The role of β-hydroxybutyrate testing in ketogenic metabolic therapies.

Accurate assessment of dietary adherence and metabolic outcomes remains a critical challenge in most nutrition studies. Ketogenic metabolic therapies (KMTs) provide a unique advantage by inducing nutritional ketosis and enabling the use of ketone bodies as biomarkers of metabolic state. This narrative review investigates the role of ketone testing as an integral component of KMTs. We introduce the key biomarkers and testing modalities currently used and present a comprehensive overview of the use of capillary blood β-hydroxybutyrate (BHB) testing across diverse therapeutic areas. Current Insights on the Impact of Gamma-Hydroxybutyrate (GHB) Abuse.

Recreational gamma-hydroxybutyrate (GHB) use, although less common than other substance use, is increasingly recognised and is over-represented in emergency toxicology presentations. This narrative review summarizes GHB pharmacology, current patterns of use, potential harms and management of GHB toxicity and withdrawal. There is a complex interplay between GHB and GABA as GHB is both a prodrug and metabolite of GABA and GHB activates both GHB and GABA receptors. GHB is rapidly absorbed, with effects seen within minutes of ingestion. Metabolism is non-linear at higher doses. While GHB is listed

Gamma-hydroxybutyrate abuse: pharmacology and poisoning and withdrawal management.

Gamma-hydroxybutyrate (GHB) is a central nervous system depressant primarily used as a recreational drug of abuse, but also for the treatment of narcolepsy with cataplexy in adult patients and as an adjuvant for control of alcohol withdrawal syndrome. The main aim of this review is to summarise updated knowledge about GHB pharmacokinetics and pharmacodynamics, acute poisoning, and clinical features of GHB withdrawal syndrome, its diagnosis and medical treatment. The most common clinical signs and symptoms of acute poisoning include sleepiness to deep coma, bradycardia, hypotension, and respira

Postmortem vitreous beta-hydroxybutyrate: interpretation in a forensic setting.

Vitreous beta-hydroxybutyrate (BHB) was retrospectively analyzed in 1795 forensic cases using the Pointe Scientific method. Comparison of vitreous BHB with vitreous glucose in 1781 of the cases showed moderately good correlation r = 0.731. Comparison with blood alcohol levels in 1561 of the cases showed no correlation r = -0.053. Vitreous BHB was a marker of diabetic ketoacidosis when above 6.0 mM with a vitreous glucose over 200 mg/dL. It was an indicator (>50%) for alcoholic ketoacidosis when above 6.0 mM with a vitreous glucose below 200 mg/dL. Recommendations for interpretation of vitreous

Hazardous use of gamma hydroxybutyrate: driving under the influence.

Focus group discussions elicited descriptive experiences of driving under the influence of gamma hydroxybutyrate (GHB), and uncovered motivations that led participants to decide whether to get behind the wheel after ingesting this illegal psychoactive substance. Of the 51 current and past users interviewed, average age 31.1 +/- 7.7 years, 40% were female. All were recruited from the San Francisco Bay Area, in 2004. Factors making users vulnerable to adverse complications of driving while under the influence of GHB are also examined. Study limitations were noted. Implications for various law en

Successful management of prolonged gamma-hydroxybutyrate and alcohol withdrawal.

A 27-year-old man was admitted with tremulousness, diaphoresis, tachypnea (28 breaths/min), full-body rigidity, irritability, paranoia, and auditory and visual hallucinations 2 days after stopping long-term gamma-hydroxybutyrate (GHB) and 8 hours after stopping alcohol intake. He received intravenous fluids and tapering dosages of lorazepam to control agitation and rigidity, and recovered with no significant sequelae after 8 days. Abrupt cessation of GHB after high-dosage abuse can precipitate a clinically significant withdrawal syndrome. Lorazepam should be considered for treatment of GHB wit

[EEG changes during sedation with gamma-hydroxybutyric acid].

Gamma-hydroxybutyric acid (GHB) is a naturally occurring transmitter in the mammalian brain, related to sleep regulation and possibly to energy balance in diving or hibernating animals. It has been used for almost 35 years as an intravenous agent for induction of anaesthesia and for long-term sedation. Its convincing pharmacological properties, without serious adverse effects on circulation or respiration, are compromised by its unpredictable duration of action. This is not a major problem with long-term sedation during ICU treatment. GHB has been used with good results for sedation of patient

Ontogeny of gamma-hydroxybutyric acid. II. Electroencephalographic effects.

The ontogeny of petit mal-like seizures induced by gamma-hydroxybutyrate (GHB) was investigated. The prodrug of GHB, gamma-butyrolactone (GBL) was administered in varying dosages under continuous EEG monitoring from cortical and depth electrodes to rats varying in postnatal age from 1 to 85 days. The brain pharmacokinetics of GHB were determined at various ages as was the effect of ethosuximide on GBL-induced EEG changes. In adult rats, GBL produced a predictable sequence of electrical events beginning with spike bursts and progressing to polyspiking separated by low voltage activity. In 1-day

gamma-Hydroxybutyrate: cerebral metabolic, vascular, and protective effects.

The cerebral protection afforded by each of several preparations of gamma-hydroxybutyrate (GHB) was examined in the hypoxic (FiO2 = 0.05) mouse model. The greatest increase in survival time (85%) occurred after pretreatment with 300 mg/kg given as buffered gamma-butyrolactone (GBL). Compared with previous studies employing the same hypoxic model, this increase was less than that observed with certain barbiturates and equal to that observed with certain anesthetics. The cerebral and systemic metabolic and vascular effects of each of several preparations of GHB were examined in a canine model. T

The novel sustained 3-hydroxybutyrate donor poly-D-3-hydroxybutyric acid prevents inflammatory bowel disease through upregulation of regulatory T-cells.

Inflammatory bowel disease (IBD) is a chronic persistent intestinal disorder, with ulcerative colitis and Crohn's disease being the most common. However, the physio-pathological development of IBD is still unknown. Therefore, research on the etiology and treatment of IBD has been conducted using a variety of approaches. Short-chain fatty acids such as 3-hydroxybutyrate (3-HB) are known to have various physiological activities. In particular, the production of 3-HB by the intestinal microflora is associated with the suppression of various inflammatory diseases. In this study, we investigated wh

Administration of gamma-hydroxybutyrate instead of beta-hydroxybutyrate to a liver transplant recipient suffering from propionic acidemia and cardiomyopathy: A case report on a medication prescribing error.

Beta-hydroxybutyrate (BHB) is a synthetic ketone body used as an adjuvant energy substrate in the treatment of patients with metabolic cardiomyopathy. A medication prescribing error led to the administration of the general anesthetic sodium gamma-hydroxybutyrate (GHB) instead of sodium BHB in a liver transplant recipient with propionic acidemia and cardiomyopathy, causing acute coma. A 15-year-old boy suffering from neonatal propionic acidemia underwent liver transplantation (LT) for metabolic decompensation and cardiomyopathy (treated with cardiotropic drugs and BHB) diagnosed a year previous

4-Hydroxybutyrate (4HB) released from poly-4-hydroxybutyrate scaffolds does not impact hallmark phenotypes of cancer in malignant or non-malignant breast cells.

BACKGROUND: Poly-4-hydroxybutyrate (P4HB) scaffolds are increasingly used to reinforce soft tissue during implant-based reconstruction after mastectomy. P4HB undergoes hydrolytic degradation to a natural metabolite, 4-hydroxybutyrate (4HB). Understanding the direct effects of 4HB on cancer cells is essential for assessing the oncologic safety of P4HB scaffolds used in breast reconstruction surgery. The aim of this study was to evaluate the effects of sodium 4-hydroxybutyrate (Na4HB) on multiple, relevant human breast cancer and epithelial cell types using a panel of well-established in vitro a

Unveiling the role of CB2 receptor in beta-hydroxybutyrate mediated modulation of.

The cannabinoid receptor type 2 (CB2R), primarily expressed in microglia, the brain's resident immune cells, acts as a central regulator of neuroinflammatory responses. When CB2R is activated, it triggers anti-inflammatory signaling, making it a promising target for modulating microglial function in neuroinflammatory diseases. The ketone body, β-hydroxybutyrate (BHB), is gaining attention as a therapeutic agent for neurodegenerative disorders due to its ability to modulate neuroinflammation and preserve blood-brain barrier integrity. One mechanism by which BHB exerts anti-inflammatory effects

Barbiturates for the Management of Gamma-Hydroxybutyrate (GHB) Withdrawal: A Systematic Review.

Gamma-hydroxybutyrate (GHB) withdrawal is an uncommon but potentially severe syndrome which may not respond to benzodiazepines, with no clear guidelines for treatment escalation. This systematic review examines the evidence of barbiturates as alternative or adjunct therapies in managing GHB withdrawal. PubMed was searched for articles published from 1964-2025 using terms related to GHB, its precursors, withdrawal, and barbiturates. Two independent reviewers screened articles, with a third resolving discrepancies. English-language primary reports involving barbiturate use for GHB withdrawal in

Gamma-hydroxybutyrate poisoning: clinical diagnosis versus laboratory findings.

INTRODUCTION: Patients poisoned with gamma-hydroxybutyrate may need urgent medical treatment. The clinical manifestations are heterogeneous, and the level of consciousness is often unstable, with fluctuations between agitation and coma. We aimed to investigate the accuracy of the clinical diagnosis of gamma-hydroxybutyrate poisoning compared to laboratory findings in blood samples. METHODS: We did a prospective observational study, including patients ≥16 years of age admitted to hospital with a clinical diagnosis of gamma-hydroxybutyrate poisoning. The diagnosis was established by the doctor
