Exosomes are important carriers of biological information that facilitate intercellular communication and participate in the pathophysiology of different cardiovascular diseases. Myocardial infarction is among the leading causes of death worldwide. Upon myocardial infarction, massive cardiomyocyte death triggers a strong inflammatory response which is a vital process of cardiac injury, repair, and remodeling. Increasing evidence has unveiled that exosomes are involved in the inflammatory response and immune regulation after myocardial infarction. In this review, we will summarize the biologica

Leishmaniasis is caused by protozoan parasites of the genus Leishmania and causes a wide spectrum of clinical manifestations ranging from self-healing cutaneous lesions to the fatal visceral form. The use of pentavalent antimony, the mainstay of therapy of Leishmaniasis is now limited by its toxicity and alarming increase in unresponsiveness, especially in the Indian subcontinent. Furthermore, other anti-leishmanial drugs are unaffordable in many affected countries and as vaccination based approaches have not yet proved to be effective, chemotherapy remains the only alternative, emphasizing th

Important immunomodulatory properties of 14- and 15-membered macrolides may benefit patients with respiratory diseases associated with chronic inflammation. These properties include decreased neutrophil chemotaxis and infiltration into the respiratory epithelium, inhibition of transcription factors leading to decreased proinflammatory cytokine production, downregulation of adhesion molecule expression, inhibition of microbial virulence factors including biofilm formation, reduced generation of oxygen-free radicals, enhanced neutrophil apoptosis, and decreased mucus hypersecretion with improved

Acute respiratory distress syndrome is a common condition among the critically ill and is associated with high morbidity and mortality [table: see text] rates. Improved understanding of the underlying inflammatory pathogenetics has encouraged the search for strategies that, by modifying this immune response, can improve outcome for this group of patients. Some agents are obviously anti-inflammatory. Others have been used primarily for other purposes; their immune effects are incidental, but no less important. Although immunomodulatory strategies have been discussed for many years, they now are

Activation of the opioid receptors by delta-agonist DSLET and by kappa-agonist rimorphin led to a significant inhibition of plaque-forming and rosette-forming cells in the CBA mice. On the other hand, mu-agonist DAGO stimulated the immune response on the 4th and 5th days after immunization with SRBC (5 x 10(8)). Lesion of the hypothalamo-hypophyseal connection prevented immuno-stimulating as well as immuno-depressive effects. The latter seems to be due to an interaction with the serotoninergic system, whereas immuno-stimulating effect involves the dopaminergic system.

BACKGROUND: Mortality rates for disseminated coccidioidomycosis remain high despite modern antifungal therapies, resulting in substantial costs to society. We propose that immunomodulation as an adjunctive therapy with antifungals can improve clinical outcomes. Previous work suggested that treatment with interferon gamma or by blocking the IL-4/IL-13 receptor with dupilumab could improve clinical outcomes.
METHODS: We present a case series of 18 patients at UCLA with severe or persistent coccidioidomycosis receiving maximal antifungal treatment who received immunomodulatory therapy. Treatment

Dysregulated host immune responses are characteristic of a variety of inflammatory-mediated diseases. As such, novel immune-modulating therapies have been developed for the treatment of these inflammatory conditions. One condition that has seen a paradigm shift in therapeutic strategies is periodontal disease (PD), a common oral inflammatory condition. While initiated by bacteria, a dysregulated host immune response drives the disease. In this review, we discuss key innate and adaptive immune cells that have a role in PD progression, as well as different ways to target them. We highlight thera

Pediatric sepsis presents a unique clinical challenge due to the distinct characteristics of the developing immune system. The immune response in children differs significantly from that in adults, exhibiting a unique combination of resistance, disease tolerance, and resilience. These factors influence the clinical presentation and prognosis of pediatric patients with sepsis. Over the past few years, various studies have explored the role of immunomodulatory therapies in managing sepsis, including the use of immunoglobulins, corticosteroids, monoclonal antibodies, and immunostimulatory treatme

Tolerance is the Holy Grail of solid organ transplantation (SOT) and remains its primary challenge since its inception. In this topic, the seminal contributions of Thomas Starzl at Pittsburgh University outlined foundational principles of graft acceptance and tolerance, with chimerism emerging as a pivotal factor. Immunologically, intestinal transplantation (ITx) poses a unique hurdle due to the inherent characteristics and functions of the small bowel, resulting in increased immunogenicity. This necessitates heavy immunosuppression (IS) while IS drugs side effects cause significant morbidity.

Fractures and bone nonunion commonly require surgical intervention. Serious outcomes of non-healing in the late stages of fracture place a significant financial burden on society and families. Bone nonunion occurs when a fracture stops healing, for many reasons, and leads to a variety of bad outcomes. Numerous factors, including biomechanics and immunology, are involved in the complicated mechanisms of bone nonunion. The immune-inflammatory response plays a significant part in the emergence of bone nonunion, and the occurrence, control, and remission of inflammation in the bone healing process

Severe outcomes and death from the novel coronavirus disease 2019 (COVID-19) appear to be characterized by an exaggerated immune response with hypercytokinemia leading to inflammatory infiltration of the lungs and acute respiratory distress syndrome. Risk of severe COVID-19 outcomes is consistently lower in women than men worldwide, suggesting that female biological sex is instrumental in protection. This mini-review discusses the immunomodulatory and anti-inflammatory actions of high physiological concentrations of the steroids 17β-estradiol (E2) and progesterone (P4). We review how E2 and P4

The Gram-positive bacterium Staphylococcus aureus is one of the most frequent pathogens that causes severe morbidity and mortality throughout the world. S. aureus can infect skin and soft tissues or become invasive leading to diseases such as pneumonia, endocarditis, sepsis or toxic shock syndrome. In contrast, S. aureus is also a common commensal microbe and is often part of the human nasal microbiome without causing any apparent disease. In this review, we explore the immunomodulation and disease tolerance mechanisms that promote commensalism to S. aureus.

Inflammatory neuropathies are treatable immune-mediated disorders of the peripheral nerves, the prevalence of which is higher than previously believed. Although there are now established treatments of proven benefit for acute and chronic inflammatory neuropathies consisting of intravenous immunoglobulins, corticosteroids and plasma exchanges, there are patients who fail to respond to these therapies. Immunosuppressants have been used in these refractory patients and also to lower the requirements of therapies of established efficacy due to their cost (e.g. immunoglobulins) or sideeffects (e.g.

Many epidemiological data support the postulate that infection with helminths might provide some protection against allergic and autoimmune diseases. Hence arises the concept that helminths strongly influence the immune system and enable protective pathways against these hyperimmune-associated disorders. This review discusses how Trichinella spiralis can make the immune system smarter in dealing with hyperimmune-associated disorders. T. spiralis possesses the capacity to direct the immune system towards a mixed Th1/Th2 phenotype with predominance of Th2 response, or it may interfere with dendr

Current data on therapeutic immunomodulation used to treat systemic vasculitides are reviewed in this paper, which also discusses ongoing and future developments in the field. In vasculitides associated with anti-neutrophil cytoplasmic antibodies, rituximab is a validated induction treatment that can serve as an alternative to cyclophosphamide and must be followed by maintenance treatment. In addition, the usefulness of rituximab as maintenance treatment was established recently. Immunoglobulins can be helpful adjuncts, most notably in patients with severe immunodepression. Plasmapheresis is i

Prion diseases are a unique category of illness, affecting both animals and humans, where the underlying pathogenesis is related to a conformational change of a normal self protein called cellular prion protein to a pathological and infectious conformer known as scrapie prion protein (PrP(Sc)). Currently, all prion diseases lack effective treatment and are universally fatal. Past experiences with bovine spongiform encephalopathy and variant Creutzfeldt-Jakob disease mainly in Europe, as well as the current epidemic of chronic wasting disease in North America, have highlighted the need to devel
