Evaluation of the South Korean national long-term care insurance-funded cognitive function training programme for older people with mild dementia.

OBJECTIVES: despite the extensive literature on the effectiveness of cognitive training, its effectiveness has not been demonstrated within a population-based long-term care system. To provide cognitive training services to older people with mild dementia, Korea introduced a special dementia rating, as a long-term care grades, in the national long-term care insurance in July 2014. These services are only offered to those with the special dementia rating. This study evaluated the national long-term care insurance-funded cognitive function training programme for older people with mild dementia.

Care needs level in long-term care insurance system and family caregivers' self-perceived time-dependent burden in patients with home palliative care for cancer: a cross-sectional study.

PURPOSE: Although home care improves patients' quality of life (QOL), several studies have suggested that home care lowers the QOL of family caregivers and decreases their mortality. To alleviate the deleterious impact of home care on caregivers, the major burdens on caregivers and the clinical characteristics of the caregivers vulnerable to the major burden needs to be clarified. METHOD: A survey questionnaire was distributed to 710 family caregivers of patients with cancer in Japan, and 342 valid responses were obtained (valid response rate: 48.2%). The Burden Index of Caregivers was used t

Does the Pilot Long-Term Care Insurance Work as Expected in China? Examining its Impact on Hospital Utilizations, Medical Expenses, and Health.

This study investigates the impact of long-term care insurance (LTCI) on older adults' hospital service utilization, medical expenses, and health. Using national representative data from four waves of CHARLS (2011, 2013, 2015, 2018), we find that LTCI decreases older adults' monthly outpatient visits but is associated with an increase in both annual inpatient stays and inpatient medical expenses. Further evidence suggests that older adults covered by the pilot LTCI program have better health status compared to their counterparts in non-pilot cities. Overall, findings offer empirical support fo

Reconsidering the Caregiving Stress Appraisal scale: validation and examination of its association with items used for assessing long-term care insurance in Japan.

The Caregiving Stress Appraisal (CSA) scale is a simple scale for measuring family caregiver stress. However, information on its validity and association with new social welfare systems in Japan is inadequate. The purpose of the present study is to examine the criterion validity of the CSA and to explore its association with the variables used for assessing long-term care insurance in Japan. In the present study, 219 family caregivers completed a self-administered questionnaire. Of these, 50 and 202 caregivers were used for the analysis of the criterion validity and construct validity, respect

Long-Term Care Insurance Pilot Programme in China: Policy Evaluation and Optimization Options-Taking the Pilot Programme in the Northeast of China as an Example.

China's population is ageing rapidly and the increase in life expectancy is accompanied by a loss of capability with advancing age, especially in the Northeast. This study adopts qualitative research methods to analyze the overall status and problems of China's LTCI policy pilots. Taking four LTCI pilot cities in three northeastern provinces as samples, we used purposive sampling to recruit 10 beneficiaries and providers of LTCI in nursing homes of different kinds, as well as 2 operators (Medical Insurance Bureau staff) for semi-structured in-depth interviews. We developed a social welfare pol

Who is protected and who is not? Evaluating the effects of the long-term care insurance system and disparities in healthcare utilization and costs of the South Korean elderly population.

BACKGROUND: There is a growing global interest in investigating the substitutability between long-term care service (LTCS) and medical care use, particularly in countries where elderly medical care use is rapidly increasing. The use of an appropriate LTCS can reduce unnecessary medical utilization and the resulting costs such as those for hospital admission, delayed discharge, and emergency room visits. METHODS: This study evaluated whether the LTCS has a savings effect on medical use for older adults and whether the effect varies depending on income level, using longitudinal data obtained fr
