Methodological Approaches to Real-World Evidence Generation for Glucagon-like Peptide-1-Based Therapies: Synopsis of a National Institute of Diabetes and Digestive and Kidney Diseases Workshop.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have reshaped the clinical approach to managing obesity and type 2 diabetes. As approved indications have expanded, use of GLP-1RAs has increased rapidly in the United States. Although randomized trials demonstrate strong efficacy, many questions remain about their optimal use in clinical practice. Real-world data (RWD) from electronic health records, registries, insurance claims, and other sources offer a promising avenue to address these questions. However, concerns about data quality, selection bias, and incomplete ascertainment of medica

Promotion or suppression? Does internet use by the Chinese public affect physician trust?

OBJECTIVE: This study aims to explore the causal relationship between internet use and the level of physician trust among Chinese residents, providing rigorous empirical evidence for understanding the changes in doctor-patient relationships in the digital era. METHODS: Using four waves of panel data from the China Family Panel Studies (CFPS) spanning 2016 to 2022, we construct a two-way fixed effects model to control for individual and time heterogeneity. We further employ propensity score matching (PSM) to alleviate sample selection bias, thereby robustly examining the relationship between i

Identifying Likely Incidents of Traumatic Spinal Cord Injury and Elevated Risk of Alzheimer's Disease and Related Dementia: Health Care Cost Institute Data.

OBJECTIVES: To examine the association between incident traumatic spinal cord injury (TSCI) and the risk of Alzheimer's disease and related dementia (ADRD). METHODS: This is a longitudinal case-control study. We used 2012-2019 commercial insurance claims data from the Health Care Cost Institute (HCCI) and applied semi-parametric Cox survival models to compute hazard ratio (HR) for ADRD diagnosis comparing cases with incident TSCI with their controls without TSCI, adjusting for age, sex, a set of comorbid conditions, and any use of 6 classes of prescription medications. A Comparison of Matching and Weighting Methods for Causal Inference Based on Routine Health Insurance Data, or: What to do If an RCT is Impossible.

Due to a multitude of reasons Randomized Control Trials on the basis of so-called "routine data" provided by insurance companies cannot be conducted. Therefore the estimation of "causal effects" for any kind of treatment is hampered since systematic bias due to specific selection processes must be suspected. The basic problem of counterfactual, which is to evaluate the difference between two potential outcomes for the same unit, is discussed. The focus lies on the comparison of the performance of different approaches to control for systematic differences between treatment and control group. Th

Insights into the protective effects of influenza vaccination: More hospitalizations but lower follow-up mortality during the 2014/15 influenza season in a Swiss cohort.

BACKGROUND: Observational studies of influenza vaccination are criticized as flawed due to unmeasured confounding. The goal of this cohort study was to explore the value and role of secondary claims data to inform the effectiveness of influenza vaccination, while systematically trying to reduce potential bias. METHODS: We iteratively reviewed the components of the PICO approach to refine study design. We analyzed Swiss mandatory health insurance claims of adult patients with chronic diseases, for whom influenza vaccination was recommended in 2014. Analyzed outcomes were all-cause mortality, h

Incidence of autoimmune disease after hernia surgery with a mesh implant: national retrospective cohort study.

BACKGROUND: Some patients report experiencing various systemic symptoms after surgical placement of a (polypropylene (PP)) mesh implant, including fatigue, arthritis, and myalgia. As these complaints could resemble systemic symptoms of autoimmune disease (AID), there are concerns that mesh implants could induce such disorders. The aim of this study was to identify the incidence of AID after mesh implantation, compared with controls in whom no mesh was implanted. METHODS: In this national retrospective cohort study, approximately 4 million people insured with Dutch health insurer Coöperatie VG

Glomerular Injury Findings in Patients with Thalassemia Minor.

Renal involvement in β-thalassemia minor (β-TMin) has been described mainly in case reports and small observational studies, and its clinical significance remains incompletely characterized. Using real-world data from routinely collected electronic medical records, we performed a retrospective cohort study including 1516 adult patients with β-TMin insured by Clalit Healthcare Services to explore renal abnormalities identified during routine clinical care. Urine testing for hematuria, microalbuminuria, and proteinuria was not performed systematically but was ordered at clinicians' discretion, r

CHS-DRG 2.0 and insurance-specific patient cost sharing in gestational diabetes admissions: a regression discontinuity in time study.

OBJECTIVES: Diagnosis-related group reform aims to restrain inpatient spending, but obstetric evaluations commonly emphasize average hospital costs rather than insurance-specific patient cost sharing. We evaluated early discontinuities associated with CHS-DRG 2.0 in hospitalization costs, out-of-pocket (OOP) payment, self-pay ratio, and selected perinatal outcomes among gestational diabetes mellitus (GDM) admissions. METHODS: We analyzed 8,690 singleton GDM delivery admissions at a tertiary maternal and child health hospital in Fujian, China. The main cohort excluded eight stays exceeding 60

Clinical perceptions of the risk of vertebral artery dissection after cervical manipulation: the effect of referral bias.

BACKGROUND CONTEXT: The growing recognition of cervical manipulation as a treatment of neck pain and cervicogenic headaches has lead to increased interest in potential complications that may result from this treatment approach. Recent surveys have reported that many neurologists will encounter cases of vertebral artery dissection that occur at various times after cervical manipulation, whereas most practitioners of spinal manipulation are of the opinion that these events are extremely rare. We asked the question whether these differences in perception could be explained in part by referral or

The Benefits and Bias in Neurodevelopmental Evaluation for Children with Congenital Heart Disease.

Neurodevelopmental (ND) impairment is common in children with congenital heart disease (CHD). While routine ND surveillance and evaluation of high-risk patients has become the standard-of-care, capture rate, barriers to referral, and potential patient benefits remain incompletely understood. Electronic data warehouse records from a single center were reviewed to identify all eligible and evaluated patients between July 2015 and December 2017 based on current guidelines for ND screening in CHD. Diagnoses, referring provider, and payor were considered. Potential benefit of the evaluation was def

Using Focus Groups to Understand Dialysis Staff Perspectives on Delivering Transplant Education.

INTRODUCTION: Kidney transplant education in dialysis facilities could be optimized with internet resources, like videos, but most qualitative research predates widespread availability of online video education about kidney transplantation. To improve understanding of dialysis staff transplant education practices, as well as the potential value of video, we conducted focus groups of dialysis center staff members in Buffalo, NY. METHODS/APPROACH: Seventeen focus groups (97 participants: 53 nurses, 10 dialysis technicians, 6 social workers, 6 dieticians, 7 administrative personnel, 2 trainees,

Health administrative data enrichment using cohort information: Comparative evaluation of methods by simulation and application to real data.

BACKGROUND: Studies using health administrative databases (HAD) may lead to biased results since information on potential confounders is often missing. Methods that integrate confounder data from cohort studies, such as multivariate imputation by chained equations (MICE) and two-stage calibration (TSC), aim to reduce confounding bias. We provide new insights into their behavior under different deviations from representativeness of the cohort. METHODS: We conducted an extensive simulation study to assess the performance of these two methods under different deviations from representativeness of
