The association between hospital obstetric volume and perinatal outcomes in California.

OBJECTIVE: We sought to analyze the association between hospital obstetric volume and perinatal outcomes in California. STUDY DESIGN: This was a retrospective cohort study of births occurring in California in 2006. Hospitals were divided into 4 obstetric volume categories. Unadjusted rates of neonatal mortality and birth asphyxia were calculated for each category, overall and among term deliveries with birthweight >2500 g. Multivariable logistic regression was used to control for confounders. Deliveries in rural hospitals were analyzed separately using different volume categories. RESULTS: P

Hospital Birth Volume and Neonatal Resuscitation Prior to Therapeutic Hypothermia.

BACKGROUND AND OBJECTIVE: Clinicians working in hospitals with low birth volumes lack regular opportunities to maintain neonatal resuscitation skills, which may impact resuscitation practices in these settings. We assessed the impact of birth volume on resuscitation practices using data from an existing database of neonates treated with therapeutic hypothermia. METHODS: We conducted a retrospective, multicenter review of neonates treated with therapeutic hypothermia at 4 Northern New England hospitals from 2009 to 2024. Overall, 561 neonates were identified; neonates younger than 35 weeks' ge

Center Volume and Failure to Rescue in Pediatric Cardiac Surgery: Is There a Relationship?

BACKGROUND: Failure to rescue (FTR) is a validated quality metric for evaluating outcomes in pediatric cardiac surgery. Using a national administrative database, we sought to assess the relationship between annual hospital case volume and FTR. METHODS: Cardiopulmonary bypass (CPB) cases were extracted from the Kids' Inpatient Database for 2016 and 2019. FTR was defined as inpatient mortality of patients with at least 1 postoperative complication. Hospitals were divided into high-, mid-, and low-volume hospitals on the basis of annual CPB case volume terciles. A mixed effects multivariable log

The Need for Centralization for Small Intestinal Neuroendocrine Tumor Surgery: A Cohort Study from the GTE-Endocan-RENATEN Network, the CentralChirSINET Study.

BACKGROUND: The concept of surgical centralization is becoming more and more accepted for specific surgical procedures. OBJECTIVE: The aim of this study was to evaluate the relationship between procedure volume and the outcomes of surgical small intestine (SI) neuroendocrine tumor (NET) resections. METHODS: We conducted a retrospective national study that included patients who underwent SI-NET resection between 2019 and 2021. A high-volume center (hvC) was defined as a center that performed more than five SI-NET resections per year. The quality of the surgical resections was evaluated betwee

Is there a relationship between hospital volume and patient outcomes in gastroschisis repair?

PURPOSE: Given the well-established relationship between surgical volume and outcomes for many surgical procedures, we examined whether the same relationship exists for gastroschisis closure. METHODS: We conducted a retrospective analysis of infants who underwent gastroschisis closure between 1999 and 2007 using a California birth-linked cohort. Hospitals were divided into terciles based on the number of gastroschisis closures performed annually. Using regression techniques, we examined the effects of hospital volume on patient mortality and length of stay while controlling for patient and ho

Hospital volume stratification using stratum-specific likelihood ratios for pancreatectomy.

BACKGROUND: Recent literature suggests that volume-outcome associations for pancreatectomy have attenuated over time, leading some researchers to question the continued relevance of volume thresholds. However, this perceived attenuation may reflect the methodological limitations of single, binary cutoffs rather than a true weakening of the underlying relationship. Stratum-specific likelihood ratios (SSLRs) generate multiple empirically derived volume strata that may detect persistent gradients that are obscured by binary stratification. The objectives of this study were to (i) define volume st

The Impact of Hospital Volume on Postoperative Outcomes for Esophagectomy and Gastrectomy: A Systematic Review and Meta-analysis.

BACKGROUND: High-volume hospitals tend to have better outcomes in complex surgeries, but outcome variations and volume thresholds complicate conclusions. This systematic review/meta-analysis assessed the impact of hospital volume on postoperative outcomes after esophagogastric surgery and identified volume thresholds beyond which benefits plateau. METHODS: PubMed/MEDLINE was searched for cohort studies (2013-2023) on volume and outcomes after esophagectomy/gastrectomy for cancer. Primary outcome was 30-day mortality. Secondary outcomes included 90-day mortality, complications, length of stay

Evidence-based thresholds for the volume-value relationship in shoulder arthroplasty: outcomes and economies of scale.

BACKGROUND: Whereas several studies suggest that high-volume surgeons and hospitals deliver superior patient outcomes with greater cost efficiency, no evidence-based thresholds separating high-volume surgeons and hospitals from those that are low or medium volume exist in shoulder arthroplasty. The objective of this study was to establish meaningful thresholds that take outcomes and cost into consideration for surgeons and hospitals performing shoulder arthroplasty. METHODS: Using 9546 patients undergoing primary shoulder arthroplasty for osteoarthritis from an administrative database, we cre

Effects of Extracorporeal Life Support Volume on Neonatal Mortality in Congenital Diaphragmatic Hernia: A Retrospective Cohort Study.

We conducted a retrospective cohort study of 29 499 neonatal cases requiring extracorporeal life support (ECLS) at 237 centers from 2000 to 2023; there were 6747 infants with congenital diaphragmatic hernia (CDH) and 22 752 without CDH. Infants with CDH treated at ECLS centers with low total neonatal volumes (<28.5 total cases) had increased odds of inpatient mortality compared with those at high-volume centers (> 140 total cases, adjusted odds ratio [aOR]: 1.9; 95% confidence interval [CI]: 1.2-2.9). Similarly, centers with low non-CDH ECLS volumes were associated with higher odds of mortalit

Do hospital and surgeon volumes impact the outcomes of patients undergoing cholecystectomy in Ireland? A national registry based study.

INTRODUCTION: Biliary pathology can have an enormous burden on both the patient and the health service with acute cholecystitis being diagnosed in 10 % of patients that present with acute abdominal pain. Emergency or elective laparoscopic cholecystectomy is the recommended treatment option for these patients. It has been shown previously that surgical and hospital volume affect outcomes from emergency abdominal surgery in Ireland, but no such investigation has been done specifically for cholecystectomies. AIM: To determine if there is a link between the hospital and surgeon volume of cholecys
