We report results from 108 consecutive patients followed up for one year after chymopapain injection. The patients were selected on strict clinical and radiological criteria. In nine patients (8%) the result was poor, and they were surgically treated within one year after the injection. 99 (92%) patients improved and after one year 87 of these (81% of the total material) had returned full time to their previous occupation. A randomized clinical trial was conducted to compare the results of automated percutaneous discectomy with those of chemonucleolysis in 141 patients with sciatica caused by a disk herniation; 69 underwent automated percutaneous discectomy and 72 were subjected to chemonucleolysis. During a 6-year period from 1983 to 1989 we performed lumbal discography in 403 disc levels on 144 patients. We used the discography to determine the therapeutic way in patients with chronic back pain. In the greater half it was possible to define the symptomatic level exactly. About 90% the discography was very helpful to get a diagnosis or to plan the therapy. The reproduction of pain with discography is helpful in the assessment of abnormal disc levels. Even today in view of CT and MRI.

Disk herniation can manifest as isolated low back pain, which is usually intermittent and accompanied with stiffness of the lumbar spine. There is almost general agreement that neither neurosurgical treatment nor chemonucleolysis is appropriate in patients with this clinical pattern. We used chemonucleolysis in 19 patients with recurrent episodes of isolated low back pain and one patient with severe permanent isolated low back pain dating back to a conservatively-treated episode of sciatica. The purpose of this study was to evaluate the outcome after chymopapain chemonucleolysis in elderly patients with low back pain and sciatica due to lumbar disc herniation without concomitant stenosis, unresponsive to conservative therapy. PURPOSE OF THE STUDY: We studied the efficacy of papaine for treatment of herniated discs at a mean 10-year follow-up and compared results with other series and other treatments.
MATERIAL AND METHODS: From an initial group of 160 patients, 96 patients, 53 men and 43 women, mean age 39 years, were selected for evaluation. After treatment of a symptomatic herniated disc with chymopapain, 14 patients were re-examined by magnetic resonance imaging (MRI) at a mean follow-up of 72 months. Epidural adhesions were discovered in seven of nine patients undergoing surgery for failed chymopapain chemonucleolysis. Clinically the adhesions were very vascular, prolific and adherent to the nerve root, dura and the porieties. The decision about operative or non-operative treatment of herniated lumbar discs has to be based on the results of clinical and neurological examination. OBJECTIVE Chemonucleolysis with condoliase has the potential to be a new, less invasive therapeutic option for patients with lumbar disc herniation (LDH). The aim of the present study was to determine the most suitable therapeutic dose of condoliase. METHODS Patients between 20 and 70 years of age with unilateral leg pain, positive findings on the straight leg raise test, and LDH were recruited. All eligible patients were randomly assigned to receive condoliase (1.25, 2.5, or 5 U) or placebo. The primary end point was a change in the worst leg pain from preadministration (baseline) to week 13.
