Stakeholder Perspectives on the Use of Telehealth to Support Deprescribing for Older Patients Transitioning From the Hospital to Home Health Care.

BACKGROUND: Multimorbidity affects most older adults in home health care (HHC), leading to inappropriate polypharmacy and increased risks for adverse outcomes. While deprescribing can mitigate these risks, fragmented communication during hospital-to-home transitions remains a major barrier. This study explored stakeholder perspectives on using telehealth to facilitate deprescribing for older adults with multimorbidity while receiving post-acute HHC. METHODS: We conducted semi-structured interviews with 44 stakeholders across 12 U.S. states, including 14 HHC patients and 30 clinicians (physici

Mapping the Evolving AI Preferences and Care Needs in Orthopedic Transitional Care From Hospitals to Home: Cross-Sectional Study.

BACKGROUND: Enhanced recovery after surgery protocols have shortened orthopedic hospital stays but have shifted rehabilitation and safety-monitoring tasks to patients and families after discharge. In this study, AI refers to patient-facing digital systems for orthopedic transitional care, including large language model chatbots, computer vision or platform-based monitoring tools, and wearable sensor-enabled systems for education, rehabilitation guidance, motion correction, and risk alerts. However, patient-reported preferences for different AI-supported functions across the hospital-to-home tr

Do hospital-to-home transitions work for older adults with multiple long-term conditions including dementia? A realist review.

BACKGROUND: Hospital-to-home transitions involve multiple providers and are particularly complex for older adults with dementia, who often live with additional conditions. Frequent transitions increase the risk of errors, miscommunication, and treatment delays, compromising patient safety and leading to potentially increased mortality, morbidity, and preventable readmissions. Understanding what works and does not work in these processes is essential to improving outcomes. AIM: This realist review synthesised existing literature to explore how, for whom, and to what extent hospital-to-home tra

Healthcare providers' perceived acceptability of a warning signs intervention for rural hospital-to-home transitional care: A cross-sectional study.

INTRODUCTION: There is a pressing need for transitional care that prepares rural dwelling medical patients to identify and respond to the signs of worsening health conditions. An evidence-based warning signs intervention has the potential to address this need. While the intervention is predominantly delivered by nurses, other healthcare providers may be required to deliver it in rural communities where human health resources are typically limited. Understanding the perspectives of other healthcare providers likely to be involved in delivering the intervention is a necessary first step to avert

A qualitative formative evaluation of a patient facing intervention to improve care transitions for older people moving from hospital to home.

BACKGROUND: The Partners at Care Transitions (PACTs) intervention was developed to support older people's involvement in hospital to improve outcomes at home. A booklet, question card, record sheet, induction leaflet, and patient-friendly discharge letter support patients to be more involved in their health and wellbeing, medications, activities of daily living and post-discharge care. We aimed to assess intervention acceptability, identify implementation tools, and further develop the intervention. METHODS: This was a qualitative formative evaluation involving three wards from one hospital.

Feasibility and preliminary effects of an integrated hospital-to-home transitional care intervention for older adults with stroke and multimorbidity: A study protocol.

BACKGROUND: Stroke is a major life-altering event and the leading cause of death and disability in Canada. Most older adults who have suffered a stroke will return home and require ongoing rehabilitation in the community. Transitioning from hospital to home is reportedly very stressful and challenging, particularly if stroke survivors have multiple chronic conditions. New interventions are needed to improve the quality of transitions from hospital to home for this vulnerable population. OBJECTIVES: The primary objective of this study is to examine the feasibility of implementing a new 6-month

Accelerating patient transitions from hospital to home in the United Kingdom with single-use negative pressure wound therapy: a discharge pathway.

BACKGROUND: Initially limited to inpatient use, negative pressure wound therapy (NPWT) is now frequently used in community settings. However, complexities in wound management step-down strategies in the United Kingdom, including regional variations in referral processes, lack of consensus on funding criteria, and limited availability of NPWT units, have led to extended hospital length of stay (LOS) for patients ready for discharge but still needing NPWT. Single-use NPWT (sNPWT) can serve as a bridge between hospital and community NPWT. OBJECTIVE: To assess the feasibility, effectiveness, and

Exploring Stakeholder Perspectives on the Transitional Care Needs of Elderly Patients from Hospital to Home: A Phenomenological Study in Shanxi Province, China.

BACKGROUND: Elderly patients, due to their complex medical conditions and extensive care needs, are at risk of experiencing low-quality or fragmented care during transitions between different healthcare settings. After transitioning from hospital to home, inadequate self-care abilities may result in further health deterioration and increased risks of adverse outcomes. Currently, China lacks effective transitional support services from hospital to home, hindering the smooth transition for elderly patients. Therefore, understanding the specific care needs of elderly patients during this period p

Supporting resident-centred decision-making about transitions from long-term care homes to hospital: a qualitative study protocol.

INTRODUCTION: Burdensome care transitions may occur despite clinicians' engagement in care planning discussions with residents and their family/friend care partners. Conversations about potential hospital transfers can better prepare long-term care (LTC) residents, their families and care providers for future decision-making. Lack of such discussions increases the likelihood of transitions that do not align with residents' values. This study will examine experiences of LTC residents, family/friend care partners and staff surrounding decision-making about LTC to hospital transitions and codesig

A person-centered continuity pathway for pediatric contrast-enhanced imaging: sustaining one multisensory context across home, hospital, and scanner transitions.

BACKGROUND: Pediatric contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) require sustained cooperation, procedural immobility, and venous access, creating operational and experiential challenges for children, families, and imaging services. Existing nonpharmacological interventions generally address isolated procedural segments and do not preserve a familiar context across home, hospital, and scanner transitions. INNOVATION: Euterpe-Radiology (Euterpe-RAD) is a consent-gated, five-module institutional pathway that sustains one autobiographically anchored multisens

Home is "a different planet" when transitioning from hospital after acquired brain injury: can digital technology help bridge the two worlds?

PURPOSE: Adjusting to life following an acquired brain injury (ABI) is challenging particularly navigating the timepoint of transitioning from hospital-to-home. Digital technology has been proposed to mitigate broader transitional issues, however whilst still in its infancy more research is recommended within the ABI population. It is important to understand individual experiences in healthcare design. This study aims to identify for people with ABI [1] their unique problems experienced when transitioning hospital-to-home, and [2] how they believe digital technological solutions could solve th

Transitioning patients with inflammatory bowel disease from hospital-based to rapid home-based infliximab: A stepwise, safety and patient-orientated process towards sustainability.

BACKGROUND: Infliximab and other intravenous biologic infusions are increasingly used for chronic disorders like inflammatory bowel disease (IBD). Rapid infliximab and home-based infusions are attractive solutions to address resource and capacity issues for infusion centres, yet infliximab infusion reactions reportedly occur in up to 25% of patients with IBD, even at the manufacturers' recommended infusion duration of 2 h. AIM: To evaluate the safety, cost and patient satisfaction of transitioning from hospital-based, standard 2 h to rapid home-based, 30-min infliximab infusions. METHODS: Al

Effects of Low-Intensity Exercise in Older Adults With Chronic Heart Failure During the Transitional Period From Hospital to Home in China: A Randomized Controlled Trial.

The transitional period from hospital to home is critical to decreasing rates of preventable, poor post-discharge outcomes. Older adults with chronic heart failure (CHF) may be challenged to exercise during the transitional period. The purpose of the current study was to test the effects of low-intensity exercise on health-related quality of life (HRQoL), physical function, and heart function in Chinese older adults with CHF during the transitional period. The study was randomized, single-blinded, and controlled. Seventy-eight older adults with stable CHF were evaluated. The intervention group

Multidisciplinary Checklist for Safe Transitions to Home Total Parenteral Nutrition From Hospital-Based Palliative Care: A Case Study From Al Ain, Abu Dhabi.

We present a complex case of a multimorbid elderly patient admitted with septic shock, suspected to be secondary to aspiration pneumonia, who subsequently developed an intestinal obstruction due to an ileocecal junction mass. Despite conservative management, the patient's clinical status deteriorated and required comprehensive palliative care. This case highlights the challenges in managing patients with multimorbidities, the importance of a multidisciplinary approach, and the central role of palliative care in the setting of advanced disease. We demonstrate the effectiveness of the above meth
