Trends in Involuntary Psychiatric Hospitalization in British Columbia: Descriptive Analysis of Population-Based Linked Administrative Data from 2008 to 2018.

INTRODUCTION: Involuntary psychiatric hospitalization occurs when someone with a serious mental disorder requires treatment without their consent. Trends vary globally, and currently, there is limited data on involuntary hospitalization in Canada. We examine involuntary hospitalization trends in British Columbia, Canada, and describe the social and clinical characteristics of people ages 15 and older who were involuntarily hospitalized between 2008/2009 and 2017/2018. METHOD: We used population-based linked administrative data to examine and compare trends in involuntary and voluntary hospita

Comorbid opioid use is undertreated among forensic patients with schizophrenia.

BACKGROUND: Substance use disorders are associated with poorer clinical outcomes in patients with schizophrenia. There is no specific treatment for amphetamine or cannabis use disorder, but methadone and buprenorphine are used as replacement therapy in the treatment of opioid dependence. Our aim was to study whether patients with schizophrenia have received opioid replacement therapy for their opioid use disorder. METHODS: The study sample consisted of 148 individuals diagnosed with schizophrenia who were in involuntary psychiatric treatment as forensic patients in Finland in 2012. The propor

Risk factors for involuntary admission in a Romanian patient sample.

BACKGROUND: Involuntary admission is widely used in psychiatry, usually requiring that the patient present an imminent danger to himself or others. Previous studies have established several predictors for involuntary admission, but they have been almost exclusively conducted in Western European or North American countries. By contrast, data on this topic from Eastern European countries is virtually absent. Historically, involuntary admission has been often used as a tool for political repression in Romania before the fall of the communist regime. While there have been significant changes in th

Does the gender of emergency physicians have an impact on the prehospital care of psychiatric emergencies? a retrospective cohort analysis.

BACKGROUND: Psychiatric emergencies pose a special challenge for emergency physicians. It is known from other areas of medicine that the influence of a doctor's gender can have an impact on the type of treatment and quality of patient care. However, this has not yet been investigated in the context of prehospital care in psychiatric emergencies. OBJECTIVES: To identify whether the gender of the prehospital emergency physicians has an influence on the "on-scene" time, treatment strategy and on the potential escalation of interventions for patients with a psychiatric diagnosis. METHODS: A retr

Coerced hospital admission and symptom change--a prospective observational multi-centre study.

INTRODUCTION: Coerced admission to psychiatric hospitals, defined by legal status or patient's subjective experience, is common. Evidence on clinical outcomes however is limited. This study aimed to assess symptom change over a three month period following coerced admission and identify patient characteristics associated with outcomes. METHOD: At study sites in 11 European countries consecutive legally involuntary patients and patients with a legally voluntary admission who however felt coerced, were recruited and assessed by independent researchers within the first week after admission. Involuntary and voluntary electroconvulsive therapy: A case-control study.

BACKGROUND: It is not known whether results of clinical research in ECT can be used to guide treatment decisions for those having involuntary ECT, who are not represented in trial populations. OBJECTIVE: We aimed to compare courses of involuntary ECT with matched voluntary ECT courses in terms of clinical and demographic factors, treatment requirements, and outcomes. METHOD: We performed a retrospective case-control study examining a five-year sample of involuntary ECT courses and an age-, gender- and time-matched voluntary ECT control sample. RESULTS: We examined 48 involuntary and 96 cont

Refractory dry eye disease associated with Meige's syndrome induced by long-term use of an atypical antipsychotic.

BACKGROUND: We report a case of Meige's syndrome induced by an atypical antipsychotic (blonanserin) that presented with refractory dry eye disease. CASE PRESENTATION: A 37-year-old woman with a 6-month history of foreign body sensation in the eyes and difficulty in opening her eyes was treated at a local clinic for dry eye disease. Despite this treatment, her symptoms did not improve and she was transferred to our attention. Our assessment revealed involuntary movements of her eyelids accompanied by repetitive pursing of her lips. She had been undergoing treatment with blonanserin for 5 years

[Abdominal dystonia in a patient with schizophrenia: a case report].

The case of 31-year-old woman suffering from schizophrenia with movement disorder is described. The patient had a 7-year history of schizophrenia. In course of the psychiatric treatment the patient presented dystonic movements within abdominal muscles. The dystonic movements were of mixed character, including voluntary and involuntary ones what might have suggested their psychogenic origin. Subsequent to the exclusion of the neurological origin of the movement disorder and poor response to antipsychotic treatment, clozapine was introduced resulting in full remission of positive symptoms and fu

Involuntary Psychotropic Treatment in the Correctional System: Revisiting the Legal Standards.

Correctional facilities are obligated to provide psychotropic medications, as part of standard psychiatric care, to inmates with serious mental illness. The right to refuse such medication treatment has become one of the most important and contested areas of legal regulation of correctional mental health. This article will focus on the three cases that have come before the U.S. Supreme Court thus far, as well as their implications for future medicolegal directions in pursuing involuntary treatment.

Insights into involuntary hospital admission procedures for psychiatric patients: A 3-year retrospective analysis of police records.

The procedure of involuntary hospitalization has been an ongoing subject of study. Its implementation requires the systematic co-ordination between the Justice and Health Care systems around the globe. In the case of Greece, the procedure under discussion is regulated by Law 2071/1992, which designates the Police as the agent that links the aforementioned systems together. The present study aims to shed light upon the procedure of involuntary hospitalizations, regarding the preparatory stage and the Police involvement up to the individuals' admission to the on-call hospital for a mental health

Clinical and Ethical Dilemmas in the Involuntary Treatment of Anorexia Nervosa.

LEARNING OBJECTIVES: After completing this activity, practitioners will be better able to:• Discuss the growing body literature emphasizing moderation and harm-reduction in patients with severe and enduring anorexia nervosa (SE-AN)• Outline and discuss the legal, ethical, and medical challenges inpatient providers face when treating patients with SE-AN. ABSTRACT: Patients with severe and enduring anorexia nervosa (SE-AN) present numerous clinical and ethical challenges for the hospital psychiatrist. Patients typically come to the hospital in a state of severe medical compromise. Common diffic

Supreme court of Canada's "Beautiful Mind" case.

The Supreme Court of Canada's (SCC) first case involving capacity and the refusal of involuntary psychiatric treatment involved a self described "professor" who had been referred to as "Canada's Beautiful Mind". He had been found not criminally responsible on account of mental disorder for uttering death threats. While considered incapable of making a treatment decision by psychiatrists and a review board, three levels of court, including the SCC, found him to be capable. "Professor" Starson therefore continued to refuse treatment for his psychosis and spent over seven years detained because h

[Huntington chorea. A clinically relevant differential diagnosis in geriatric psychiatry patients?].

We report on seven geropsychiatric patients, aged 62 to 86, who had received either in- or outpatient psychiatric treatment for different disorders before being diagnosed with Huntington's disease (HD) by molecular biological methods. At the time of diagnosis, these patients presented only with mild involuntary movements in addition to other, nonspecific psychiatric symptoms such as depressive, paranoid, or dementia symptoms. In six of the seven cases, the HD symptoms had previously been misdiagnosed as tardive dyskinesia because these patients had been treated with neuroleptics in the past. F

Familial occurrence of tardive dyskinesia.

Tardive dyskinesia remains a poorly understood consequence of neuroleptic treatment for psychiatric illness. To date, there have been no reports in the literature regarding the presence of dyskinetic symptomatology in first degree relatives or other family members. The following two cases reveal the development of abnormal involuntary movement in brothers with similar mental illness and treatment histories, and suggest the importance of examining neuroleptically treated relatives of an affected individual for dyskinesia and may suggest a genetic factor in vulnerability.

Do outcomes of psychiatric hospital treatment differ for migrants and non-migrants?

PURPOSE: Providing effective treatment for immigrants is an increasing challenge for mental health services across Europe. Yet, little is known as to whether current practice is associated with different outcomes in migrant and non-migrant patients. We compared outcomes of inpatient psychiatric treatment for migrants and non-migrants in a sample from five European countries. METHODS: Patients with psychotic disorders, affective disorders or anxiety/somatisation disorders admitted to routine psychiatric inpatient treatment were assessed in hospitals in Belgium, Germany, Italy, Poland and the U

Recurrent posterior shoulder instability starting in childhood and adolescence.

AIMS: We aimed to address the question on whether there is a place for shoulder stabilization surgery in patients who had voluntary posterior instability starting in childhood and adolescence, and later becoming involuntary and uncontrollable. METHODS: Consecutive patients who had an operation for recurrent posterior instability before the age of 18 years were studied retrospectively. All patients had failed conservative treatment for at least six months prior to surgery; and no patients had psychiatric disorders. Two groups were identified and compared: voluntary posterior instability starti

Anti-NMDA Receptor Encephalitis Misdiagnosed As Generalized Anxiety Disorder: A Case Report.

The autoimmune encephalitis is a rare group of neurological disorders mediated by immune mechanisms. Neuropsychiatric symptoms often occur in the early stages of the disease, so many patients seek treatment in psychiatry for the first time. If psychiatrists lack understanding and vigilance of the disease, it is very easy to be misdiagnosed as various primary psychiatric diseases, thus delaying diagnosis and treatment. Here, we report an older woman with autoimmune encephalitis. In the early stage, the patient was misdiagnosed as having generalized anxiety disorder because of obvious psychiatri

Legal guardianship in drug refusal: an illusory solution.

The use of legal guardianship appears to offer a ready resolution to the complex forensic issue of involuntary medication for psychiatric inpatients. The authors, with the aid of clinical examples, show that the gap between theory and practice may render this solution illusory and illustrate the profound complications for treatment introduced by the guardianship process itself. After examining the complexities of this problem, they consider alternative methods for preserving the patient's rights both to receive and to refuse medication.

Characterization of Psychiatric Inpatients: The Role of Gender Differences in Clinical and Pharmacological Patterns.

BACKGROUND: Severe mental disorders that require hospitalization are disabling conditions that contribute to the burden of mental diseases. They pose increased clinical challenges and highlight the need to thoroughly explore variables emerging from daily clinical practice. In this study, we assessed to what extent gender differences may characterize a large population of psychiatric inpatients. METHODS: We conducted a cross-sectional study in 2 Italian teaching medical centers, which included 2358 patients who were consecutively admitted to the psychiatric emergency units. We explored and cha

Suicidality and Hostility following Involuntary Hospital Treatment.

BACKGROUND: Psychiatric patients showing risk to themselves or others can be involuntarily hospitalised. No data is available on whether following hospitalisation there is a reduction in psychopathological indicators of risk such as suicidality and hostility. This study aimed to assess changes in suicidality and hostility levels following involuntary admission and their patient-level predictors. METHODS: A pooled analysis of studies on involuntary treatment, including 11 countries and 2790 patients was carried out. Suicidality and hostility were measured by the Brief Psychiatric Rating Scale.
