Intravesical instillation of Bacillus Calmette-Guerin is one of the standard treatment options for superficial bladder cancer. While Bacillus Calmette-Guerin therapy is usually well tolerated with most patients experiencing only cystitis, in rare cases, it can lead to disseminated granulomatous disease. We present a case of a 72-year-old man with disseminated granulomatous disease from intravesical BCG instillation whose treatment was complicated by antimycobacterial drug toxicity.

We studied the intraocular pharmacokinetics of dorzolamide hydrochloride eye drops and the effect of dorzolamide on carbonic anhydrase activity and localization in ocular tissues. Carbonic anhydrase activity was detected in normal ocular tissues. The study was carried out on the eyes of patients (aged 20-75 eyes) without glaucoma and with primary open-angle glaucoma (POAG). PURPOSE OF REVIEW: Nonmuscle-invasive bladder cancer is a common malignancy, standardly treated by transurethral resection and adjuvant intravesical instillations of chemotherapy or immunotherapy. Adjuvant instillations are needed to reduce recurrence and/or progression. With currently existing adjuvant treatments, however, recurrences still occur, progression is hardly influenced and these treatments induce local or systemic side effects. In search for efficacy improvement and side effect reduction, we reviewed the promising properties of the relatively new intravesical drug, gemcitabine.

A 54-year-old man who had received intravesical instillation of bacillus Calmette-Guerin(BCG) after transurethral resection for bladder cancer suffered from multiple arthritis, bilateral conjunctivitis, miction pain and high fever. Under the diagnosis of Reiter's syndrome, a nonsteroidal anti-inflammatory drug, histamine antagonists, anti-tubercular agent and corticosteroid were administered. The symptoms were improved within one month. It is important in early diagnosis and treatment of Reiter's syndrome to observe carefully the complications following intravesical instillation of BCG.
