Use of methadone in a highly tolerant patient receiving parenteral hydromorphone.

A 59-year-old woman with metastatic breast cancer experienced a poor response to increasing doses of hydromorphone, possibly related to the neuropathic nature of her pain. Ultimately, the cost of this treatment was more than $1000 per day. Administration of methadone, initially as an adjuvant and eventually as the sole analgesic, at a much smaller dose than expected resulted in very satisfactory analgesia, without the development of serious side effects. In addition, the cost of treatment decreased to less than $25 per day. This case illustrates the rationale and advantages for the use of meth

Differential naltrexone antagonism of hydromorphone and pentazocine effects in human volunteers.

Naltrexone is more potent in blocking mu receptor activity than kappa receptor activity. This study examined the interactions of naltrexone with the mu agonist, hydromorphone, and with the mixed agonist-antagonist, pentazocine, to assess the apparent receptor mechanisms of their effects. After oral, 2-hr pretreatment with placebo or naltrexone (12.5 or 25 mg), a within-session cumulative dosing procedure (four intramuscular injections at 1-hr intervals) was used to assess the effects of hydromorphone (0, 0.75, 1.5 and 3 mg) and pentazocine (0, 15, 30 and 60 mg) vs. saline placebo. Adverse effects in hospice patients with chronic kidney disease receiving hydromorphone.

BACKGROUND: Pain is one the most common symptoms experienced by palliative care patients. The treatment of pain involves the use of strong opioids such as hydromorphone, morphine, methadone, fentanyl, oxycodone, oxymorphone, or levorphanol for moderate to severe pain. Hydromorphone is metabolized by the liver to hydromorphone-3-glucuronide (H3G), a compound that can potentially cause neuroexcitatory phenomena with accumulation. Pharmacokinetic studies have shown that H3G levels in patients with renal insufficiency are 4 times as high as those with normal renal function; however, reports have b

Antitumor Effects of Hydromorphone on Human Gastric Cancer Cells in vitro.

Источник пометил публикацию как отозванную

INTRODUCTION: Experimental data indicate that morphine and fentanyl may have antitumor effects in gastric cancer cells (GC). Hydromorphone, as an analgesic, is used against refractory cancer pain in recent years. However, the data on hydromorphone influencing the biological characteristics of human gastric cancer cells are lacking. The aim of this study was to investigate how hydromorphone affected the growth of human gastric cancer in vitro. MATERIAL AND METHODS: Human GC cell lines (HGC-27, MGC-803, AGS and SGC-7901) and human gastric epithelial cells GSE-1 were exposed to various concentra

Intranasal hydromorphone for treatment of acute pain in children: A pilot study.

OBJECTIVES: We aimed to describe the analgesic efficacy, duration of analgesia, and adverse event profile associated with intranasal hydromorphone in children with acute pain presenting to an emergency department. METHODS: Prospective dose titration pilot study of otherwise healthy children 4 to 17-years-old with moderate to severe pain who required a parenteral opioid. All patients received an initial intranasal hydromorophone dose of 0.03 mg/kg. The need for additional analgesia was assessed at 15 and 30 min; an additional 0.015 mg/kg was given at each assessment, if required. Need for resc

Use of hydromorphone (Dilaudid) and morphine for patients with hepatic and renal impairment.

Management of pain in the elderly is very challenging. First, the compromised ability to perceive pain because of loss of sensory neurons and other comorbid conditions such as dementia and degenerative joint diseases make the assessment of severity source and localization of pain very difficult. Second, decrease in the renal and hepatic blood flow and decrease in the hepatic and renal mass, along with decrease in volume of distribution caused by decrease in total body water and hypoproteinemia, makes the elderly very sensitive to adverse effects of different pain medicines. Third, many elderly

Hydromorphone levels and pain control in patients with severe chronic pain.

To better understand the use of narcotic analgesics, the hydromorphone concentration was measured in serum samples from 43 patients with chronic severe pain who were receiving this drug. At the time of blood sampling, pain intensity, mood, and cognitive performance were assessed. There was large individual variation in the dose-drug level relationship. Seven patients with bone or soft tissue pain and drug levels of greater than or equal to 4 ng/ml had good pain control, whereas 10 did not. None of 15 patients with levels less than 4 ng/ml had pain control, despite drug doses similar to those g

Intrathecal hydromorphone for analgesia after partial hepatectomy: a randomized controlled trial.

INTRODUCTION: There is substantial interest in adding intrathecal opioids, such as hydromorphone to the multimodal pain management strategies. We conducted a randomized controlled trial to examine whether adding intrathecal hydromorphone to a multimodal strategy could safely improve analgesic efficacy for patients undergoing partial hepatectomy. METHODS: A total of 126 adult patients undergoing partial hepatectomy under general anesthesia were randomly assigned to receive intrathecal hydromorphone (100 μg) or no block. The primary outcome was the incidence of moderate to severe pain during mo

Comparison of dezocine and hydromorphone in post-cesarean epidural analgesia: a retrospective study.

BACKGROUND: Postoperative pain management following cesarean delivery is essential for optimizing maternal recovery and reducing complications. Patient-controlled epidural analgesia (PCEA) is widely used; however, the choice of opioid remains debated due to variations in effectiveness and side effect profiles. This study aims to compare the analgesic effectiveness and adverse effects of dezocine and hydromorphone in PCEA regimens for cesarean delivery. METHODS: In this retrospective study, 1,582 parturients who underwent cesarean delivery under neuraxial anesthesia and received postoperative

Circulating miRNA Signature as a Potential Biomarker for the Prediction of Analgesic Efficacy of Hydromorphone.

No practical biomarkers currently exist for the prediction of the analgesic efficacy of opioids. Previously, we reported circulating miRNA signatures differentially regulated by µ-opioid receptor (MOR) agonists in healthy subjects. We hypothesized that these miRNAs could be potential pharmacodynamic biomarkers to estimate MOR stimulation, and predict the efficacy of opioids; i.e., patients with low MOR stimulation may be more vulnerable to strengthening of the MOR signal upon hydromorphone treatment. To test this hypothesis, plasma samples were obtained from 25 patients with cancer pain prior

The Conversion Ratio From Intravenous Hydromorphone to Oral Opioids in Cancer Patients.

CONTEXT: The lack of knowledge of the accurate conversion ratio (CR) between intravenous (IV) and oral hydromorphone and opioid rotation ratio (ORR) between IV hydromorphone and oral morphine equivalent daily dose (MEDD) may lead to poorly controlled pain or overdosing in cancer inpatients. OBJECTIVES: We aimed to determine the CR and ORR from IV hydromorphone to oral hydromorphone and MEDD (obtained from oral morphine and oxycodone). METHODS: A total of 4745 consecutive inpatient palliative care consults during 2010-14 were reviewed for conversions from IV hydromorphone to oral hydromorphon

Bioavailability of morphine, methadone, hydromorphone, and oxymorphone following buccal administration in cats.

Buccal administration of buprenorphine is commonly used to treat pain in cats. It has been argued that absorption of buprenorphine through the buccal mucosa is high, in part due to its pKa of 8.24. Morphine, methadone, hydromorphone, and oxymorphone have a pKa between 8 and 9. This study characterized the bioavailability of these drugs following buccal administration to cats. Six healthy adult female spayed cats were used. Buccal pH was measured prior to drug administration. Morphine sulfate, 0.2 mg/kg IV or 0.5 mg/kg buccal; methadone hydrochloride, 0.3 mg/kg IV or 0.75 mg/kg buccal; hydromor

Hydromorphone does not influence the immune response in patients affected by chronic pain.

Hydromorphone (HMP) is a semisynthetic opioid that has been widely used over many years in the treatment of chronic moderatesevere pain. A number of studies have demonstrated the use of HMP in patients with cancer pain as safe and affective, and more recently HMP has been also utilised in the treatment of chronic degenerative pain (CDP). Then, some opioids are able to influence several immune parameters. Aim of our study was to assess HMP in patients with CDP in terms of safety and efficacy, to evaluate effects of HMP, within 2 months of observation, on cutaneous cell-mediated immunity (CCMI)
