BACKGROUND: Congenital cytomegalovirus (cCMV) affects 1 in every 200 United States infants, at present there are 9 states which mandate newborn cCMV screening. With more infants being diagnosed, more children will need continuing care from providers who are knowledgeable about cCMV.
OBJECTIVES: To examine pediatric provider knowledge, practices, and beliefs around cCMV.
METHODS: Primary care and newborn hospitalist pediatricians (N = 103) from Michigan, who "regularly care for infants" were invited to participate in a survey about their cCMV-related knowledge, clinical practices, and beliefs

BACKGROUND: Rotating medical consultants, hospitalists or geriatricians, are involved in the care of patients with hip fracture, often after medical complications have already occurred. In August 2012, we implemented a unique surgical comanagement (SCM) model in which the same Internal Medicine hospitalists are dedicated year-round to the orthopaedic surgery service. We examine whether this SCM model was associated with a decrease in medical complications, length of stay, and inpatient mortality in patients with hip fracture admitted at our institution, compared with the previous model.

Diabetic foot infections (DFIs) are common and represent the leading cause for hospitalization among diabetic complications. Without proper management, DFIs may lead to amputation, which is associated with a decreased quality of life and increased mortality. However, there is currently significant variation in the management of DFIs, and many providers fail to perform critical prevention and assessment measures. In this review, we will provide an overview of the diagnosis, management, and discharge planning of hospitalized patients with DFIs to guide hospitalists in the optimal inpatient care

IMPORTANCE: Antibiotic overuse is harmful to patients and the health care system. Antibiotic prescribing report feedback has been described in outpatient and long-term care settings but is relatively untested in inpatient settings.
OBJECTIVE: To assess the association between peer comparative inpatient antibiotic prescribing feedback and changes to antibiotic prescribing rates among hospitalists in a large health care network.
DESIGN, SETTING, AND PARTICIPANTS: This quality improvement study linked data on billing and prescribing of broad-spectrum antibiotics for hospital-onset infections (B

Fluoroquinolones are widely used antibiotics, however, concerns about QTc prolongation often limit their use in inpatient care. Ciprofloxacin, despite demonstrating minimal QTc effects and the lowest arrhythmic risk within its class, is frequently avoided due to non-specific electronic medical record warnings. This perspective reviews pharmacologic and real-world data to understand the real QTc risk, emphasizing ciprofloxacin's favorable cardiac safety profile. Aligning evidence with clinical practice may reduce unnecessary hospitalization, avoid prolonged intravenous therapy, and support safe

INTRODUCTION: Patients with hematologic malignancies frequently receive elective anticancer therapy as inpatients. The impact of introducing hospitalists on quality outcomes in this subset of patients is unknown.
METHODS: Patients with leukemia or lymphoma electively admitted for anticancer therapy to either a hematologist-led service (TS; n=59) or to a hospitalist-led service (HS; n=102) during two parallel 18-month time periods were included. Mixed linear regression models with first-order random effects for intercept (individual) and slope (time) were used to estimate the association betwe

A virtual hospitalist program expanded our ability to confront the challenges of the COVID-19 crisis at the epicenter of the pandemic in New York City. In concert with on-site hospitalists and redeployed physicians, virtual hospitalists aimed to expand capacity while maintaining high-quality care and communication. The program addressed multiple challenges created by our first COVID-19 surge: high patient census and acuity; limitations of and due to personal protective equipment; increased communication needs due to visitor restrictions and the uncertain nature of the novel disease, and limita

BACKGROUND: The National Academies' Improving Diagnosis in Health Care report outlined eight goals for improving the diagnostic process and building a culture of diagnostic excellence. Despite a decade passing since these recommendations, development of programs remains inconsistent.
OBJECTIVES: To explore diagnostic excellence improvement efforts across hospital medicine divisions in largely academic settings to identify opportunities.
METHODS: This study used rapid qualitative methods to analyze focus groups with hospitalist clinicians recruited from sites participating in an AHRQ-funded s

IMPORTANCE: The association of cardiopulmonary point-of-care ultrasonography (POCUS) with length of stay (LOS) and hospitalization costs for patients admitted to internal medicine wards remains uncertain.
OBJECTIVE: To evaluate a collaborative implementation model involving hospitalists, sonographers, and a remote cardiologist for integrating cardiopulmonary POCUS into the assessment of adult patients (≥18 years) hospitalized with undifferentiated dyspnea, and to assess its association with LOS and hospitalization costs.

OBJECTIVES: Pediatric hospitalists have increasingly been relied on to care for hospitalized patients. While hospitalist care models have shown many benefits, studies examining the transition from subspecialist to hospitalist-led teams are lacking. This study aimed to monitor the impact of transitioning an inpatient pediatric neurology service to a hospitalist model on several key outcome measures, with the hypothesis that these measures would be unaffected by the transition.
METHODS: In July 2019, the neurology service at a freestanding pediatric academic center transitioned a hospitalist mo

The purpose of this quality improvement project was to determine hospitalists' knowledge, practices, and perspectives related to management of pressure injuries and neuropathic/diabetic foot complications (having a foot ulcer or subsequent development of a foot infection because of a foot ulcer). We also sought to identify resources for and knowledge-based barriers to management of these wounds. This quality improvement effort targeted an interdisciplinary group of 55 hospitalists in internal medicine that consisted of 8 nurse practitioners, 10 physician assistants, and 38 physicians. IntroductionHospitalized patients with advanced cancer frequently experience high symptom burden, prognostic uncertainty, and complex decision-making needs. Although specialty palliative care improves patient-centered outcomes, access remains limited in many inpatient settings. Oncology hospitalists may be uniquely positioned to deliver elements of primary palliative care during acute hospitalizations. Communication is a core clinical skill in hospital medicine, yet many oncology hospitalists feel underprepared for breaking bad news, discussing prognosis and leading goals-of-care conversations

Background: Hospitalists, comprised of nurse practitioners and physician assistants (collectively, advanced practice providers [APPs]) and physicians, have opportunities to counsel patients and reduce SARS-CoV-2 related coronavirus disease 2019 (COVID-19) vaccine hesitancy. However, hospitalist perspectives on the COVID-19 vaccine and potential differences between APPs and physicians are unknown. Understanding hospitalist perspectives could help to address vaccine hesitancy among patients. Methods: We conducted an online survey of hospitalists at Mayo Clinic sites in Arizona, Florida, Minnesota

Exacerbations of chronic obstructive pulmonary disease (COPD) are associated with high rates of hospitalizations, costs, and morbidity. Therefore, hospitalists and the multidisciplinary team (hospital team) need to take a proactive approach to ensure patients are effectively managed from hospital admission to postdischarge. Comprehensive screening and diagnostic testing of patients at admission will enable an accurate diagnosis of COPD exacerbations, and severity, as well as other factors that may impact the length of hospital stay. Depending on the exacerbation severity and cause, pharmacothe

BACKGROUND: Cirrhosis and chronic liver disease carry appreciable morbidity and mortality. Cirrhotic patients frequently require hospitalization and their care is both extremely complex and labor-intensive.
AIM: We seek to provide a review for gastroenterologists, hepatologists, internists, and hospitalists on the approach to care in patients hospitalized for complications related to end-stage liver disease.
METHODS: The Mount Sinai Medical Center's inpatient liver service has developed an integrated team approach for cirrhotic patients and throughout the years has educated fellows-in-traini
