Neurophysiologic intraoperative monitoring during surgery for tethered cord syndrome.

Tethered cord syndrome (TCS) occurs when the distal spinal cord is adherent to inelastic tissue. This results in sensorimotor deficits in the lower extremities, bowel and bladder dysfunction, and musculoskeletal deformities. Tethered cord syndrome is often found in childhood, but may be first noticed in adults as well. The symptoms are usually progressive unless halted by surgical correction of the spinal cord tethering. Surgery for TCS can be complicated by inadvertent injury to nerves that are either embedded in the tether or in close proximity to it. In an attempt to reduce this iatrogenic

Intraoperative motor evoked potential monitoring - a position statement by the American Society of Neurophysiological Monitoring.

The following intraoperative MEP recommendations can be made on the basis of current evidence and expert opinion: (1) Acquisition and interpretation should be done by qualified personnel. (2) The methods are sufficiently safe using appropriate precautions. (3) MEPs are an established practice option for cortical and subcortical mapping and for monitoring during surgeries risking motor injury in the brain, brainstem, spinal cord or facial nerve. (4) Intravenous anesthesia usually consisting of propofol and opioid is optimal for muscle MEPs. (5) Interpretation should consider limitations and con

ION-Sim: A Novel Open-Source Simulation Framework for Intraoperative Neurophysiological Monitoring.

The educational pathway for expertise in intraoperative neurophysiological monitoring (IONM) is complex and lengthy, requiring a solid foundation in neuroscience, neurophysiology, and neuroanatomy. It also demands direct familiarity with a broad range of neurosurgical scenarios, including supratentorial, infratentorial, and spinal procedures, gained through exposure to at least ten distinct surgical approaches. Intraoperative neurophysiology must be tailored to each patient's preoperative assessments. It relies on a variety of methods to collect, analyze, and report neurophysiological signals

A multichannel electromyography dataset for continuous intraoperative neurophysiological monitoring of cranial nerve.

Continuous Intraoperative Neurophysiologic Monitoring (cIONM) is a widely used technology to improve surgical outcomes and prevent cranial nerve injury during skull base surgery. Monitoring of free-running electromyogram (EMG) plays an important role in cIONM, which can be used to identify different discharge patterns, alert the surgeon to potential nerve damage promptly, etc. In this dataset, we collected clinical multichannel EMG signals from 11 independent patients' data using a Neuromaster G1 MEE-2000 system (Nihon Kohden, Inc., Tokyo, Japan). Through innovative classification methods, the

Early results of intraoperative neurophysiological monitoring in surgical treatment of hemifacial spasm.

BACKGROUND: Neurophysiological monitoring in surgery for hemifacial spasm has been used since the 1990s, when Moller et al. demonstrated the effectiveness of intraoperative assessment of lateral spread response (LSR) regarding postoperative outcomes. Currently, there are conflicting data on effectiveness and feasibility of this technique. Widespread hemifacial spasm determines the relevance of neurophysiological monitoring in surgical treatment of these patients. OBJECTIVE: To evaluate the effectiveness of various methods of intraoperative neurophysiological monitoring in surgical treatment o

Microvascular decompression for glossopharyngeal neuralgia using intraoperative neurophysiological monitoring: Technical case report.

BACKGROUND: Glossopharyngeal neuralgia (GN) is a rare functional disorder representing around 1% of cases of trigeminal neuralgia. Lancinating throat and ear pain while swallowing are the typical manifestations, and are initially treated using anticonvulsants such as carbamazepine. Medically refractory GN is treated surgically. Microvascular decompression (MVD) is reportedly effective against GN, superseding rhizotomy and tractotomy. METHODS: We encountered three patients with medically refractory GN who underwent MVD using intraoperative neurophysiological monitoring (IONM). The offending ve

Impact of brain shift on intraoperative neurophysiological monitoring with cortical strip electrodes.

BACKGROUND: Intraoperative neurophysiological monitoring has become the standard procedure for locating eloquent regions of the brain. Such continuous electrical stimulation of motor pathways is usually applied by means of flat silicon-embedded electrodes placed directly on the motor cortex. However, shifting of the silicon strip on the cortical surface as well as electrode displacement due to brain shift underneath the electrode can lead to inaccurate recordings not directly caused by intraoperative impairment of the motor cortex or the motor pathways. METHOD: This prospective study was cond

The Role of Intraoperative Neurophysiologic Monitoring in Asleep AVM Resection: Indication or Redundancy?

PURPOSE: The surgical resection of cerebral arteriovenous malformations (AVMs) presents a significant neurosurgical challenge, particularly because of the need to achieve complete obliteration of the nidus while preserving neurologic function. AVMs located within or adjacent to eloquent regions such as the motor or language cortex carry a high risk of postoperative deficits. To mitigate these risks, a variety of intraoperative tools have been developed to enhance surgical safety and decision making. METHODS: Cortical and subcortical mapping is a dynamic technique that helps identify and prese

Progress in Intraoperative Neurophysiological Monitoring for the Surgical Treatment of Thoracic Spinal Stenosis.

Thoracic spinal stenosis (TSS) is a group of clinical syndromes caused by thoracic spinal cord compression, which always results in severe clinical complications. The incidence of TSS is relatively low compared with lumbar spinal stenosis, while the incidence of spinal cord injury during thoracic decompression is relatively high. The reported incidence of neurological deficits after thoracic decompression reached 13.9%. Intraoperative neurophysiological monitoring (IONM) can timely provide information regarding the function status of the spinal cord, and help surgeons with appropriate performa
