Treatment of Severe Emotionally Unstable Personality Disorder with Comorbid Ehlers-Danlos Syndrome and Functional Neurological Disorder in an Inpatient Setting: A Case for Specialist Units without Restrictive Interventions.

We present the case of a young woman with an Emotionally Unstable Personality Disorder (EUPD) diagnosis suffering from high-risk self-injurious behaviour. She was also diagnosed with Ehlers-Danlos Syndrome and Functional Neurological Disorder, manifesting as nonepileptic seizures and immobility. Our patient, "A," endured traumatic childhood abuse and became highly dependent on services in her late teens. Recurrent suicide attempts resulted in twenty to thirty acute psychiatric admissions, Intensive Care Unit stays, and multiple failed trials of psychological therapy. Nonepileptic seizures and

Deinstitutionalization and attitudes toward mental illness in Jamaica: a qualitative study.

OBJECTIVE: To consider whether or not deinstitutionalization and the integration of community mental health care with primary health care services have reduced stigma toward mental illness in Jamaica. METHODS: A qualitative study of 20 focus groups, with a total of 159 participants grouped by shared sociodemographic traits. Results were analyzed using ATLAS.ti software. RESULTS: Participant narratives showed that stigma had transitioned from negative to positive, from avoidance and fear of violent behavior during the period of deinstitutionalization to feelings of compassion and kindness as

Significant reductions in tertiary hospital encounters and less travel for families after implementation of Paediatric Care Coordination in Australia.

BACKGROUND: Over a third of Australian children have long-term health conditions, often involving multiple organ systems and resulting in complex health care needs. Our healthcare system struggles to meet their needs because of sectoral fragmentation and episodic models of care. Children with medical complexity (CMC) currently rely on tertiary paediatric hospitals for most of their healthcare, but this is not sustainable. We evaluated the impacts of Care Coordination on tertiary hospital service use and family outcomes. METHODS: A pre- and post-implementation cohort evaluation of the Care Coo

Institutionalization and home health care after acute hospitalizations of older persons in Norway.

BACKGROUND: Population ageing and strained public resources challenge the future provision of health and care services. We assess the uptake of formal health-related care services (FCS), either home health care (HHC) or short- or long-term institutional care (IC), before and after an acute hospital admission in a full-population sample of older persons. METHODS: Multinominal logistic population average regression models were applied to Norwegian registry data on older persons (aged 75+) in 2021 (N = 68,803) at hospital discharge following admission for select diagnoses (N = 94,748) to examine

Costs of hospital-based methadone maintenance treatment in HIV/AIDS control among injecting drug users in Indonesia.

OBJECTIVE: To assess the cost of hospital-based methadone maintenance treatment (MMT) for injecting drug users (IDUs) in Bandung, Indonesia; to address concerns of financial sustainability at the hospital level and financial accessibility and economic attractiveness at the health care policy level. METHODS: In a 1 year observation period in 2006-2007, MMT service delivery costs were estimated on the basis of a micro-costing approach. Patient costs were estimated on the basis of a survey among 48 methadone clients. RESULTS: A total number of 129 clients attended the MMT clinic, resulting in a

US Hospital Service Availability and New 340B Program Participation.

IMPORTANCE: The US 340B Drug Pricing Program enables eligible hospitals to receive substantial discounts on outpatient drugs to improve hospitals' financial sustainability and maintain access to care for patients who have low income and/or are uninsured. However, it is unclear whether hospitals use program savings to subsidize access as intended. OBJECTIVE: To evaluate whether the 340B program is associated with improvements in access to hospital-based services and to test whether the association varies by hospital ownership. DESIGN, SETTING, AND PARTICIPANTS: Difference-in-differences and c

Out-of-hospital paediatric emergencies: a prospective, population-based study.

BACKGROUND: We wanted to study the incidence, distribution and characteristics of paediatric out-of-hospital emergency care on a population level. This knowledge could ameliorate the design and education of emergency medical services and their personnel. METHODS: We studied all (n = 1863) emergency medical services responses and the patient records for paediatric patients (age 0-16 years) in Helsinki, Finland (population 603,968, paediatric population 92,742) during a 12-month period (2012). Patient characteristics, diagnoses, time intervals, medical treatments, procedures, vital measurements

New institutionalization as a rebound phenomenon? The case of Israel.

OBJECTIVE: To explore whether the history of significant deinstitutionalization in Western European countries since the 1950s, and in Israel more recently, may have led to the establishment of new alternative institutions as a "rebound" phenomenon. METHODS: Data on service provision in Israel are analyzed and compared with published data from Europe. We considered five commonly used indicators of mental health services to reflect trends in institutionalization in psychiatric hospitals, the forensic system and supported housing. RESULTS: In Israel, there has been a substantial increase in pla

A process model of psychological acceptance in technology-based service innovation: a symbolic interactionism perspective.

As digital technologies become increasingly embedded in service systems, the diffusion of technology-based service innovations is often gradual and contested rather than automatic. Prior research has provided robust explanations for early-stage technology evaluation by treating adoption as a discrete decision or momentary experience. However, it provides limited insight into the longer-term process through which user acceptance gradually develops through ongoing use. Drawing on symbolic interactionism, this study reconceptualizes diffusion of technology-based service innovation as a process of

Implementing care for healthy ageing.

The WHO concept of Healthy Ageing (ie, the process of developing and maintaining the functional ability that enables well-being in older age) has initiated a global discussion about the need for shifting paradigms to reorient health and social services towards person-centred and coordinated models of care. In particular, the integration of health and social care services is critical to provide the basis for comprehensive information sharing and service delivery to support the evolution of the older person over time. The capability to monitor and respond to an older person's changing health and

HIV service delivery in the time of COVID-19: focus group discussions with key populations in India.

INTRODUCTION: There are limited data on the impact of COVID-19-associated disruptions and novel HIV service delivery strategies among key populations (KPs) in low- and middle-income countries. In March 2020, in response to COVID-19, the Government of India revised HIV service delivery policies to include community antiretroviral therapy (ART) distribution and multi-month dispensing (MMD) of ART for all people living with HIV (PLHIV). METHODS: To assess the acceptability of these adaptations and impact of the pandemic among KPs, we conducted focus groups in November-December 2020 with purposiv

Munchausen syndrome presenting as major trauma.

The case is described of a man who feigned being struck by a vehicle, leading to an unnecessary major trauma response by the ambulance service and hospital. Suspicion that the patient suffered from Munchausen syndrome was confirmed by later investigation. Accident and emergency staff should file details of such patients on the department computer records system, where available, so that staff are alerted automatically to their presence, and share this information with neighbouring hospitals. All such patients should be treated according to ATLS guidelines until injury is ruled out, as for any

Patient outcomes and experience of a task-shared screening and brief intervention service for problem substance use in South African emergency centres: a mixed methods study.

BACKGROUND: Screening, brief intervention and referral to treatment (SBIRT) programmes have resulted in generally positive outcomes in healthcare settings, particularly for problem alcohol use, yet implementation is hampered by barriers such as concerns regarding the burden on healthcare professionals. In low-resourced settings, task-sharing approaches can reduce this burden by using non-professional healthcare workers, yet data are scarce regarding the outcomes and acceptability to patients within a SBIRT service. This study aims to evaluate patient-reported outcomes, patient acceptability, p

The role and functions of community-based multidisciplinary teams in two integrated care and support Pioneers: Perspectives from local system leaders.

ObjectivesCentral government has been promoting closer integration between the National Health Service (NHS) and local government social services in England for more than five decades. Improved coordination between primary, hospital, community health and social services has been advocated as a cost-effective response to growing care needs in an ageing population. This paper concentrates on one of the principal local care coordination mechanisms: community-based multidisciplinary teams (MDTs) involving NHS and social services staff. It reports local leaders' perceptions of MDTs' current and fut

The ice storm in eastern Canada 1998 KAMEDO-Report No. 74.

This is a report of the impact of the ice storm that struck eastern Canada on 04-09 January 1998. The storm deposited ice some 100 mm thick on the ground and on the electric power lines and eventually left 1.4 million households and much of the infrastructure without electrical power. Data were obtained through non-structured interviews of those involved. Most of the larger hospitals were equipped with emergency generating equipment and were able to provide most essential services. For most hospitals, non-emergency services were compromised. Many other medical facilities, including clinics had
