ADVISE: First Results of a European Interoperative Information System Network Developed for the ADenoVirus Initiative Study in Epidemiology.

Adenoviral conjunctivitis is extremely contagious, causes a form of conjunctivitis. Therefore, it is important to identify patients who suffer from adenoviral conjunctivitis, as early as possible, in order to contain the disease. We present the 6 independent but interoperable platforms developed for the purpose of a large European epidemiologic study (ADVISE), which has been implemented independently in France, Germany, Spain, Italy and the UK. ADVISE is a non-interventional, observational epidemiology study with the objectives of assessing clinical characteristics and incidence of adenovirus

Profiling swallowing safety impairments, dysphagia severity, and risk factors for postoperative aspiration in heart transplant recipients.

OBJECTIVE: Pretransplant frailty and postoperative dysphagia significantly contribute to morbidity and mortality in heart transplant (HT) recipients. Therefore, we aimed to profile postoperative swallowing safety impairments, overall dysphagia severity, and risk factors for aspiration in HT recipients. METHODS: We performed a retrospective cohort study of adults who underwent HT and were clinically referred for a postoperative instrumental swallow evaluation (videofluoroscopy, flexible endoscopic evaluation of swallowing) at a large academic medical center between 2018 and 2024 by extracting

Serum phosphate in stone formers: what does a low serum phosphate level mean?

PURPOSE: Metabolic evaluation is indicated in stone formers to identify and subsequently modify risk factors for nephrolithiasis. Some believe that serum phosphate may be an important indicator of nephrolithiasis risk. We determined whether serum phosphate is an independent risk factor for recurrent calcium stone formation and whether it may be used as an early marker for occult disease, such as hyperparathyroidism. MATERIALS AND METHODS: The charts of all patients with regular long-term stone clinic followup were reviewed. Initial serum phosphate levels were compared with stone recurrence, c

Pitt-Hopkins Syndrome: A Unique Case Study.

OBJECTIVES: Pitt-Hopkins syndrome (PTHS) is a rare genetic disorder caused by insufficient expression of the TCF4 gene. Most cases are characterized by severe intellectual disability, absent speech, motor delays, and autism spectrum disorder. Many have abnormal brain imaging, dysmorphic facial features, and medical comorbidities: myopia, constipation, epilepsy, and apneic spells. The present case study expands existing understanding of this disorder by presenting a unique phenotype with higher cognitive abilities and fewer medical comorbidities. METHODS: The present case study reports on a 13

Clinical Usefulness of Computed Tomography Enterography as a Diagnostic Modality for Isolated Terminal Ileitis.

BACKGROUND/AIMS: Guidelines for further diagnostic evaluation in patients with isolated terminal ileitis have not been established. Although computed tomography enterography (CTE) is an established modality for small bowel evaluation, its diagnostic value in this setting remains unclear. This study aimed to evaluate the diagnostic utility of CTE in patients with isolated terminal ileitis identified during ileocolonoscopy and to identify patient subgroups most likely to benefit from additional CTE evaluation. METHODS: We retrospectively reviewed the medical records of 98 patients who were diag

The role of the interview in residency selection: A mixed-methods study.

CONTEXT: Residency programmes invest considerable time and resources in candidate interviews as a result of their perceived ability to reveal important social traits. However, studies examining the ability of interviews to predict resident performance have shown mixed findings, and the role of the interview in candidate evaluation remains unclear. This mixed-methods study, conducted in an anaesthesiology residency programme at a large academic medical centre, examined how interviews contributed to candidate assessment and whether the addition of behavioural questions to interviews altered thei

Longitudinal effects of medical students' communication skills on future performance.

BACKGROUND: The Essential Elements of Communication (EEC) were developed from the Kalamazoo consensus statement on physician-patient communication. The Uniformed Services University of the Health Sciences (USU) has adopted a longitudinal curriculum to use the EEC both as a learning tool during standardized patient encounters and as an evaluation tool culminating with the end of preclerkship objective-structured clinical examinations (OSCE). Medical educators have recently emphasized the importance of teaching communication skills, as evidenced by the United States Medical Licensing Examination

The Schedule for Evaluating Persistent Symptoms (SEPS): a new method of recording medically unexplained symptoms.

BACKGROUND: Medically unexplained symptoms are difficult to measure and in most cases the diagnosis is made either from independent data such as consultants' opinions or medical outcomes, or by proxy measures such as numbers of symptoms or consultations. A valid self-rated measure would be of value in assessing this highly prevalent condition. AIMS: To describe a new scale of nine items, the Schedule for Evaluation of Persistent Symptoms (SEPS), its properties, its internal consistency, its distribution in a sample of 470 medical patients, its relationship to social functioning and health anx

Validation of the commitment index of skin and mucous membranes in pemphigus vulgaris for the clinical evaluation of patients with pemphigus vulgaris.

BACKGROUND: Pemphigus vulgaris is a severe bullous disease, produced by the autoimmune destruction of desmosomes, resulting in the formation of intraepidermal blisters, affecting skin and mucous membranes, with a mortality of 5 to 10%. Side effects of therapy contributed to increased morbidity, accounting for considerable part of the immediate causes of death due to PV today. There is no reproducible score for clinical evaluation of patients with Pemphigus vulgaris, making the therapeutic decision subjective, and its results, uncertain. OBJECTIVES: Create and evaluate the reproducibility of a

Treatment of Primary Aldosteronism Reduces the Probability of Obstructive Sleep Apnea.

BACKGROUND: Aldosterone excess is hypothesized to worsen obstructive sleep apnea (OSA) symptoms by promoting peripharyngeal edema. However, the extent to which primary aldosteronism (PA), hypertension, and body mass index (BMI) influence OSA pathogenesis remains unclear. METHODS: We conducted a cross-sectional study of PA patients from our endocrine database to retrospectively evaluate OSA probability before and after adrenalectomy or medical management of PA. A control group of patients undergoing adrenalectomy for nonfunctioning benign adrenal masses was also evaluated. We categorized patie

Persistent Sleepiness After Effective Positive Airway Pressure Therapy Deserves Independent Clinical Evaluation.

Positive airway pressure (PAP) therapy often improves excessive daytime sleepiness in obstructive sleep apnea (OSA), yet effective control of sleep-disordered breathing does not invariably restore normal daytime alertness. Persistent sleepiness after PAP therapy should therefore be regarded as a clinical observation requiring explanation rather than automatically attributed to treated OSA. The term "residual excessive daytime sleepiness" may become misleading when it is interpreted as an etiologic conclusion rather than a descriptive label. Evaluation should distinguish PAP adherence from trea

[The importance of addressing issues related to bilateral cataract surgery].

Age-related cataract, the primary cause of blindness among middle-aged and elderly populations, represents a clinical condition where the bilateral surgical strategy cannot be simplified to the superposition of two independent operations, but constitutes a systematic project targeting the reconstruction of binocular visual function. With the increasing diversity of presbyopia-correcting intraocular lenses, clinical decision-making for bilateral cataract surgery has encountered new challenges: how to formulate a reasonable bilateral cataract surgical protocol? This paper reviews the clinical tr

Breast masses: mammographic evaluation.

The systematic mammographic evaluation of a breast mass involves independent assessments of its size, location, density, shape, clarity of margins, and interval change from prior examination. Additional fine-detail mammograms should be obtained to facilitate this analysis, especially when an equivocal interpretation is planned. Definitively benign masses (those localized to the skin, of fat density, or of mixed density) will not require more attention. Among the remaining water-density lesions, those that have an even slightly stellate appearance should be considered suspicious for malignancy;
