Evaluating administrative service quality in public hospitals: a SERVPERF-based study in Portugal.

PURPOSE: Administrative services constitute the first point of contact for hospital users and play a critical role in shaping perceptions of service quality. However, quality assessments in health-care have largely prioritised clinical indicators, leaving administrative functions underexamined. This study addresses this gap by analysing user perceptions of administrative service quality in a Portuguese public hospital, aiming to identify strengths, weaknesses and opportunities for improvement. DESIGN/METHODOLOGY/APPROACH: A quantitative observational study was conducted across four administra

Evaluation of the impact of high altitude on pre-hospital anaesthetic administration: a scoping review.

BACKGROUND: Increasing high-altitude travel and habitation drive demand for care in these environments. High altitude presents unique challenges to anaesthetic administration as hypobaric hypoxia affects physiology, pharmacology, and equipment performance, yet no guidelines exist. This scoping review aimed to identify anaesthetics used at HA, evaluate the impact of high altitude on anaesthetic use, and highlight research gaps. METHODS: This review followed Arksey and O'Malley's methodological framework and is reported according to the Preferred Reporting Items for Systematic Reviews and Meta-

Term Delivery After Late-Preterm Antenatal Steroid Administration in U.S. Hospitals.

Given the challenges in accurately predicting birth timing, we sought to quantify the proportion of individuals who receive late-preterm steroids in routine clinical practice who ultimately deliver at term. This retrospective cohort study of singleton gestations used data from January 2021 to December 2024 from the Premier Healthcare Database. We calculated the proportion of late-preterm steroid exposures in women who delivered at term and analyzed rates by hospital. The cohort included 519 hospitals and 97,322 singleton delivery hospitalizations of women with late-preterm betamethasone exposu

Do discharge delays explain longer stays at veterans health administration hospitals?

BACKGROUND: Discharge delays are difficult to quantify without standardized indicators for when patients are medically ready for discharge. We aimed to estimate the proportion of increased hospital length of stay attributable to discharge delays, as proxied by increases in 'avoidable' days. METHODS: We conducted a retrospective cohort study of Veterans Health Administration hospitals in the continental United States with emergency departments between 1 March 2019 and 28 February 2023. We included Veterans who were discharged from an acute medicine service without a COVID-19 diagnosis. We used

Early antibiotics administration reduces mortality in sepsis patients in tertiary care hospital.

INTRODUCTION: Early antibiotic administration is one of the core treatments of sepsis which associated with reduced mortality rate. However, the appropriate timing of antibiotics remains a controversial issue, especially in patients without septic shock. Here, we reported the outcomes of early antibiotic administration within one hour from the time of infection suspicion in a tertiary care hospital. METHODS: We reviewed the medical records and sepsis protocols in Chulabhorn Hospital, Bangkok, Thailand, from January 2021 to December 2023 for patients presenting with sepsis. We had our own seps

Impact of domiciliary administration of NSAIDs on COVID-19 hospital outcomes: an unCoVer analysis.

Background: Effective domiciliary treatment can be useful in the early phase of COVID-19 to limit disease progression, and pressure on hospitals. There are discrepant data on the use of non-steroidal anti-inflammatory drugs (NSAIDs). Aim of this study is to evaluate whether the clinical outcome of patients who were hospitalized for COVID-19 is influenced by domiciliary treatment with NSAIDs. Secondary objective was to explore the association between other patient characteristics/therapies and outcome. Methods: A large dataset of COVID-19 patients was created in the context of a European Union-

Multisource and temporal variability in Portuguese hospital administrative datasets: Data quality implications.

BACKGROUND: Unexpected variability across healthcare datasets may indicate data quality issues and thereby affect the credibility of these data for reutilization. No gold-standard reference dataset or methods for variability assessment are usually available for these datasets. In this study, we aim to describe the process of discovering data quality implications by applying a set of methods for assessing variability between sources and over time in a large hospital database. METHODS: We described and applied a set of multisource and temporal variability assessment methods in a large Portugues
