Right common iliac aneurysm by peripheral computed tomographic angiography.

This is a 77-year-old diabetic woman with history of claudication (Rutherford Category 2). Due to fear of underestimating disease with ankle-brachial index in a person with diabetes and advancing age, a CT angiogram was performed. The right common iliac artery had aneurysmal disease (3.4 cm) and a thrombus of right internal iliac artery. There was also a 40% stenosis of right external iliac artery and a left subtotal internal iliac stenosis at the origin. Given it's non-invasive nature and high diagnostic accuracy, CTA is poised to become the noninvasive test of choice in patients with suspect

Ruptured internal iliac artery aneurysm mimicking a hip fracture.

Internal iliac artery aneurysms are difficult to detect due to their wide range of clinical presentations. The mortality rate from ruptured internal aortic aneurysms is particularly high in part because of delayed diagnosis. This report reviews the management of a patient with a ruptured internal iliac artery aneurysm which presented mimicking a hip fracture.

Internal iliac artery revascularisation versus internal iliac artery occlusion for endovascular treatment of aorto-iliac aneurysms.

BACKGROUND: Endovascular aortic aneurysm repair (EVAR) is used to treat aorto-iliac and isolated iliac aneurysms in selected patients, and prospective studies have shown advantages compared with open surgical repair, mainly in the first years of follow-up. Although this technique produces good results, anatomic issues (such as common iliac artery ectasia or an aneurysm that involves the iliac bifurcation) can make EVAR more complex and challenging and can lead to an inadequate distal seal zone for the stent-graft. Inadequate distal fixation in the common iliac arteries can lead to a type Ib en

The treatment of isolated iliac artery aneurysm in patients with non-aneurysmal aorta.

OBJECTIVES: The aim of the study was to evaluate early and mid-term results of surgical repair of isolated iliac artery aneurysm (IAA) in patients with non aneurysmal abdominal aorta. METHODS: From January 1996 to December 2006, 34 patients with IAA had elective surgery. In 32 cases open repair was performed. Two patients had endovascular repair using a tube endoprosthesis and internal iliac artery coil embolization. The diameters of the abdominal aorta and iliac arteries were measured preoperatively and during follow-up. Early and late results in terms of mortality, major morbidity, reinterv

A Case of Iliac Aneurysm with Persistent Sciatic Artery Treated by Endovascular Aneurysm Repair.

A persistent sciatic artery (PSA) is a rare congenital vascular anomaly that occurs in approximately 0.01 to 0.06% of the population. We encountered a patient with aneurysms in the common iliac-internal iliac artery continuous to an occluded right PSA. The patient was an 85-year-old male in whom intermittent claudication of the right lower limb appeared 3 months ago. The right ankle-brachial index (ABI) was 0.48. On contrast computed tomography, the right PSA was present and was occluded over the popliteal artery. The right superficial femoral artery was hypoplastic. Moreover, abdominal aortic

A Giant Internal Iliac Artery Aneurysm with Arteriovenous Fistulization.

BACKGROUND: Isolated iliac artery aneurysms are rare and occur predominantly in men at an older age. Such aneurysms can rupture into an adjacent organ (such as the bowel, bladder or ureter) or into the adjacent common iliac vein, resulting in an arteriovenous fistula. INTRODUCTION: Formation of an internal iliac arteriovenous fistula caused by spontaneous rupture of an atherosclerotic iliac artery aneurysm wall is an exceedingly rare yet serious complication. CASE PRESENTATION: This article presents a case of an internal iliac arteriovenous fistula caused by rupture of an atherosclerotic gia

Balloon Endoclamping for Postdissection Infrarenal Aorto-Iliac Aneurysm Repair.

BACKGROUND: Isolated post dissection infrarenal and iliac aneurysm is a rare condition that often requires surgical treatment. Surgical repair should involve the replacement of the aneurysmal segments and a wide fenestration in the residual proximal untreated abdominal aorta. However, in these patients proximal aortic clamping may be challenging. Indeed, infrarenal clamping may hamper an appropriate fenestration in the proximal dissecting lamella, and suprarenal or supraceliac clamping can be dangerous and highly demanding, especially in acute and subacute patients. Here we report our initial

Endovascular management of iliac rupture during endovascular aneurysm repair.

BACKGROUND: Inadequate iliac artery diameter, calcification, and tortuosity are associated with increased incidence of iliac injury during abdominal (EVAR) and thoracic endovascular aneurysm repair (TEVAR). Despite careful preoperative assessment and use of iliac conduits, inadvertent iliac rupture is a source of morbidity and mortality. This report details our single-center, 10-year experience with intraoperative iliac artery rupture and describes a successful endovascular salvage technique. METHODS: All patients undergoing EVAR and TEVAR between August 1997 and June 2008 were reviewed. Comp

Iliac artery aneurysm causing isolated superior gluteal nerve lesion.

Lesions of the superior gluteal nerve (SGN) lead to weakness of hip abduction, manifesting itself as a gait abnormality, with contralateral tilting of the pelvis with each step. Causes are numerous and may occur at different anatomical locations before the nerve enters the suprapiriform foramen, in the foramen itself, or after the nerve has exited the foramen. This case report describes an SGN lesion by a large iliac artery aneurysm in a patient presenting with a gait disorder.

Iliac artery aneurysm presenting as a complication of hysterectomy.

Aneurysms rarely presenta as a complication of hysterectomy. A case is reported of a left common iliac artery aneurysm causing readmission 2 weeks after total abdominal hysterectomy. The aneurysm remained undiagnosed prior to percutaneous rupture and subsequent laparotomy. The aneurysm was ligated and resected. No graft or arterial repair was attempted because of the infection that was present. Twenty-three days after hysterectomy, the patient underwent above-the-knee amputation. A review of the literatureis undertaken and possible etiolgic mechanisms discussed.

Histopathology of an iliac aneurysm in a case of Menkes disease.

In Menkes disease, arterial tortuosity is frequent, whereas true aneurysms are rare. Here, we report aneurysmal pathology occurring in an infant with Menkes disease. An iliac aneurysm was diagnosed in a 2-month-old boy and attributed to Menkes syndrome on the basis of plasma copper deficiency. Samples of the aneurysmal wall were taken during surgery for histopathological analysis. As in other forms of aneurysm, the arterial wall was characterized by smooth muscle cell (SMC) disappearance, linked to SMC apoptosis and oxidative stress, areas of mucoid degeneration, and extracellular matrix break

Endovascular repair of isolated iliac artery aneurysms.

PURPOSE: To retrospectively evaluate a 12-year experience with endovascular repair of isolated iliac artery aneurysm (IAA). METHODS: From August 1997 through July 2009, 91 patients (81 men; mean age 71 years, range 31-90) underwent endovascular treatment for isolated IAA at our department. Of these, 77 patients received stent-grafts either alone or in combination with coils or an Amplatzer vascular plug (n = 2); 1 patient received a Smart stent combined with coils, and 13 patients were treated with coil embolization only. The aneurysms were classified according to location: type I = common il

Endovascular management of isolated iliac artery aneurysms.

OBJECTIVE: We reviewed our experience with endovascular treatment of isolated iliac artery aneurysms (IAAs). METHODS: Medical records for consecutive patients undergoing endovascular IAA repair from 1995 to 2004 were reviewed. Computed tomography (CT) angiograms were used to assess IAA location, size, and presence of endoleaks after endovascular repair. Rates of primary patency and freedom from secondary interventions were estimated using the Kaplan-Meier life-table method. RESULTS: From July 1995 to November 2004, 45 patients (42 men), with a mean age of 75 years, underwent endovascular rep

Urological manifestations of isolated iliac artery aneurysms.

Three patients with iliac artery aneurysms were seen at our institution with the chief complaints owing to urological manifestations. Iliac artery aneurysms may present with a transrectally palpable mass, hematuria, bladder outlet obstruction, ureteral obstruction, retroperitoneal fibrosis or renal failure. Any such manifestations may bring the aneurysm to the attention of a urologist before diagnosis.

The association of aorto-iliac aneurysms with ureteral obstruction.

Aorto-iliac aneurysms may entrap the ureters in perianeurysmal fibrosis, causing medical ureteral deviation and/or obstruction. The latter has been described only in male patients; a further 5 cases due to iliac and 3 to aortic aneurysm are reported. The radiologic picture resembles retroperitoneal fibrosis; the correct diagnosis may be suggested when ureteral tethering or narrowing lies near arterial calcification. Sonography is usually the most appropriate investigation to confirm the diagnosis. When sonography shows an aortic aneurysm, the kidneys should be scanned to rule out hydronephrosi

Successful Percutaneous Coil Embolization of a Ruptured Internal Iliac Artery Aneurysm Remnant after Abdominal Aortic Aneurysm Repair via the Deep Iliac Circumflex Artery.

Here, we describe a case of an 83-year-old man treated with percutaneous IIA coil embolization for an enlarging remnant IIA aneurysm. CT scans revealed a contained rupture and persistent flow in the right IIA with the enlargement. We selected percutaneous embolization via the deep iliac circumflex artery, that was communicating with the superior gluteal artery and the IIA. Coil embolization of the arteries supplying the IIA aneurysm was successfully performed with 12 embolization coils placed in the IIA and its branches. The absence of blood flow and shrinkage of the aneurysm were confirmed by

[Isolated iliac artery aneurysms : Interventional treatment].

CLINICAL ISSUE: Isolated iliac artery aneurysms occur considerably less often than abdominal aortic aneurysms. Mainly older men are affected by this disease. Most of these aneurysms are asymptomatic and are incidentally detected during cross-sectional imaging. Iliac aneurysms with a diameter larger than 3 cm are at risk for rupture, which is associated with high morbidity and mortality. STANDARD TREATMENT: To prevent their rupture as well as for symptomatic or ruptured aneurysms, endovascular treatment has recently been established as the primary approach due to the decreased morbidity and mo
