In a pilot study, DOTA-d-Phe(1)-Tyr(3)-octreotide (DOTATOC), which can be labelled with the beta-emitting radioisotope yttrium-90, has recently been used for the treatment of patients with advanced somatostatin receptor-positive tumours who had no other treatment option. The aim of the present work was to obtain stabilized high specific activity (HSA) (111)In-labeled bombesin conjugates for preclinical evaluations. Parameters influencing the kinetics of labeling were investigated and the effect of stabilizers on HSA radiopeptides stability at room temperature were systematically categorized applying chromatography techniques. A SA of 174 GBq/μmol was achieved with high radiochemical purity, but the labeled compounds exhibited low stability. The addition of stabilizers avoided their radiolysis and significantly increased their stability.

A 54-yr-old woman with cutaneous nodules showed accumulation of 67Ga- and 111In-labeled leukocytes in skin nodules, bilateral pulmonary hilar lymph nodes and the right upper mediastinum. Biopsy of the skin nodule proved epithelioid cell granuloma with moderate lymphocyte infiltration peripherally consistent with sarcoidosis. Sarcoidosis is a potential pitfall in labeled leukocyte imaging, which is performed for detecting a febrile focus.

A miniature osmotic pump has been developed (Osmet) with ALZA, Palo Alto, USA, which can be swallowed, will pass through the stomach and small intestine and then deliver its contents (240 microliters) over eight hours in the large bowel. In vitro studies showed the pumps started to discharge after four to five hours and emptied at a reasonably constant rate of 20.4 microliters/h from 9-16 h (9.6%/h). Scintigraphic detection of infectious foci with In-111 labeled granulocytes or leukocytes is a well-established technique in nuclear medicine. The technique is commonly used to demonstrate inflammatory activity in Crohn's disease and ulcerative colitis. Its application, however, may be influenced by infectious expectoration located within the gastrointestinal tract. In performing this technique, it is important to have this differential diagnostic possibility in mind; it can be clarified by repeated imaging.

The use of isotope scanning of the spleen in conjunction with 51Cr-labelled red blood cells has become an established technique in the evaluation of patients with hypersplenism. As far as we are aware the similar technique using labelled white blood cells to demonstrate splenic sequestration in a neutropenic patient has not been described. We report a case where this technique proved valuable in confirming the diagnosis and in predicting a favourable response to splenectomy.

A 25-year-old man had clinical features suggesting a diagnosis of amebic abscess of the liver. Ultrasound examination showed an encapsulated lesion in the right lobe of the liver consistent with amebic or pyogenic abscess. In-111-leukocyte scan of the liver showed a rim of increased activity surrounding a photopenic "cavity"; Tc-99m-sulfur colloid scan showed a larger defect. The combined appearance of the scans was thought to be typical of those expected for amebic abscess. Metronidazole therapy was started and the patient made a good recovery.

A patient with severe alcoholic liver disease, in whom there was almost complete suppression of function of the reticuloendothelial system of the liver as noted on a 99mTc-SC scan is discussed. Obstructive biliary disease was suggested on 131I-rose bengal imaging. The depressed RES function was confirmed on 111In-chloride scan, showing abnormal uptake in the kidneys which could be secondary to inflammatory infiltration in the renal parenchyma from urinary tract infection with gamma enterococci.

Many complications can occur following insertion of silicone elastomer implants into the foot. Postoperative infection may be difficult to distinguish from other conditions such as dislodgment, fracture, ectopic and heterotopic new bone formation, synovitis, and bursitis. White blood cell scintigraphy, in conjunction with the clinical scenario, may prove to be an invaluable tool in the diagnosis of postoperative osteomyelitis, subsequent to implant arthroplasties.

Labelling platelets with 111 Indium-oxine has advantages over the conventional 51 Chromium method: labelling is more efficient and the radiations emitted almost exclusively consist of gamma-rays. Owing to these advantages, autologous platelets can be used for kinetic studies in patients with idiopathic thrombocytopenic purpura, even when thrombocytopenia is severe. 111 Indium labelling also provides accurate information on the sites of platelet destruction, which may help to predict the patient's response to splenectomy.
