Changes in Nonprofit Hospitals' Finances, Operations, and Quality of Care After Using Management Consultants.

IMPORTANCE: The presence of management consultants in the US health care industry has increased dramatically in recent decades and is now higher than in most other sectors of the US economy. Hospitals hire management consultants to provide external expertise and advise on strategic planning, organizational change, cost cutting, and revenue-enhancement activities. Despite the prominence and influence of these firms, there is no empirical evidence in the US documenting either how much is spent on these services or whether that spending leads to measurable improvements. OBJECTIVE: To quantify no

Factors predictive of changes in the legal status of psychiatric inpatients.

The objective of this study was to identify factors predictive of duration of involuntary hospitalization, i.e., factors that would predict early versus late conversion to voluntary status. Charts of 209 patients admitted involuntarily to an acute psychiatric inpatient unit were studied using a naturalistic, prospective design. Multivariate regression analysis was used to determine the effect of a number of variables in predicting duration of involuntary hospitalization. Of all variables studied, only smoking preference was found to be a statistically significant predictor of duration of invol

Coerced hospital admission and symptom change--a prospective observational multi-centre study.

INTRODUCTION: Coerced admission to psychiatric hospitals, defined by legal status or patient's subjective experience, is common. Evidence on clinical outcomes however is limited. This study aimed to assess symptom change over a three month period following coerced admission and identify patient characteristics associated with outcomes. METHOD: At study sites in 11 European countries consecutive legally involuntary patients and patients with a legally voluntary admission who however felt coerced, were recruited and assessed by independent researchers within the first week after admission. Mutual blood donation is safer at small blood collection stations in China.

BACKGROUND: The assumption that the level of safety of voluntary non-remunerated donors is significantly higher than that of family replacement donors is supported by global data without stratifying for first-time or repeat volunteer, or according to age, but the viral marker prevalence between replacement donors and first-time voluntary non-remunerated donors is similar. MATERIALS AND METHODS: From 2006 to 2013, replacement and voluntary donors were respectively recruited by the hospitals and the Center Blood Station in Zhaoqing, Guangdong, according to the existing procedures, and all the d

Utilization of low-volume hospitals for total hip replacement.

OBJECTIVE: To identify factors associated with utilization of low-volume hospitals for total hip replacement (THR) and to estimate differences in the distances that Medicare beneficiaries had to travel to reach low- or high-volume hospitals. METHODS: We studied a population-based sample of 1,146 Medicare beneficiaries who underwent elective THR in 1995. Using multiple data sources including medical record review, Medicare claims data, 1990 Census data, and a patient survey, we examined factors independently associated with utilization of low-volume hospitals for elective THR. We estimated the

States turn up the heat. As self-regulation sputters, state legislatures are leading charge for more accountability in governance of not-for-profit hospitals.

Recent scandals have pushed some states to tackle transparency and governance issues at healthcare not-for-profits, which the federal government has been reluctant to touch. Rhode Island Lt. Gov. Charles Fogarty, left, supports a bill in his state that would establish a code of ethics for hospital boards. Fogarty sees leaders as getting too comfortable in their jobs and working without accountability.

Health care for the poor: some policy alternatives.

Changes in the financing and organization of medical care are most likely to affect adversely the poor who have significant needs for care, but face increasingly stringent eligibility criteria in Medicaid and other public programs. Americans estimated to have neither private nor public health insurance coverage number 33 million persons, and with increased cost pressures, voluntary and proprietary hospitals are less willing to treat such patients. One quarter of hospitals provide 60 percent of all care to the poor, and many of these nonprofit and public hospitals face economic difficulties and

The Importance of Using Multiple Safety Evaluation Systems in Hospitals: A Comparison of Critical Incident Reporting System (CIRS), Assurance of Quality in Surgery (AQC), and the Global Trigger Tool (GTT).

BACKGROUND: Accurate detection of adverse events (AEs) and patient harm is fundamental for improving patient safety in hospitals. The primary objective of this study is to evaluate the sensitivity of the Institute for Healthcare Improvement (IHI), the Global Trigger Tool (GTT) in detecting adverse events compared with the Assurance of Quality in Surgery (AQC) surgical database and the voluntary Critical Incident Reporting System (CIRS) in a Swiss hospital setting. Specifically, we aim to quantify the detection gap between these 3 systems. METHODS: A retrospective review was conducted over a 1

Organ procurement after euthanasia: Belgian experience.

Euthanasia was legalized in Belgium in 2002 for adults under strict conditions. The patient must be in a medically futile condition and of constant and unbearable physical or mental suffering that cannot be alleviated, resulting from a serious and incurable disorder caused by illness or accident. Between 2005 and 2007, 4 patients (3 in Antwerp and 1 in Liège) expressed their will for organ donation after their request for euthanasia was granted. Patients were aged 43 to 50 years and had a debilitating neurologic disease, either after severe cerebrovascular accident or primary progressive multi

COVID-19 impact on Nigeria's national blood service commission - Lessons for Low- and Middle-Income Countries (LMICs).

BACKGROUND: In February 2020, Nigeria officially announced its first case of COVID-19. As numbers rose, government-led non-pharmaceutical interventions such as lockdowns, curfews, restrictions on mass gatherings and other physical distancing measures ensued, negatively affecting blood donor mobilisation activities. OBJECTIVES: We aimed to assess the blood service activities across 17 National Blood Service Commission (NBSC) centres in Nigeria, including number of blood donations, mobile blood drives, blood units screened, screening outcomes, number of hospitals NBSC provided services to and n
