Biofilm Production in Intensive Care Units: Challenges and Implications.

The prevalence of infections amongst intensive care unit (ICU) patients is inevitably high, and the ICU is considered the epicenter for the spread of multidrug-resistant bacteria. Multiple studies have focused on the microbial diversity largely inhabiting ICUs that continues to flourish despite treatment with various antibiotics, investigating the factors that influence the spread of these pathogens, with the aim of implementing sufficient monitoring and infection control methods. Despite joint efforts from healthcare providers and policymakers, ICUs remain a hub for healthcare-associated infe

[Therapeutic healthcare professional staffing requirements in intensive care units].

BACKGROUND: Therapeutic healthcare professionals in the multiprofessional intensive care unit (ICU) team are important for early mobilization, dysphagia therapy, and psychosocial care of critically ill patients. OBJECTIVE: Despite the high relevance of therapeutic healthcare professions for care in ICUs, there are no recommendations on the specific staffing of therapists in ICUs. RESULTS: Considering the main areas of activity of the individual professional groups and based on productivity time, a requirements analysis for staffing ICUs of different care levels with physiotherapists, occupat

Dysphagia management by nurses in Spanish intensive care units.

INTRODUCTION/PURPOSE: Dysphagia is a disorder that presents with specific signs and symptoms in critically ill patients. Intensive care unit (ICU) nurses are responsible for monitoring and detecting abnormalities in critically ill patients, so they must be trained to assess swallowing and the complications that may arise. The aim of this research is to analyse the dynamics of the detection and assessment of dysphagia by ICU nurses. METHOD: Cross-sectional descriptive study using an electronic questionnaire to nurses from different Spanish ICUs. The survey was adapted from previous research an

Serratia marcescens Infections in Neonatal Intensive Care Units (NICUs).

Serratia marcescens belongs to the family Enterobacteriaceae, which is commonly found in water, soil, animals, insects, plants. Although S. marcescens displays relatively low virulence, it causes nosocomial infections and outbreaks in severely immunocompromised or critically ill patients, particularly in settings such as intensive care units (ICUs), especially neonatal units (NICUs). This microorganism gives rise to a wide range of clinical manifestations in newborns: from asymptomatic colonization to keratitis, conjunctivitis, urinary tract infections, pneumonia, surgical wound infections, se

Prevention of acute kidney injury in Intensive Care Units.

Acute kidney injury (AKI) is a growing concern in Intensive Care Units. The advanced age of our patients, with the increase in associated morbidity and the complexity of the treatments provided favor the development of AKI. Since no effective treatment for AKI is available, all efforts are aimed at prevention and early detection of the disorder in order to establish secondary preventive measures to impede AKI progression. In critical patients, the most frequent causes are sepsis and situations that result in renal hypoperfusion; preventive measures are therefore directed at securing hydration

[Strategies for antifungal treatment failure in intensive care units].

Recent epidemiologic studies reveal both an increasing incidence and an escalation in resistance of invasive fungal infections in intensive care units. Primary therapy fails in 70 % of cases, depending on the underlying pathogens and diseases. The purpose of this review is to raise awareness for the topic of antifungal therapy failure, describe the clinical conditions in which it occurs, and suggest a possible algorithm for handling the situation of suspected primary therapy failure.

Catheter-related bloodstream infections in neonatal intensive care units.

Central venous catheters (CVCs) are regularly used in intensive care units, and catheter-related bloodstream infection (CRBSI) remains a leading cause of healthcare-associated infections, particularly in preterm infants. Increased survival rate of extremely-low-birth-weight infants can be partly attributed to routine practice of CVC placement. The most common types of CVCs used in neonatal intensive care units (NICUs) include umbilical venous catheters, peripherally inserted central catheters, and tunneled catheters. CRBSI is defined as a laboratory-confirmed bloodstream infection (BSI) with e

[Management of acute pleural diseases in intensive care units].

Pleural involvement is frequently seen among patients hospitalized in Intensive Care Units (ICU). In most cases, patients are hospitalized with or will develop scarce transsudative effusion secondary to cardiac failure or atelectasis. Other pleural issues in ICU concern pneumothorax in ventilated patients (barotrauma), empyema following nosocomial pneumonia or investigation procedures. More rarely hemo(pneumo)thorax or chylothorax will be diagnosed. As a rule, acute pleural pathologies rarely justify hospitalization in the ICU, depending on the etiologic mechanism or concomittant clinical sign

Infection control in intensive care units: U.K. national survey.

A completed questionnaire was returned by 246 (85%) intensive care units participating in a national survey of infection control practice in intensive care. Thirty-three units had no provision for isolating patients in single side wards. Sixty percent of responding ICU had fewer than one washbasin per bedspace. Several units reported using ventilator filters or tubing more frequently than is currently recommended. Excessive numbers of catheter urine specimens were sent for laboratory examination by some units. A small number of units used open urinary drainage systems. A significant proportion

[Long time hospitalization in pediatric surgical intensive care units].

If the great majority of children stay in intensive care unit only for few days, a small group of patients stay for a long period of time, varying from 3 months to five years or even more. The small group include two very different entities. On the one hand, newborns with a congenital malformation, often digestive e.a. with a short small intestine, need a long time parenteral nutrition. In such cases, the family disturbance, the opening of the unit to the parents, the awakening of the infant, his psychomotor and affective attainments are as many problems for the whole medical team. [Problems with hyperalimentation in intensive care units (author's transl)].

The application of parenteral nutrition in patients in intensive care units can be limited by shock, septicemia and metabolic disorders. Stress- or trauma-induced glucose intolerance sometimes makes it difficult to maintain the caloric requirements with glucose. Sugar substitutes seem to be of advantage in these cases. Meticulous care is essential to avoid severe complications arising from the use of the central venous feeding catheters.

Perceived barriers to mobility in the intensive care units of Singapore: The Patient Mobilisation Attitudes and Beliefs Survey for the intensive care units.

PURPOSE: Prolonged bed rest and immobility in the intensive care units (ICU) increase the risk of ICU-acquired weakness (ICUAW) and other complications. Mobilisation has been shown to improve patient outcomes but may be limited by the perceived barriers of healthcare professionals to mobilisation. The Patient Mobilisation Attitudes and Beliefs Survey for the ICU (PMABS-ICU) was adapted to assess perceived barriers to mobility in the Singapore context (PMABS-ICU-SG). METHODS: The 26-item PMABS-ICU-SG was disseminated to doctors, nurses, physiotherapists, and respiratory therapists working in I

Management of spontaneous septic hypothermia in intensive care. A national survey of French intensive care units.

The benefit of temperature control in sepsis or septic shock is still under debate in the literature. We developed a national survey to assess the current state of knowledge and the practical management of spontaneous septic hypothermia in French intensive care units. Out of more 764 intensivists who were contacted, 436 responded to the survey. The majority of doctors (52.4%) considered spontaneous septic hypothermia to be a frequently encountered situation in intensive care, and 62.1% were interested in this problem. Definition of spontaneous septic hypothermia among French intensivists was n

[Isolation and restraint in units for difficult patients and in psychiatric intensive care units].

The units for difficult patients (UMD) and the intensive psychiatric care units (Usip) are psychiatric services that are not successively sectorized, created to meet the needs of intensive care in a closed environment and sometimes of a forensic nature. These two systems are used to care for patients whose clinical condition often makes it too complex to maintain them in sector psychiatric units, and many of their operating rules differ. This is not the case for seclusion and restraint measures and the application of the law governing these measures.

Bereavement care practices: A national survey of Australian paediatric intensive care units.

OBJECTIVES: Hospital-based bereavement care is an important source of support for families following the death of a child in a paediatric intensive care unit (PICU) but has been identified as lacking in many critical care units worldwide. Recent surveys exploring bereavement practices in intensive care units have not reported comprehensive paediatric data, meaning the extent of this problem within the PICU setting is currently unknown. The aim of this study was to describe current bereavement care practices in Australia PICUs. DESIGN: A cross-sectional, self-reported, online survey, conducted

Palliative Care in Intensive Care Units: Nurses' Perspectives on Challenges and Strategies.

BACKGROUND: The integration of palliative care into intensive care units is increasingly recognised as essential to ensuring quality end-of-life care, yet persistent barriers continue to challenge its implementation. Nurses, as continuous bedside providers, are central to delivering comfort, but their perspectives remain underexplored in the Portuguese context. AIM: To explore the challenges and strategies identified by intensive care nurses in the provision of palliative care, guided by Kolcaba's Comfort Theory. STUDY DESIGN: A qualitative descriptive design was adopted. Data were collected
