Surgical planning using three-dimensional imaging and computer modeling.

The acquisition, processing, and use of three-dimensional (3D) imaging provide new insights into normal and abnormal craniofacial anatomy. In this article, characteristics of CT and MRI scanning are reviewed along with the methods used to delineate tissues and produce 3D patient displays, including contouring, shaded surface, and volumetric processing. The use of 3D model fabrication for surgical planning is discussed with examples of the production of a custom CT-based auricle model and the use of rapid prototyping technology to create models for custom alloplastic implant surgery. The review

Surface contour three-dimensional imaging in congenital aural atresia.

Sixty-five patients with congenital aural atresia-stenosis had three-dimensional reconstructions of their high-resolution computed tomographic scans. Surface anatomy of the temporal bone was readily demonstrated, including its relation to the temporomandibular joint. Three important findings were noted. (1) Contrary to popular belief, the condyle of the mandible does not rest against the anterior face of the mastoid bone. (2) A bony cleft or groove is often in the lateral temporal bone through which the facial nerve may exit. (3) Duplications of bony structures attached to or part of the tempo

Magnetic resonance imaging of the breast: functional T1 and three-dimensional imaging.

Thirty-two patients with breast abnormalities and 10 normal volunteers were examined with magnetic resonance imaging (MRI) utilizing a 0.5-Tesla superconducting magnet imaging system. Sagittal, coronal, and transverse breast imaging with the 50-cm body coil and with a specialized 16 X 8 cm breast coil allowed the evaluation of a single breast. Soft-tissue contrast was clearly apparent using proton density-weighted, T1 weighted, calculated T1 images. In addition, the feasibility and potential clinical utility of three-dimensional color imaging was evaluated using computer-based interactive algo

Three-dimensional printing of intracardiac defects from three-dimensional echocardiographic images: feasibility and relative accuracy.

BACKGROUND: With the advent of three-dimensional (3D) printers and high-resolution cardiac imaging, rapid prototype constructions of congenital cardiac defects are now possible. Typically, source images for these models derive from higher resolution, cross-sectional cardiac imaging, such as cardiac magnetic resonance imaging or computed tomography. These imaging methods may involve intravenous contrast, sedation, and ionizing radiation. New echocardiographic transducers and advanced software and hardware have optimized 3D echocardiographic images for this purpose. Thus, the objectives of this

Intraoperative imaging of the shoulder: A comparison of two- and three-dimensional imaging techniques.

BACKGROUND: Isocentric three-dimensional C-arms allow for more effective intraoperative fracture reduction control compared to two-dimensional imaging techniques. However, this design is not appropriate for shoulder scanning. OBJECTIVE: To assess the feasibility of using a newer generation variable isocentric flat detector 3D C-arm for intraoperative glenohumeral and acromioclavicular joint assessment and to compare the accuracy of its intraoperative 3D imaging technology to a standard two-dimensional (2D) flat detector fluoroscope. METHODS: Five whole-body human cadavers were used (ten shou

[Three-dimensional visualization of intercondylar notch based on MRI two-dimensional images].

OBJECTIVE: To study the feasibility of virtual intercondylar notchplasty by applying MRI two-dimensional (2D) images to reconstruct three-dimensional (3D) images and measure the size of intercondylar notch. METHODS: Thirty healthy volunteers who had no knee joint disease and surgery history were selected. There were 15 females and 15 males with an age range of 20-30 years, weight range of 45-74 kg, and height range of 150-185 cm. They were divided into male group and female group, and the knees of each group were divided into 2 subgroups (the left group and right group). MRI scan of the left

Grasping two-dimensional images and three-dimensional objects in visual-form agnosia.

Visually guided prehension is controlled by a specialized visuomotor system in the posterior parietal cortex. It is not clear how this system responds to visual stimuli that lack three-dimensional (3D) structure, such as two-dimensional (2D) images of objects. We asked a neurological patient with visual-form agnosia (patient D.F.) to grasp 3D objects and 2D images of the same objects and to estimate their sizes manually. D.F.'s grip aperture was scaled to the sizes of the 2D and 3D target stimuli, but her manual estimates were poorly correlated with object size. Control participants demonstrat

Comparison of two-dimensional and three-dimensional images for phenotypic assessment of craniofacial microsomia.

Background and Purpose :  Three-dimensional surface imaging is used in many craniofacial centers. However, few data exist to indicate whether such systems justify their cost. Craniofacial microsomia is associated with wide phenotypic variability and can affect most facial features. The purpose of this study is to compare three-dimensional versus two-dimensional images for classification of facial features in individuals with craniofacial microsomia. Methods :  We obtained a series of two-dimensional and three-dimensional images of 50 participants, aged 0-20 years, diagnosed with craniofacial m

Use of Three-dimensional US for Troubleshooting in Gynecologic Imaging.

Three-dimensional (3D) US is an invaluable, rapidly evolving tool in gynecologic imaging. This examination, comparable to CT and MRI, enables volumetric data to be obtained, reconstructed, and displayed in multiple planes. It also enables the sonologist to better understand the spatial and anatomic relationships of uterine and adnexal abnormalities, thereby improving diagnostic accuracy. Three-dimensional US can be performed routinely following a two-dimensional (2D) transabdominal and/or transvaginal US examination or as an adjunctive troubleshooting technique, since no additional patient pre

Three-dimensional image analysis for staging chronic rhinosinusitis.

BACKGROUND: Traditional methods of staging chronic rhinosinusitis (CRS) through imaging do not differentiate between degrees of partial mucosal sinus inflammation, thus limiting their utility as imaging biomarkers. We hypothesized that software-aided, quantitative measurement of sinus inflammation would generate a metric of disease burden that would correlate with clinical parameters in patients with suspected sinus disease. METHODS: Adults with rhinologic complaints undergoing computed tomography imaging were recruited at an urban, academic, tertiary care center (n = 45 with Lund-Mackay [LM]

Three-dimensional HDlive rendering images of the fetal heart.

Our objective is to describe our experience with reconstruction of normal fetal cardiac structures and congenital heart anomalies using the 3-D HDlive rendering mode with spatiotemporal image correlation (STIC). Four normal fetuses and three fetuses with congenital heart anomalies (Ebstein's anomaly, hypoplastic left heart syndrome and tetralogy of Fallot) at 25-35 wk of gestation were studied using the 3-D HDlive rendering mode with STIC. In normal fetuses, the natural appearance of the dynamic motion of the foramen ovale flap and both atrioventricular valves was clearly visualized in real ti

[Klippel-Feil syndrome. Three-dimensional tomographic images].

Klippel-Feil syndrome (KPS) is characterized by congenital vertebral fusion believed to result from faulty segmentation along the embryo's developing axis. The hallmark phenotypic findings of low hairline, short neck, and limited range of motion of the neck are found as al triad in < 50% of patients. Congenital musculoskeletal and systemic anomalies are commonly found. These include, but are not limited to elevation of the scapula, scoliosis and renal and cardiac abnormalities. Here, we present the three-dimensional images by computed tomography of a 9-year-old boy with multiple fusion of the

Three-dimensional computed tomography of the foot: optimizing the image.

The complex anatomy of the foot can be imaged using high resolution computed tomography. High resolution serial nonoverlapping CT scans of foot anatomy have a high degree of soft tissue contrast, and excellent geometrical accuracy (no magnification error). Three-dimensional surface reconstruction from CT scans of the foot were performed using specially developed computer software. These surface reconstructions display the osseous and soft tissue anatomy of the foot in a form similar to anatomic preparations. The removal of overlying skin, disarticulation of the ankle, tarsals and metatarsals w

Three-dimensional imaging of vascular development in the mouse epididymis.

Long considered an accessory tubule of the male reproductive system, the epididymis is proving to be a key determinant of male fertility. In addition to its secretory role in ensuring functional maturation and survival of spermatozoa, the epididymis has a complex immune function. Indeed, it must manage both peripheral tolerance to sperm antigens foreign to the immune system and the protection of spermatozoa as well as the organ itself against pathogens ascending the epididymal tubule. Although our knowledge of the immunobiology of this organ is beginning to accumulate at the molecular and cell

Intraoperative three-dimensional imaging in the treatment of calcaneal fractures.

BACKGROUND: Displaced intra-articular calcaneal fractures are frequently treated by open reduction and internal fixation. The usual intraoperative monitoring by means of fluoroscopy does not always provide complete intraoperative information for the surgeon. The aims of this study were to analyze the percentage of patients for whom intraoperative three-dimensional imaging leads to intraoperative revision and whether the avoidance of an intra-articular step or gap influences the clinical outcome. METHODS: From August 2001 to June 2009, 377 consecutive, operatively treated calcaneal fractures w
