Current concepts review: intra-articular fractures of the calcaneus.

1) Intra-articular fractures of the calcaneus are associated with severe long-term consequences for function and pain. The condition of the soft tissues is of paramount importance when determining the method of treatment, the timing of surgery, and the post-injury rehabilitation.2) Intra-articular fractures are difficult to fully evaluate with plain radiographs. Computed tomography may assist in assessing the fracture pattern and planning for surgery. The likelihood of a good to excellent outcome is increased when an anatomic reduction is obtained.3) The outcome after operative management is di

Chondrocyte viability after intra-articular calcaneal fractures in humans.

BACKGROUND: Chondral damage from the impact of injury may contribute to the high incidence of post-traumatic arthritis after calcaneal fractures, but this has yet to be proven. We sought to study the effect of intra-articular calcaneal fractures on chondrocyte viability and to correlate these effects with injury severity, time from injury to surgery, and patient age and co-morbidities. METHODS: Irreducible osteochondral fragments from 12 patients undergoing operative treatment for intra-articular calcaneal fractures were analyzed. Control cartilage was obtained from four tissue donors who die

The arthroscopic management of intra-articular distal radius fractures.

Intra-operative arthroscopy and fluoroscopy provide improved visualisation and guide the restoration of intra-articular distal radial fractures while minimising the operative dissection required for their stabilisation. Radial styloid fractures, distal radial fractures with dorsal, palmar or combined ulnar-sided "die punch" fragments, palmar and dorsal Barton's fractures, and various three- and four-part intra-articular fractures without significant bone loss or defect are especially suited for this technique. The experienced arthroscopist may wish to apply the technique to more severely commi

Subtalar arthrodesis for complications of intra-articular calcaneal fractures.

Eighty six subtalar arthrodeses performed between 1985 and 1996 for complications associated with intra-articular calcaneal fractures were retrospectively evaluated. Patients were divided into three Groups: (I) 59 patients with calcaneal malunions (II) 13 patients with failed open reduction and internal fixation, and (III) eight patients undergoing open reductions and primary fusion for highly comminuted fractures. In each scenario, internal fixation was achieved with cancellous lag screws. Bone graft material consisted of either autogenous iliac crest graft, local graft obtained from the late

Fracture Lines and Patterns in Intra-articular Distal Femur Fractures.

INTRODUCTION: Distal femur fractures are complex injuries that often present with multiple fragments, posing notable challenges to fixation. This study aimed to (1) use preoperative CT scans to graphically display fracture lines in intra-articular distal femur fractures and (2) identify common fracture patterns in these injuries. METHODS: All skeletally mature patients that underwent surgical fixation of Orthopaedic Trauma Association type 33C distal femur fractures between 2012 and 2022 were identified across two level 1 trauma centers (n = 63). Preoperative axial, sagittal, and coronal comp

Pilon fractures of the wrist. Displaced intra-articular fractures of the distal radius.

22 patients who sustained high energy wrist injuries are reported. These complex injuries resulted in articular disruption of the distal radius. Associated injuries included scapho-lunate dissociation (18%), central die-punch injuries (14%), ulnar fractures (41%) and diastasis at the distal radioulnar joint (23%). Nine fractures (41%) were open and almost a third of patients had other skeletal injuries. All patients were treated by external fixation and reviewed after a mean follow-up of 2 1/2 years. There were no excellent results and only ten good ones (45%). The mean functional impairment w

Scaffold Fixation of Multi-Fragmented Intra-Articular Distal Radius Fractures.

OBJECTIVE: Volar locking plates (VLPs) are commonly used to treat displaced intra-articular distal radius fractures (DRFs), but multi-fragmented fractures may require additional dorsal fixation. This study assessed the outcomes of a technique combining VLP and a dorsal bone clamp, described here as "scaffold fixation," and evaluated whether dorsal ulnar corner (DUC) fixation is essential for radiocarpal alignment. METHODS: In this retrospective cross-sectional study of the ICUC database, 87 patients with DRF with preoperative CT-identified DUC fragments were included. The range of motion was

Surgical outcomes after reoperation of intra-articular proximal ulna fractures.

BACKGROUND: Literature on outcomes after reoperation for intra-articular proximal ulna fractures is lacking, even though reoperation rates for these fractures are reportedly one of the highest of any anatomic site, ranging from 22%-89%. This study aims to evaluate range of motion (ROM) and complication rates after reoperation for these fractures. METHODS: In this retrospective single institution cohort study, we identified 134 patients with intra-articular, comminuted fractures of the proximal ulna initially treated with open reduction internal fixation between January 2015 and March 2022. Cartilage reconstruction using Chondrofiller in intra-articular distal radius fractures.

INTRODUCTION: Intra-articular distal radius fractures (DRF) frequently result in post-traumatic cartilage defects, which can lead to early osteoarthritis, particularly when residual step-offs or gaps ≥ 2 mm remain after fracture reduction. While cartilage repair techniques are well established in large joints, options for small joints like the wrist remain limited. Chondrofiller liquid, a cell-free collagen type I hydrogel, has demonstrated regenerative potential in larger joints; however, its application in the wrist has not yet been studied. METHODS: In this prospective clinical study, 59 p

Arthroscopic Scaffold Fixation Technique for Intra-articular Distal Radius Fractures.

Arthroscopic-assisted reduction and internal fixation of displaced intra-articular fractures of the distal radius is a useful adjunct in the management of these complex injuries. This paper describes an arthroscopic technique to reduce and internally fix the anterior endplate articular surface with subchondral wires to create a two-part fracture. The wire configuration resembles a scaffold so that the distal fracture component can then be indirectly reduced using a volar locking plate to correct the anteroposterior and radioulnar inclination of the articular surface of the distal fragment.

Intra-articular fragment mapping in associated both-column acetabular fractures.

INTRODUCTION: Associated both-column acetabular fractures (ABC-AF) are complex. A detailed description of the morphology of the articular surface of the acetabulum is not reported. This study was designed to describe the morphology of the intra-articular fragments using a fracture mapping technique to provide reference in more appropriate approach selection in surgical treatment of the fractures. MATERIALS AND METHODS: Three-dimensional reconstruction CT images from 100 cases of ABC-AFs were retrospectively analyzed using Mimics and 3-matic software. The fractured as well as the reduced three

Outcomes of posterior malleolar fractures with intra-articular impacted fragment.

INTRODUCTION: An intra-articular impacted fragment (IAIF) could lead to articular incongruity and malreduction in ankle fractures with posterior malleolar fractures (PMFs). No studies have been conducted to determine whether the presence of IAIF affects the outcome of ankle fractures with PMF. The aim of our retrospective study was to evaluate the effect of IAIF on postoperative outcomes in PMF and analyze the relationship between area of IAIF and outcomes. MATERIALS AND METHODS: We conducted a retrospective study of patients with a posterior malleolar fractures between June 2012 and January

[Complete intra-articular distal radius fractures in young active patients].

Management of complex intra-articular fractures of the distal radius in young active patients must aim to restore normal anatomy. For optimal planning of the surgical procedure, a thorough analysis of the fracture with appropriate radiological exams and diagnosis of frequently associated lesions are necessary. Surgical management involves use of various types of internal or external fixation. Routine use of standardized analysis tools for these fractures along with prospective follow-up should allow us to define guidelines for each situation in the near future.

Does joint architecture influence the nature of intra-articular fractures?

INTRODUCTION: The architecture of joints has potentially the greatest influence on the nature of intra-articular fractures. We analysed a large number of intra-articular fractures with two aims: (1) to determine if the pattern of injuries observed supports our conjecture that the local skeletal architecture is an important factor and (2) to investigate whether associated dislocations further affect the fracture pattern. METHODS: A retrospective study of intra-articular fractures over a 3.5-year period; 1003 joints met inclusion criteria and were analysed. Three independent investigators deter

Arthroscopically detected intra-articular lesions associated with acute ankle fractures.

BACKGROUND: Anatomic surgical realignment of ankle fractures may still be associated with poor clinical outcomes, possibly as a result of occult intra-articular injury. The aim of this study was to determine if the severity of an acute ankle fracture is correlated with an increased number of arthroscopically detected intra-articular chondral lesions. METHODS: We conducted a retrospective review of the medical charts on 283 ankle fractures that had been treated with open reduction and internal fixation and for which ankle arthroscopy had been routinely performed. The severity of the ankle frac

Intra-articular Radial Head Fractures: A Deceptive Type of Injury.

BACKGROUND: Isolated intra-articular radial head (IARH) fractures in skeletally immature patients represent a rare injury. Despite their initial benign radiologic appearance, these fractures are at risk for progressive radial head subluxation and may end with degenerative irreversible changes of the radiocapitellar joint. The aim of this study is to highlight the seriousness of these injuries and the importance of early diagnosis and a proper follow-up to achieve optimal outcomes. METHODS: We retrospectively reviewed 6 patients with IARH fractures treated at our institution between 2011 and 2

Lateral bone flap approach for displaced intra-articular calcaneus fractures.

BACKGROUND: The traditional extended lateral approach for calcaneus fractures can provide sufficient exposure for reduction, but complications are frequent. The minimally invasive approach does limited damage to the soft tissue and its complication rate is low, but provides limited surgical exposure for complicated fracture. Thus, an approach that could provide wide exposure with less soft tissue injury is important. METHODS: The lateral bone flap approach, we proposed, involved the same incision as the extended lateral approach. After incision, osteotomy was performed around the lateral bulg
