Structural neuroanatomy of tinnitus and hyperacusis in semantic dementia.

INTRODUCTION: Tinnitus and hyperacusis are common symptoms of excessive auditory perception in the general population; however, their anatomical substrates and disease associations continue to be defined. PATIENTS: with semantic dementia (SemD) frequently report tinnitus and hyperacusis but the significance and basis for these symptoms have not been elucidated. METHODS: 43 patients with a diagnosis of SemD attending a specialist cognitive disorders clinic were retrospectively studied. 14 patients (32% of the cohort) reported at least moderately severe chronic auditory symptoms: seven had tin

Clinical and investigational tools for monitoring noise-induced hyperacusis.

Hyperacusis is a recognized perceptual consequence of acoustic overexposure that can lead to debilitating psychosocial effects. Despite the profound impact of hyperacusis on quality of life, clinicians and researchers lack objective biomarkers and standardized protocols for its assessment. Outcomes of conventional audiologic tests are highly variable in the hyperacusis population and do not adequately capture the multifaceted nature of the condition on an individual level. This presents challenges for the differential diagnosis of hyperacusis, its clinical surveillance, and evaluation of new t

[Subjective ear noise and hyperacusis. Treatment with fluctuating currents].

The prevalence of hyperacusis, and subjective ear noise in particular, tends to increase in the general population. The data available in the literature show that physiotherapeutical procedures are the most effective treatment of this abnormality. The techniques and devices using fluctuating currents were employed to treat 40 patients. A "SLUKH-OTO-1" (HEARING-OTO) device is of use now. This procedure proved to be the most effective in case of hyperacusis and subjective ear noise without hearing disorder, there was also a high percentage of positive results in chronic neurosensory hypoacusis.

Hyperacusis in Williams syndrome: characteristics and associated neuroaudiologic abnormalities.

BACKGROUND: Hyperacusis and phonophobia are common, debilitating symptoms in Williams syndrome (WS), yet little is known about their underlying audiologic and neurologic processes. METHODS: The mothers of 49 subjects with WS were asked to complete the Hyperacusis Screening Questionnaire. Subjects with reported hyperacusis and sufficient developmental capacity underwent comprehensive audiological and brain auditory evoked response (BAER) testing. Findings were compared with those from pair-matched typically developing control subjects. RESULTS: Forty-one of the 49 children with WS (84%) had h

Altered intrinsic brain connectivity in misophonia, with and without hyperacusis.

Misophonia and loudness hyperacusis are debilitating sound intolerance conditions marked by extreme emotional and physiological responses to everyday sounds. Although frequently co-occurring, their distinct neural correlates remain poorly delineated. In an exploratory data-driven analysis, we identified neural-connectivity based markers of misophonia among cortical and subcortical networks in the brain using resting-state fMRI data. We leveraged an optimized and cross-validated machine learning framework to sift through >85 thousand functional connections and to evaluate detectability of misop

Sound Therapy to Reduce Auditory Gain for Hyperacusis and Tinnitus.

PURPOSE: Hyperacusis is the most common of the different types of sound tolerance conditions. It has been defined as physical discomfort or pain when any sound reaches a certain level of loudness that would be comfortable for most people. Because hyperacusis and tinnitus occur together so often, it has been theorized that they have a common neural mechanism. A leading contender for that mechanism is enhancement of auditory gain. The purpose of this tutorial is to review the evidence that sound/acoustic therapy can reduce auditory gain and, thereby, can increase loudness tolerance for people wi

Development of Tinnitus and Hyperacusis in a Mouse Model of Tobramycin Cochleotoxicity.

Aminoglycosides (AG) antibiotics are a common treatment for recurrent infections in cystic fibrosis (CF) patients. AGs are highly ototoxic, resulting in a range of auditory dysfunctions. It was recently shown that the acoustic startle reflex (ASR) can assess behavioral evidence of hyperacusis and tinnitus in an amikacin cochleotoxicity mouse model. The goal of this study was to establish if tobramycin treatment led to similar changes in ASR behavior and to establish whether ebselen can prevent the development of these maladaptive neuroplastic symptoms. CBA/Ca mice were divided into three group

Hyperexcitability of the Nucleus Accumbens Is Involved in Noise-Induced Hyperacusis.

Reduced tolerance to sound stimuli (hyperacusis) is commonly seen in tinnitus patients. Dysfunction of limbic systems, such as the nucleus accumbens (NAc), may be involved in emotional reactions to the sound stimuli in tinnitus patients. To study the functional changes in the NAc in hyperacusis, we have examined the neural activity changes of the NAc using c-Fos staining in an animal model of hyperacusis. The c-Fos staining was also examined in the medial geniculate nucleus (MGN), a central auditory pathway which has neural projections to the NAc. Postnatal rats (14 days) were exposed to loud

Separate auditory pathways for the induction and maintenance of tinnitus and hyperacusis?

Tinnitus and hyperacusis often occur together, however tinnitus may occur without hyperacusis or hyperacusis without tinnitus. Based on animal research one could argue that hyperacusis results from noise exposures that increase central gain in the lemniscal, tonotopically organized, pathways, whereas tinnitus requires increased burst firing and neural synchrony in the extra-lemniscal pathway. However, these substrates are not sufficient and require involvement of the central nervous system. The dominant factors in changing cortical networks in tinnitus patients are foremost the degree and type

Abnormal Auditory Gain in Hyperacusis: Investigation with a Computational Model.

Hyperacusis is a frequent auditory disorder that is characterized by abnormal loudness perception where sounds of relatively normal volume are perceived as too loud or even painfully loud. As hyperacusis patients show decreased loudness discomfort levels (LDLs) and steeper loudness growth functions, it has been hypothesized that hyperacusis might be caused by an increase in neuronal response gain in the auditory system. Moreover, since about 85% of hyperacusis patients also experience tinnitus, the conditions might be caused by a common mechanism. However, the mechanisms that give rise to hype

A systematic review on sound-based therapy for individuals with hyperacusis.

Hyperacusis is marked by an abnormal sensitivity to everyday sounds, causing considerable distress and hindrance in daily life. It is frequently linked with other hearing disorders like tinnitus and may occur alongside neurological and psychiatric conditions such as autism, migraines, and depression. Recent research into hyperacusis treatment has explored various sound therapy methods, including counselling and the use of therapeutic sounds. Consequently, the current systematic review investigated whether sound-based therapy programs could alleviate the effects of hyperacusis. A total of 149 a

Audiometric Profiles Across a Clinical Population of Children With Hyperacusis.

PURPOSE: Hyperacusis (sound sensitivity) is well documented among adults, but little is known about hyperacusis in children. Here, we assess relationships among the presence of hyperacusis, audiometric profiles, and nonauditory factors in a pediatric clinical population. METHOD: A retrospective review of clinical records from 329 children, ages 2-17 years, was performed to assess the prevalence of hyperacusis and correlations among hearing status, behavioral or communication concerns, and the presence of hyperacusis. RESULTS: In 329 study participants, 18% reported hyperacusis (58% male and

Functional Neuroanatomy of Salicylate- and Noise-Induced Tinnitus and Hyperacusis.

Tinnitus and hyperacusis are debilitating conditions often associated with aging or exposure to intense noise or ototoxic drugs. One of the most reliable methods of inducing tinnitus is with high doses of sodium salicylate, the active ingredient in aspirin. High doses of salicylate have been widely used to investigate the functional neuroanatomy of tinnitus and hyperacusis. High doses of salicylate have been used to develop novel behavioral methods to detect the presence of tinnitus and hyperacusis in animal models. Salicylate typically induces a hearing loss of approximately 20 dB which great

A Novel Mouse Model of Aminoglycoside-Induced Hyperacusis and Tinnitus.

Aminoglycosides (AG) such as amikacin are commonly used in cystic fibrosis patients with opportunistic pulmonary infections including multi-drug resistant mycobacterium tuberculous and non-tuberculous mycobacterium. Unfortunately, this class of drugs is known to cause peripheral damage to the cochlea leading to hearing loss that can fluctuate and become permanent over time or multiple exposures. However, whether amikacin can lead to central auditory dysfunction like hyperacusis (increased sensitivity to sound) or tinnitus (perception of sound in the absence of acoustic stimulation) is not well
