accurately captured. OBJECTIVE To assess the accuracy of administrative data regarding in-hospital bloodstream infections (BSIs) and all-body-site infections due to MRSA. METHODS A retrospective study of all (adult and pediatric) in-hospital MRSA infections was conducte

Mediating hospital attendance: Invisible administrative work and structural vulnerability in outpatient care.

Non-attendance at outpatient appointments is commonly understood as a problem of patient behaviour. However, this perspective overlooks how organisational arrangements shape patients' opportunities to attend care. This study examines non-attendance as an organisationally mediated outcome, focusing on how administrative practices influence access to outpatient services. Using an institutional ethnographic approach, we conducted observations and interviews with medical secretaries in two outpatient surgical departments in Denmark and analysed relevant policy documents. The analysis was informed
