Toward a Consensus Aproach for Assessing Capsular Contracture Severity and Progression: A Systematic Review.

BACKGROUND: Breast implants are the most commonly used medical devices in plastic surgery, and capsular contracture (CC) is one of the most common complications. However, our assessment of CC is based largely on Baker grade, which is problematically subjective and affords only four possible values. METHODS: The authors performed a systematic review concluding in September of 2021 in compliance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. It identified 19 articles that propose approaches to measuring CC. RESULTS: In addition to Baker grade, the authors i

Feasibility of Breast-Conservation Therapy and Hypofractionated Radiation in the Setting of Prior Breast Augmentation.

PURPOSE: Cosmetic outcomes and rate of implant loss are poorly characterized among patients with breast cancer with previous breast augmentation (BA) who undergo breast-conservation therapy (BCT). Here we determine capsular contracture and implant loss frequency after BCT among patients receiving contemporary whole-breast radiation therapy (RT). METHODS AND MATERIALS: Patients with breast cancer with a history of BA presenting to our institution from January 2006 to January 2017 who elected for BCT were included. Seventy-one breast cancers in 70 patients with a history of BA electing for BCT

Comparative Postoperative Complications of Acellular Dermal Matrix and Mesh Use in Prepectoral and Subpectoral One-Stage Direct to Implant Reconstruction: A Retrospective Cohort Study.

BACKGROUND: One-stage direct-to-implant (DTI) breast reconstruction is increasingly popular with the use of prepectoral reconstruction leading to increased demand for structural scaffolds. It is vital to determine if differences in safety profiles exist among scaffolds. METHODS: We performed a retrospective cohort study of consecutive patients in our breast cancer center undergoing DTI reconstruction. Outcomes relating to postoperative infections, wound complications, and implant related complications were extracted. Outcomes were grouped into major, minor, and long-term complications. Univar

Radiation Modality (Proton/Photon), Timing, and Complication Rates in Patients With Breast Cancer Receiving 2-Stages Expander/Implant Reconstruction.

PURPOSE: Our purpose is to explore the effect of postmastectomy radiation therapy (PMRT) modality and timing on complication rates in breast cancer patients receiving immediate 2-stages expander/implant. METHODS AND MATERIALS: We reviewed the charts of 661 patients who underwent immediate 2-stages expander/implant with/without PMRT at our institution from 2000 to 2019. Patients were divided into 3 cohorts: no radiation, PMRT to expanders (RTE), and PMRT to implants after expander exchange (RTI). PMRT was delivered either with 3-dimensional conformal photon with or without chest wall boost (CW

Myofibroblasts and capsular tissue tension in breast capsular contracture.

BACKGROUND: This study aimed to observe the relationship between the number of myofibroblasts, the tensile strength of the breast implant capsule, and the degree of breast capsular contracture. METHODS: The study enrolled 21 women with 31 capsular contractures after aesthetic breast augmentation. The capsular tissue specimens were obtained during capsulectomy, open capsulotomy, and other revisional procedures. The tensile strength of capsular tissues (1 × 3 cm) was measured by tensiometer. The capsular tissues were immunostained by alpha smooth muscle actin, and the immunostained myofibroblas

Topical Tranexamic Acid in Primary Breast Augmentation Surgery: Short- and Long-term Outcomes.

BACKGROUND: Breast augmentation mammaplasty (BAM) remains the most popular cosmetic procedure done worldwide. Bleeding in this procedure increases the chance of capsular contracture. Tranexamic acid (TXA), an antifibrinolytic, has been widely used by other surgical specialties to reduce bleeding. OBJECTIVES: We aimed to evaluate the use of TXA in BAM surgery. METHODS: This was a single-surgeon case series of all patients who underwent primary BAM from March 2017 to March 2018 and received topical TXA spray to the implant pocket before implant insertion. Early postoperative complications and

Baker Grade IV Capsular Contracture Is Correlated with an Increased Amount of Silicone Material: An Intrapatient Study.

BACKGROUND: Breast implant surgery is one of the most frequently performed procedures by plastic surgeons worldwide. However, the relationship between silicone leakage and the most common complication, capsular contracture, is far from understood. This study aimed to compare Baker grade I with Baker grade IV capsules regarding their silicone content in an intradonor setting, using two previously validated imaging techniques. METHODS: Twenty-two donor-matched capsules from 11 patients experiencing unilateral complaints were included after bilateral explantation surgery. All capsules were exami

Impact of post-operative infection on revision procedures in breast reconstruction: A marketscan database analysis.

BACKGROUND: Esthetic complications, such as capsular contracture and soft-tissue contour defects, hinder the desired outcomes of breast reconstruction. As subclinical infection is a prevailing theory behind capsular contracture, we investigated the effects of post-operative infections on these issues and revision procedures. METHODS: We conducted a retrospective database study (2007-2021) on breast reconstruction patients from the MarketScan® Databases. Esthetic complications were defined by their associated revision procedures and queried via CPT codes. Severe capsular contracture (Grade 3-4

COVID-19-Related Immune Activation and Capsular Contracture: A Cohort Study of Infection, Vaccination, and Explantation.

BACKGROUND: Capsular contracture, a prevalent complication following breast augmentation, is characterized by pain, implant distortion, and often necessitates explantation. While its multifactorial pathogenesis involves bacterial biofilms and implant characteristics, the influence of immune activation, particularly in the context of COVID-19 infection and vaccination, remains underexplored. Growing evidence suggests COVID-19 vaccination can trigger immune-mediated reactions, including delayed inflammatory responses to dermal fillers. Given that capsular contracture is a fibro-inflammatory resp

Long-term follow-up of breast capsule contracture rates in cosmetic and reconstructive cases.

BACKGROUND: Silicone gel breast implants are associated with long-term adverse events, including capsular contracture, with reported incidence rates as high as 50 percent. However, it is not clear how long the follow-up period should be and whether there is any association with estrogen or menopausal status. In addition, the placement of Baker grade II subjects in the majority of reports has been in data sets of controls instead of capsular contracture. METHODS: A retrospective medical study (1998 to 2004) was performed in women (n = 157) who received textured silicone breast implants for aes

Hypofractionated Versus Conventional Postmastectomy Radiotherapy in Implant-Based Breast Reconstruction: A Systematic Review and Meta-Analysis.

Background/Objectives: The safety of hypofractionated postmastectomy radiotherapy (HF-PMRT) in patients undergoing implant-based breast reconstruction remains uncertain because of concerns regarding radiation-related reconstructive complications. We conducted a systematic review and meta-analysis to compare reconstruction-related complications following HF-PMRT versus conventionally fractionated (CF) PMRT. Methods: PubMed, Cochrane Library, and EMBASE databases were searched through August 2025 following PRISMA guidelines. Studies comparing HF-PMRT with CF-PMRT in patients undergoing implant-b

[Radiotherapy and delayed breast reconstruction with implant: examination of compatibility].

PURPOSE: Delayed breast reconstruction with implant is frequently used after mastectomy. However, given that many breast cancer patients receive adjuvant radiotherapy, the adaptation of this method raises questions concerning possible cumulative complications. In the present study the compatibility of post-mastectomy radiotherapy and delayed breast reconstruction with implant was examined. PATIENTS AND METHODS: We evaluated a consecutive series of 66 invasive breast cancer patients who have received delayed breast reconstruction with implant after modified radical mastectomy between January 1

The fate of breast implants: a critical analysis of complications and outcomes.

Complications and outcomes were monitored following the implantation of 1655 breast implants over a 15-year period. Smooth, polyurethane, and textured implants were used in a variety of clinical settings. The time course of capsular contracture was analyzed by the Kaplan-Meier method. Regardless of implant type or indication for surgery, the probability of contracture increased with time. Polyurethane-covered implants were associated with a significant reduction in the risk of contracture for at least 7 years following implantation. Smooth and textured silicone implants had contracture rates s

The Use of Laparoscopic Camera Sleeve in Implant-Based Breast Reconstruction: An Innovative "No-Touch" Technique.

BACKGROUND: Implant-based breast reconstruction (IBBR) is the predominant reconstructive modality following mastectomy, accounting for more than 80% of all breast reconstruction procedures. Preventing subclinical infection at the time of implant insertion is critical for reducing the incidence of capsular contracture. This study evaluated a simple, cost-effective, flexible, and disposable laparoscopic camera sleeve designed to facilitate the no-touch technique in breast implant insertion. METHODS: A prospective study was conducted between November 2021 and September 2025, including all patien

Imaging of complications of augmentation mammaplasty.

The recent Food and Drug Administration hearings have brought new attention to breast implants and the importance of mammography in these patients to detect unsuspected breast pathology and complications of this surgical procedure. More patients are now being seen with implant failures in mammography departments. Very little attention has been paid to the mammographic findings and their significance until recently in the plastic surgery literature. A retrospective review was undertaken of all patients who had undergone prior augmentation mammaplasty and were imaged between 1989 and 1992. Patie

Unilateral preoperative chest wall irradiation in bilateral tissue expander breast reconstruction with acellular dermal matrix: a prospective outcomes analysis.

BACKGROUND: Prior breast irradiation increases the rate of postoperative complications, including capsular contracture, in tissue expander/implant reconstruction. Acellular dermal matrix is heralded to decrease capsular contracture, but recent evidence suggests a possible increase in postoperative complications. The authors evaluated outcomes in patients undergoing bilateral tissue expander/implant reconstruction with acellular dermal matrix in the setting of prior unilateral irradiation. METHODS: A case-control study was conducted on all patients undergoing bilateral, acellular dermal matrix

Subpectoral to prepectoral pocket change in implant-based breast reconstruction: A retrospective cohort study at a single institution.

BACKGROUND: Patients who underwent breast reconstruction with submuscular implants frequently experience animation deformity (AD), pain, and cosmetic issues, warranting consideration of implant removal, exchange, pocket change, or autologous reconstruction. Advances in prepectoral reconstruction have prompted several patients to undergo transfer of their implants from a submuscular to prepectoral pocket ("pocket change"). METHODS: We conducted a single-center, retrospective review analyzing all patients with post-mastectomy submuscular implant-based reconstruction who underwent pocket change
