Microbial and immunological investigations and remedial action after an outbreak of humidifier fever.

ABSTRACT Humidifier fever (Monday sickness) occuring in office staff in a factory processing rayon presented as pyrexia with a polyuria and leucocytosis on the first day back to work after a break during the winter half of the year. Chest radiographs showed no abnormalities but pulmonary function tests indicated mild airways obstruction in the affected group as a whole. Respirable dust samples taken on a Monday when 11 cases occured were not pyrogenic, indicating that a mechanism other than direct pyrogen activity produced the pyrexia. Efforts were then directed to determining an immunological

Potential in-hospital modes of transmission of Legionella pneumophila. Demonstration experiments for dissemination by showers, humidifiers, and rinsing of ventilation bag apparatus.

The mode of transmission of nosocomial legionellosis remains uncertain. Aerosolization of Legionella pneumophila by showers, humidifiers, and respiratory equipment rinsed in tap water was evaluated using plate-settling culture and air aspirator methods. All protocols simulated the actual hospital setting including use of humidifier equipment used in hospital patient rooms and water from faucets and showerheads in hospitals with nosocomial Legionnaires' disease. Protocols for humidifier and shower experiments mimicked the procedure actually used in hospitals by health care personnel. Showering

Acid-humidified CO2 gas input for stable electrochemical CO2 reduction reaction.

(Bi)carbonate salt formation has been widely recognized as a primary factor in poor operational stability of the electrochemical carbon dioxide reduction reaction (CO2RR). We demonstrate that flowing CO2 gas into an acid bubbler-which carries trace amounts of acid vapor into a gas diffusion electrode for silver-catalyzed CO2RR to carbon monoxide (CO)-can prevent salt accumulation. In a 100-square-centimeter, scaled-up CO2RR membrane electrode assembly electrolyzer with single serpentine flow channels, the acid humidification method achieved the 4500 hours of stability milestone at 100 mA cm-2

Recurrent episodes of febrile dyspnoea: hypersensitivity pneumonitis caused by a household ultrasonic humidifier.

Hypersensitivity pneumonitis is an immune-mediated interstitial lung disease that presents with respiratory symptoms, with or without systemic symptoms, following exposure to an identified or unidentified external factor. It can be caused by extrinsic factors including household items such as ultrasonic humidifiers. We present an intriguing case of a previously healthy 50-year-old man who displayed recurrent episodes of progressive dyspnoea and fever after repeated exposure to his household ultrasonic humidifier. He was treated with corticosteroids, followed by the removal of the humidifier, re

Characteristics of the Molecular Weight of Polyhexamethylene Guanidine (PHMG) Used as a Household Humidifier Disinfectant.

(1) Background: Household humidifier disinfectant (HD) brands containing polyhexamethylene guanidine (PHMG) have been found to cause the most HD-associated lung injuries (HDLIs) in the Republic of Korea. Nevertheless, no study has attempted to characterize the potential association of the health effects, including HDLI, with the physicochemical properties of PHMG dissolved in different HD brands. This study aimed to characterize the molecular weight (MW) distribution, the number-average molecular weight (Mn), the weight-average molecular weight (Mw), and the structural types of PHMG used in HD

The Safety and Efficacy of Transnasal Humidified Rapid-Insufflation Ventilatory Exchange for Laryngologic Surgery.

OBJECTIVES: Transnasal humidified rapid-insufflation ventilatory exchange (THRIVE) is an intraoperative ventilatory technique that allows avoidance of tracheal intubation (TI) or jet ventilation (JV) in selected laryngologic surgical cases. Unimpeded access to all parts of the glottis may improve surgical precision, decrease operative time, and potentially improve patient outcomes. The objective of this prospective, randomized, patient-blinded, 2-arm parallel pilot trial was to investigate the safety and efficacy of THRIVE use for adult patients undergoing nonlaser laryngologic surgery of shor

Effects of Intraoperative Insufflation With Warmed, Humidified CO2 during Abdominal Surgery: A Review.

PURPOSE: During a laparotomy, the peritoneum is exposed to the cold, dry ambient air of the operating room (20°C, 0%-5% relative humidity). The aim of this review is to determine whether the use of humidified and/or warmed CO2 in the intraperitoneal environment during open or laparoscopic operations influences postoperative outcomes. METHODS: A review was performed in accordance with PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. The PubMed, OVID MEDLINE, Cochrane Central Register of Controlled Trials and Embase databases were searched for articles pub

Benefits of the humidified low-flow oxygen therapy in infants with mild-moderate bronchiolitis.

AIMS AND OBJECTIVES: To investigate the clinical benefits of using humidification in low-flow oxygen therapy. Specific objectives were to investigate via an assessment of the number of nasal lavages whether humidification can help to decrease the nasal mucus viscosity, to determine whether it can relieve feeding difficulties by comparing the weight gain in infants, to ascertain whether it can relieve respiratory distress by assessing the heart and respiratory rates and contribute to improved clinical outcomes, measured by the length of stay and oxygen requirements. BACKGROUND: There is no evi

Use of Heated Humidified Gases for Early Stabilization of Preterm Infants: A Meta-Analysis.

Background: Large observational studies in preterm infants have shown an increase in mortality and morbidity when admission temperature is below 36.5°C. Recent randomized controlled studies have shown a reduction in admission hypothermia and an increase in the number of infants admitted with normal temperature (36.5-37.5°C) when heated humidified gases were used for initial stabilization of preterm infants. Objective: The goal of this study was to perform a meta-analysis of published randomized trials using heated humidified gas compared to cold dry gas in preterm infants immediately after bir

Humidifier Disinfectant-Associated Lung Injury: Six Years after the Tragic Event.

In 2011, a cluster of peripartum patients were admitted to the intensive care unit of a tertiary hospital in Seoul with signs and symptoms of severe respiratory distress of unknown etiology. Subsequent epidemiological and animal studies suggested that humidifier disinfectant (HD) might represent the source of this pathology. Epidemiological studies, animal studies, and dose-response analysis demonstrated a strong association between HD use and lung injuries. The diagnostic criteria for HD-associated lung injury (HDALI) was defined on the basis of the clinical, pathological, and radiological at

Laparotomy causes loss of peritoneal mesothelium prevented by humidified CO2 insufflation in rats.

INTRODUCTION: Avoiding tissue desiccation is a common recommendation to reduce postoperative complications following open abdominal surgery, although difficult to achieve delicately without damaging the peritoneal mesothelium. Insufflation of humidified-warm CO2 into the abdomen during open abdominal surgery is proposed as an invisible, effortless way to prevent desiccation. We hypothesized that desiccation during open abdominal surgery would cause loss of peritoneal mesothelium that would be prevented by insufflation of humidified-warm CO2. METHODS: Nine Wistar rats were assigned to 1 h of a

[Clinical evaluation of an oxygen concentrator and humidifier that does not require additional reservoir water].

A conventional humidifier with a reservoir of water for humidification can produce micro-aerosols contaminated with bacteria. The present study was undertaken to determine the clinical efficiency of a membrane humidifier that does not require additional reservoir water. We analyzed relative room air humidity and oxygen levels obtained from 2 pressure-swing adsorption (PSA)-type oxygen concentrators with membrane humidifiers. A significant correlation was found between relative room air humidity and that of oxygen moistened by a membrane humidifier. Several patients with chronic respiratory fai

Effect of a heated humidifier during continuous positive airway pressure delivered by a helmet.

INTRODUCTION: The helmet may be an effective interface for the delivery of noninvasive positive pressure ventilation. The high internal gas volume of the helmet can act as a 'mixing chamber', in which the humidity of the patient's expired alveolar gases increases the humidity of the dry medical gases, thus avoiding the need for active humidification. We evaluated the temperature and humidity of respiratory gases inside the helmet, with and without a heated humidifier, during continuous positive airway pressure (CPAP) delivered with a helmet. METHODS: Nine patients with acute respiratory failu

Evaluation of the stability of aspirin in solid state by the programmed humidifying and non-isothermal experiments.

The influence of both moisture and heat on the stability of aspirin was investigated by a single pair of experiments, one with programmed humidity control and the other non-isothermal, rather than many standard isothermal studies, each at constant relative humidity. In experiments, we adopted the acid-base back titration method to measure the content of aspirin in the presence of its degradation products. It was found that the degradation of aspirin could be expressed as ln[(c0-c)/c]=kt+D, where D was a lag time item not related to humidity and temperature. The relationship between the degrada

Identification of fungi causing humidifier lung: 2 rare cases and a review of the literature.

BACKGROUND: Humidifier lung (HL) is a hypersensitivity pneumonitis resulting from exposure to humidifiers, with fungi from the humidifier as one of the etiologic agents. However, identification of the fungal species responsible for each case can be challenging because of difficulties in culturing fungi, their accurate identification, and interpreting the results of specific serum IgG testing. OBJECTIVE: To clarify the best way to determine the causative fungal species of each HL case. METHODS: We report 2 cases with HL in which rare fungi were identified as causative agents. In addition, we
