Salicylates as hypoglycemic agents.

Salicylates lower blood sugar and enhance glucose-stimulated insulin secretion in normal and diabetic man. Although the hypoglycemic effect appears to be mediated by enhanced insulin secretion, extrapancreatic mechanisms cannot be excluded. The mechanism of the enhanced insulin secretion appears to be mediated by prostaglandin synthesis inhibition. The effect of salicylates on insulin secretion mediated by other secretagogues is imprecisely known. In addition to their glucose lowering action, salicylates have been reported to lower free fatty acids, triglycerides, and cholesterol. Salicylates

Exploring functional beta-cell heterogeneity in vivo using PSA-NCAM as a specific marker.

BACKGROUND: The mass of pancreatic beta-cells varies according to increases in insulin demand. It is hypothesized that functionally heterogeneous beta-cell subpopulations take part in this process. Here we characterized two functionally distinct groups of beta-cells and investigated their physiological relevance in increased insulin demand conditions in rats. METHODS: Two rat beta-cell populations were sorted by FACS according to their PSA-NCAM surface expression, i.e. beta(high) and beta(low)-cells. Insulin release, Ca(2+) movements, ATP and cAMP contents in response to various secretagogues

Eriodictyol stimulates insulin secretion through cAMP/PKA signaling pathway in mice islets.

Eriodictyol, a flavonoid isolated from Lyonia ovalifolia, was found to be the most potent insulin secretagogue in our preliminary studies. Here, we explored mechanism(s) of insulin secretory activity of eriodictyol in vitro and in vivo. Mice islets and MIN6 cells were incubated in basal and stimulatory glucose containing eriodictyol with or without agonist/antagonist. Secreted insulin and cAMP contents were measured using ELISA kits. K+- and Ca2+-channels currents were recorded with patch-clamp technique. Oral glucose tolerance test and plasma insulin was evaluated in non-diabetic and diabetic

Increased methylation of endogenous 20-kDa protein in HIT beta-cell during insulin secretion.

Enzymatic methylation of endogenous proteins in clonal pancreatic beta-cell, HIT-T15, was investigated. When cell extract incubated with S-adenosyl-L-[methyl-3H]methionine was subjected to SDS-PAGE followed by fluorography, endogenous 20-kDa protein was highly [methyl-3H]-labeled. The increase of methylation was correlated with insulin secretion, when the cells were treated with secretagogue; at 5.5mM glucose, insulin secretion increased by 2.5-fold, while the 20-kDa methylation to about 3.2-fold. In the case of forskolin, another secretagogue, at 0.1mM, the methylation increased by approximat

Recombinant human GHRH(1-44)NH2: clinical utility and therapeutic development program.

Growth hormone (GH) secretagogues are becoming increasingly attractive alternatives to GH or insulin-like growth factor-I (IGF-I) for the treatment of conditions that may benefit from activation of the GH/IGF-I axis. This stems from the realization that (1) GH secretagogues stimulate the pulsatile release of endogenous GH; (2) feedback control of endogenous GH and IGF-I is preserved, guarding against imbalances between GH and IGF-I levels; and (3) GH treatment is associated with adverse effects in the elderly. Of the GH secretagogues, growth hormone-releasing hormone (GHRH) remains the best ch

Effects of two cholecystokinin variants, CCK-39 and CCK-8, on basal and stimulated insulin secretion.

The effects of two different cholecystokinin variants, CCK-39 and the carboxyl-terminal octa-peptide CCK-8, on basal and stimulated insulin secretion were studied in the mouse. It was found that both peptides had a dose-dependent stimulating action on insulin secretion with a maximal response of similar magnitude at a dose level of about 5 nmol/kg body weight. This dose induced an increase of plasma concentrations of immunoreactive insulin of about 100 microU/ml. The calculated half-maximal dose was 2.12 nmol/kg for CCK-39 and 3.18 nmol/kg for CCK-8. The insulin-secretory response to CCK-39 an

Elevated beta-cell calmodulin produces a unique insulin secretory defect in transgenic mice.

Transgenic mice with elevated levels of beta-cell calmodulin develop severe diabetes even though pancreatic beta-cells contain reserve levels of insulin. Electron microscopic examination of transgenic pancreas confirmed the presence of abundant insulin secretory granules and failed to reveal obvious morphological abnormalities. These observations suggested that excess calmodulin may specifically impair the secretory process. To directly assess the effect of excess calmodulin on beta-cell function we have isolated pancreatic islets from transgenic animals. Transgenic islets from 6- to 8-day-old

Role of endogenous nitric oxide in the control of exocrine and endocrine pancreatic secretion in humans.

BACKGROUND: Nitric oxide (NO) is an unstable vasodilator formed by NO synthetase (NOS) from L-arginine (L-Arg) in various cells but its role in the control of pancreatic secretion in humans has not been examined. AIMS: This study was designed to determine the role of endogenous NO in the control of exocrine and endocrine pancreas using NOS inhibitor, NG-monomethyl-L-Arg (L-NMMA). METHODS: Pancreatic secretion was stimulated by intravenous infusion of secretin (80 pmol/kg/h) plus caerulein (50 pmol/kg/h) and duodenal content was aspirated by gastroduodenal tube. Pancreatogenic diabetes: special considerations for management.

BACKGROUND/AIMS: Pancreatogenic, or type 3c, diabetes (T3cDM) occurs due to inherited or acquired pancreatic disease or resection. Although similar to the more prevalent type 1 and type 2 diabetes, pancreatogenic diabetes has a unique pattern of hormonal and metabolic characteristics and a high incidence of pancreatic carcinoma in the majority of patients with T3cDM. Despite these differences, no guidelines for therapy have been described. METHODS: Published studies on the prevalence, pathophysiology, and cancer associations of T3cDM were reviewed. The recent studies on the protective role an

CMR12, a novel insulinotropic compound, enhances second-phase insulin secretion with metabolic changes in pancreatic β-cells.

Insulin secretion from pancreatic β-cells is primarily regulated by glucose stimulation, while cAMP-mediated amplifying pathways also play an important role. In this study, we identified compound 42 (C42) as an insulinotropic hit compound through an in silico similarity search using the cAMP analog Sp-cAMPS as a query. Further structural development by chemical synthesis yielded CMR12, a novel derivative with enhanced insulinotropic activity. This class of compounds enhances insulin secretion in MIN6-K8 cells in a glucose concentration-dependent and compound concentration-dependent manner, and

Acylhydrazones contribute to serum glucose homeostasis through dual physiological targets.

In this study the in vivo and in vitro antidiabetic effects of four acylhydrazone derivatives were investigated in rats. The secretagogue action, oral glucose tolerance, insulinogenic index and mechanism of action of these acylhydrazones in relation to calcium uptake in pancreatic islets were studied. Also, the insulinomimetic effect on glycemia in diabetic rats was verified. Of the acylhydrazones studied, 1 and 4 were able to increase glucose tolerance in an acute time-course. A powerful secretagogue effect was exhibited by 1 and glibenclamide with an insulinogenic index around 3.9 and 1.3-fo

O-Acetyl-Serine Supplementation Enhances Insulin Secretion and Improves Postprandial Glycaemia in Lean and Prediabetic Mice.

Type 2 Diabetes (T2D), often preceded by reversible prediabetes, poses a major health challenge. Targeting early glucose dysregulation is a promising preventive strategy. Amino acids and their derivatives are emerging as key regulators of glucose homeostasis. Here, we investigate the role of O-acetyl-serine (OAS), a serine-derived metabolite produced by plants and microbes, including those of the gut microbiota, in glycaemia regulation. We developed a targeted method to quantify OAS in plasma and tissues, showing that it enters circulation and transiently accumulates in the pancreas. In vivo,

Inhibitory effect of ghrelin on insulin and pancreatic somatostatin secretion.

OBJECTIVE: Ghrelin is a 28 amino acid residue peptide identified in both human and rat stomach and which acts as an endogenous ligand for the GH secretagogue receptor (GHS-R) and stimulates GH release. GHS-Rs are expressed in a number of tissues, including the pancreas, and ghrelin-like immunoreactivity is present in peripheral plasma, where its levels increase during fasting and decrease after food intake. The relationship between nutritional status and circulating ghrelin concentrations prompted us to investigate the effect of this peptide on pancreatic hormone secretion. METHODS: The study

Three Decades of Glimepiride Analysis: A Review of Analytical and Bioanalytical Methods.

Glimepiride, a third-generation sulfonylurea, is widely prescribed for the management of Type 2 diabetes mellitus. Since its introduction, numerous analytical and bioanalytical methods have been reported. However, the available information is scattered across the literature and lacks a consolidated evaluation of the analytical strategies developed over time. The published methods predominantly address either analytical or bioanalytical approaches individually; however, no single review comprehensively encompasses both domains. Therefore, this review seeks to bridge that gap by offering an inte

Fern-9(11)-ene-2α,3β-diol Action on Insulin Secretion under Hyperglycemic Conditions.

The objective of this study was to investigate the effect and the mechanism of action of fernenediol as an insulin secretagogue. Wistar rats were treated with 0.1, 1, and 10 mg/kg fernenediol before inducing hyperglycemia by oral glucose. The glycaemia, insulin, LDH, calcium, and hepatic glycogen were analyzed. Considering the intestine and pancreas as targets for the triterpene action, the duodenum was used to verify the influence of fernenediol on intestinal glycosidases. Additionally, pancreatic islets were used for studies of 14C-deoxyglucose uptake and the influx of 45Ca2+ in hyperglycemi

Influence of dipeptidyl peptidase-4 inhibitors on glycemic variability in patients with type 2 diabetes: A meta-analysis of randomized controlled trials.

OBJECTIVE: The influence of dipeptidyl peptidase-4 (DPP4) inhibitors on glycemic variability compared to other oral antidiabetic drugs (OADs), measured based on the mean amplitude of glycemic excursions (MAGE), has not been comprehensively analyzed. The aim of the study was to perform a meta-analysis to compare the effects of DPP4 inhibitors on MAGE with other OADs in type 2 diabetes mellitus (T2DM) patients without concurrent insulin treatments. METHODS: The Medline (PubMed), Embase (Ovid), and CENTER (Cochrane Library) databases were searched for relevant randomized controlled trials (RCTs)

Successful Transition to Sulfonylurea for Relapsed Monogenic Diabetes Due to Rare 6q23.3 Duplication.

Transient neonatal diabetes mellitus (TNDM) due to 6q duplication usually presents in the first 4 months of life, resolves before 18 months of life, and recurs in adolescence or adulthood. Insulin is the first-line treatment for chromosome 6-related neonatal diabetes in infancy. While there is no ideal treatment for patients with relapsed TNDM, residual β-cell function after remission of neonatal diabetes indicates a potential role for insulin secretagogues. Patients with 6q24 duplication have been successfully transitioned from insulin to sulfonylureas (SUs) in adolescence. We present the fir

Phenotypic homogeneity of the Huntington disease-like presentation in a SCA17 family.

Diabetes is caused by combined abnormalities in insulin production and action. The pathophysiology of these defects has been studied extensively and is reasonably well understood. Their causes are elusive and their manifestations pleiotropic, likely reflecting the triple threat of genes, environment, and lifestyle. Treatment, once restricted to monotherapy with secretagogues or insulin, now involves complex combinations of expensive regimens that stem the progression but do not fundamentally alter the underlying causes of the disease. As advances in our understanding of insulin action and β-ce

Five-year follow-up of incretin-based therapy in a patient with KCNJ11-related permanent neonatal diabetes: Achieving insulin discontinuation and sulfonylurea reduction.

Permanent neonatal diabetes mellitus (PNDM) is commonly caused by pathogenic variants in KCNJ11 that impair insulin secretion through ATP-sensitive potassium (KATP) channel dysfunction. Sulfonylureas (SUs) can stimulate insulin secretion and enable insulin discontinuation, but insulin withdrawal may remain difficult even under high-dose SU, especially when therapy is initiated later in life. We describe a woman with KCNJ11-related PNDM who achieved insulin discontinuation after adding sitagliptin, followed by further improvement with semaglutide during a five-year follow-up.
