Pressure-time product during continuous positive airway pressure, pressure support ventilation, and T-piece during weaning from mechanical ventilation.

The objective of this study was to compare the effects of continuous positive airway pressure (CPAP), pressure support ventilation (PS), and T-piece on the pressure-time product (PTP) during weaning from mechanical ventilation. The PTP is an estimate of the metabolic work or oxygen consumption of the respiratory muscles. We studied 10 intubated patients recovering from acute respiratory failure of various etiologies. A modified continuous flow (flow-by) CPAP of 0 and 5 cm H2O (CPAP-0 and CPAP-5, respectively), PS of 5 cm H2O (PS-5), and T-piece were applied in random order for 30 min each. In

A historical account of the "wet lung of trauma" and the introduction of intermittent positive-pressure oxygen therapy in world war II.

During World War II, my associates and I observed for the first time in medical history that casualties with severe brain, thoracic, abdominal, and extremity trauma, who had persistent "wet" respiration (wet lung of trauma), were most difficult to resuscitate, withstood operation poorly, and had the highest mortality. The etiology appeared to be ineffectual cough and persistent bronchopulmonary fluid from hemorrhage, pulmonary transudates resulting from anoxia, airway obstruction, and unknown causes secondary to trauma, some of which have been discovered since then. Our treatment consisted of

[Surgical treatment of the flail chest (author's transl)].

Six patients with traumatic flail chest underwent surgical stabilization using a procedure introduced by Brunner, Hoffmeister and Koncz (2). Compared with internal stabilization by intermittend positive pressure respiration (IPPR), there are some advantages: Time of artificial respiration is shortened, early mobilization is possible, nursing is easier. The procedure is simple to do and is indicated in those patients, in whom longterm artificial respiration is not necessary for other reasons.

Surgical stabilization of traumatic flail chest.

Since 1970 we have stabilized the ribs to correct paradoxical movement of the chest wall in chest injuries, using an original technique, in order to avoid as far as possible the need for long-term chest wall stabilization by intermittent positive pressure respiration (IPPR). The technical details of surgical stabilization are described, and the different types of stainless steel struts are shown. Type I was originally used either as an intramedullary nail or as an external brace. Types II and III were designed for external fixation of the strut to the rib. Treatment of 29 patients with severe

Airway resistance in infants after various treatments for hyaline membrane disease: special emphasis on prolonged high levels of inspired oxygen.

Thoracic gas volume, airway resistance (Raw), and dynamic lung compliance (CL) were measured in 48 infants surviving after hyaline membrane disease. Some infants were found to have a small reduction in CL after recovery from the acute phase of the illness but no other abnormalities were detected, irrespective of the type of treatment received. When studied again between the ages of 4 and 10 months, CL had returned to normal, but all infants who had been treated with intermittent positive pressure ventilation (IPPV) during the neonatal period were found to have developed a raised Raw. In contra

Anesthetic challenges of a patient with the communicating bulla coming for nonthoracic surgery.

Management of a patient with a giant bulla coming for a nonthoracic surgery is rare, and its anesthetic management is very challenging. It is imperative to isolate only the subsegmental bronchus, in which the bulla communicates to avoid respiratory morbidities such as pneumothorax, emphysema or atelectasis of the surrounding lung parenchyma, and postoperative respiratory failure. Herewith, we want to report the anesthetic challenges of a patient with giant bulla communicating into one of the subsegmental right upper lobe bronchus for splenectomy.

[Randomized controlled study of targeted tidal volume ventilation for treatment of severe neonatal respiratory distress syndrome].

OBJECTIVE: To evaluate the efficacy of targeted tidal volume ventilation in the treatment of severe neonatal respiratory distress syndrome (RDS). METHODS: Eighty-four neonates with severe RDS between June 2008 and January 2010 were randomly assigned to 3 groups according to the ventilation mode: synchronized intermittent positive pressure ventilation plus volume guarantee (SIPPV+VG; n=31), high frequency oscillation ventilation (HFOV; n=23) and intermittent mandatory ventilation (IMV; n=30). The oxygenation status, the durations of oxygen exposure and ventilation and the incidence of complica

Amyotrophic lateral sclerosis. Successful treatment of mucous plugging by mechanical insufflation-exsufflation.

Bronchial mucous plugging is the main precipitating factor of acute respiratory failure for patients with neuromuscular disease. Manually assisted coughing and mechanical insufflation-exsufflation can effectively eliminate airway secretions without resorting to tracheal intubation provided that bulbar muscle function is sufficient to permit assisted peak cough flows of 160 l/min or greater. We now report successful use of mechanical insufflation-exsufflation to prevent acute respiratory failure for a patient with amyotrophic lateral sclerosis with no measurable peak cough flow.

[Comparative study of venous contamination with low FiO2 and with pure oxygen in disease-induced pulmonary edema].

A comparative study of the venous contamination (Qs/Qt = (Cc'O2 - CaO2) / (Cc'O2 - C-vO2) under FIO2 = 0.45 - 0.6 and under FIO2 = 1, was realised in 9 patients with lesions of pulmonary oedema. Increasing the FIO2 lead to an increase in the venous contamination three times (average increase was 36 p. 100 of the initial value), to no change two times and a decrease four times (average decrease of 34 p. 100). The values of the venous contamination under average FIO2 are statistically lower in the group where it rises on increasing the FIO2 than in that in which it falls or remains unchanged (p

The treatment of flail chest injury.

Flail chest injuries are traditionally managed by long-term artificial ventilation, which in itself is hazardous and expensive. A more conservative therapeutic regime is described, and the results of this regime are compared with those following the previous traditional regime used in the Respiratory Unit, Royal Brisbane Hospital. The place of artificial ventilation in relation to spontaneous ventilation is redefined in the context of this new regime.

[Primary alveolar hypoventilation in adulthood].

BACKGROUND: Central alveolar hypoventilation in an adult is characterized by dysfunction of the respiratory center in the brainstem and is very rare, seen mostly secondary to neurological lesions of the brainstem. CASE DESCRIPTION: We report a 48-year-old man with primary alveolar hypoventilation associated with marked cardiac arrhythmias and hemodynamic changes. Arterial blood gas analysis revealed daytime hypoxemia and hypercapnia not explained by normal pulmonary results. All night polysomnography showed sleep fragmentation following repetitive central apneas and hypoventilation with marke

Modern Spirometry Supports Anesthetic Management in Small Animal Clinical Practice: A Case Series.

Modern spirometry, like no other monitoring technique, allows insight into breath-to-breath respiratory mechanics. Spirometers continuously measure volume, airway pressure, and flow while calculating and continuously displaying respiratory system compliance and resistance in the form of loops. The aim of this case series is to show how observation of spirometric loops, similar to electrocardiogram or CO2 curve monitoring, can improve safety of anesthetic management in small animals. Spirometric monitoring cases described in this case series are based on use of the anaesthesia monitor Capnomac

Oxygen cost of breathing and weaning process in newborn infants.

Newborn infants may have a high oxygen cost of breathing (OCB) at the time of being weaned from mechanical ventilation. We hypothesized that this increase in oxygen consumption (V'O2) could be reduced by using certain weaning ventilatory modes. We designed a study to assess V'O2 during three weaning ventilatory modes: patient triggered ventilation, synchronous intermittent mandatory ventilation (SIMV) and continuous positive airway pressure in 16 newborn infants before being weaned from mechanical ventilation In seven infants whose OCB was high. V'O2 was not significantly different between CV

Ventilatory muscle support in respiratory failure with nasal positive pressure ventilation.

Long-term intermittent mechanical ventilation results in improvements in ventilatory performance and clinical status between ventilation sessions in patients with chronic respiratory failure. The application of intermittent positive pressure ventilation through a nasal mask (NPPV) is a simple, noninvasive method for the provision of chronic intermittent ventilatory support. We investigated the effects of NPPV on inspiratory muscle activity in three normal subjects and nine patients with acute or chronic ventilatory failure due to restrictive (four subjects) or obstructive (five subjects) respi

An Anesthesiologist's Perspective on the History of Basic Airway Management: The "Modern" Era, 1960 to Present.

This fourth and last installment of my history of basic airway management discusses the current (i.e., "modern") era of anesthesia and resuscitation, from 1960 to the present. These years were notable for the implementation of intermittent positive pressure ventilation inside and outside the operating room. Basic airway management in cardiopulmonary resuscitation (i.e., expired air ventilation) was de-emphasized, as the "A-B-C" (airway-breathing-circulation) protocol was replaced with the "C-A-B" (circulation-airway-breathing) intervention sequence. Basic airway management in the operating roo

[Augmented spontaneous breathing].

Impaired pulmonary gas exchange can result from lung parenchymal failure inducing oxygenation deficiency and fatigue of the respiratory muscles, which is characterized by hypercapnia or a combination of both mechanisms. Contractility of and coordination between the diaphragm and the thoracoabdominal respiratory muscles predominantly determine the efficiency of spontaneous breathing. Sepsis, cardiac failure, malnutrition or acute changes of the load conditions may induce fatigue of the respiratory muscles. Augmentation of spontaneous breathing is not only achieved by the application of differen

[Normal tension glaucoma, sleep apnea syndrome and nasal continuous positive airway pressure therapy--case report with a review of literature].

BACKGROUND: In the pathogenesis of glaucoma, besides an elevated intraocular pressure (IOP), cardiovascular risk factors, such as arterial hypotension and hypertension, vasospasms, autoregulatory defects, atherosclerosis, and diabetes mellitus are of increasing importance, especially in normal tension glaucoma. Recently, there have been several reports of an additional risk factor: obstructive sleep apnea syndrome. METHODS: Literature review (Medline) and case report. RESULTS: The authors report on a 8 1/2 years follow-up of a 60-year-old patient with normal tension glaucoma. Despite success

Fatal air embolism after subclavian vein catheterization. Case report.

The authors describe a case of fatal air embolism in a patient with an endovenous catheter introduced into the subclavian vein through a supraclavicular way. The opening of pleura caused by the needle during the thrusting of the needle caused a subcutaneous supraclavicular and laterocervical emphysema during ventilation with intermittent positive pressure (IPPV). The authors suggest that IPPV, in the postoperative period following kidney removal, was the main ancillary reason of the air embolism since opening of pleura, in a patient who breathes spontaneously, only induces pneumothorax accordi
