Predictors of outcome in the ISCHEMIA-CKD trial: Anatomy versus ischemia.

BACKGROUND: The ISCHEMIA-CKD (International Study of Comparative Health Effectiveness with Medical and Invasive Approaches-Chronic Kidney Disease) trial found no advantage to an invasive strategy compared to conservative management in reducing all-cause death or myocardial infarction (D/MI). However, the prognostic influence of angiographic coronary artery disease (CAD) burden and ischemia severity remains unknown in this population. We compared the relative impact of CAD extent and severity of myocardial ischemia on D/MI in patients with advanced chronic kidney disease (CKD). METHODS: Partic

[ACUTE MESENTERIC ISCHEMIA].

A 61-year-old man with a complex cardiovascular history and chronic kidney disease was hospitalized with an anterior STEMI and found on catheterization to have severe occlusions in multiple coronary arteries. He underwent PCI followed by urgent coronary artery bypass surgery. On the day after the surgery, he developed acute abdominal pain, and a CTA revealed thrombosis of the superior mesenteric artery (SMA) with ischemia of the small intestine - a condition that was treated surgically with bowel resection and thrombectomy.

Acute limb ischemia.

Acute limb ischemia (ALI) is an emergency situation requiring rapid diagnosis and treatment. Although the traditional treating strategy for ALI includes open surgery, novel endovascular techniques have been introduced during the last decade. Additionally, many new cases of ALI have been reported due to infection by the SARS-CoV-2 virus. The aim of this study was to present an updated overview of characteristics, diagnosis, and current treating strategies of patients with ALI.

[Spinal cord ischemia].

Spinal cord ischemia is a rare disease with a poor prognosis. As with cerebral ischemia, spinal infarction typically presents with sudden symptom onset and variable clinical manifestation (depending on the lesion location). The majority of spinal ischemias occur in the supply territory of the anterior spinal artery, involving the anterior and middle third of the myelon. The most common cause of spinal ischemia is an arterial vessel occlusion, although venous ischemia and hypotensive watershed infarcts may also occur. Magnetic resonance imaging (MRI) represents the modality of choice for diagno

Acute Mesenteric Ischemia.

BACKGROUND: Despite constant improvements in diagnostic as well as interventional and surgical techniques, acute mesenteric ischemia (AMI) remains a life-threatening emergency with high mortality rates. The time to diagnosis of AMI is the most important predictor of patients' outcome; therefore, prompt diagnosis and intervention are essential to reduce mortality in patients with AMI. The present review was performed to analyze potential risk factors and to help find ways to improve the outcome of patients with AMI. SUMMARY: Whereas AMI only applies to approximately 1% of all patients with an

Acute Limb Ischemia.

Acute limb ischemia (ALI) is a rapid decrease in lower limb blood flow due to acute occlusion of peripheral artery or bypass graft, and in ALI not only limbs but also life prognosis will be poor unless quick and appropriate treatment is given. The etiology is broadly divided into embolism and thrombosis with various comorbidities. The symptoms of ALI are abrupt with pain, numbness, and coldness of lower limb, and paresthesia, contracture, and irreversible purpura will appear with the exacerbation of ischemia. Severity and treatment strategy should be determined based on physical findings and i

[Acute mesenteric ischemia].

Acute mesenteric ischemia is a real life-threatening emergency. Mode of presentation is severe abdominal pain with sudden onset. The etiology is most of the time embolus coming from the heart or thrombosis of atherosclerotic vessel wall concerning the superior mesenteric artery. The best investigation is computed tomography with intravenous contrast media injection. Treatment consists of revascularization by open surgery or endovascular approach. Bowel resection may be necessary, as well as delayed second look surgery. This paper aims to review etiologies and therapeutics options for acute mes

[Chronic Mesenteric Ischemia].

Chronic mesenteric ischemia (CMI) is defined by stenoses or occlusions of the celiac artery, superior or inferior mesenteric artery. While it is mostly cause by atherosclerotic disease, less frequent cause in particular in younger patients are fibrowmuscular dysplasia or vasculitis. Risk factors include smoking, hypertension, dyslipidemia, higher age, and female sex. Symptomatic CMI only accounts for less than 5 % of intestinal ischemic events. The prevalence of asymptomatic CMI is unknown but may be as high as 15 % and is more frequent in peripheral artery disease (27 %) and abdominal aortic

Acute mesenteric ischemia.

BACKGROUND/AIMS: Acute mesenteric ischemia (AMI) is a serious disease in old age with low incidence but with a very high mortality rate (60-70%). The etiology is either primary (embolism or thrombosis of mesenteric arteries or veins, non-occlusive mesenteric ischemia) or secondary (mechanical obstruction such as intestinal volvulus, intussusception, tumor-caused compression). Independent of the origin of the illness, the clinical-pathological picture is the same: intestinal ischemia with subsequent necrosis. The aim of this study was to ascertain which underlying conditions lead to increased p

Critical Limb Ischemia.

Patients with critical limb ischemia (CLI) present with ischemic rest pain or with tissue loss. Tissue loss, in its mildest presentation, includes focal ischemic ulceration or nonhealing wounds. Gangrene is the manifestation of severe chronic ischemia. Without revascularization, patients with CLI are at risk for limb loss and for potentially fatal complications from the progression of gangrene and the development of sepsis. The best patient outcomes are achieved when the diagnosis and appropriate treatment for CLI are not delayed. Simple noninvasive tests, such as measurement of ankle-to-brach

Acute mesenteric ischemia.

Acute mesenteric ischemia (AMI) remains a complex and difficult clinical problem. Such acute ischemia is usually either embolic or thrombotic in nature, but other etiologies exist. The diagnosis of AMI is difficult to establish and often delayed, resulting in irreversible bowel injury that requires intestinal resection. Mortality for AMI remains high, and patients requiring extensive gut resection are unlikely to survive. Patients surviving intestinal resection may develop short gut syndrome. The prognosis dramatically improves if revascularization can be achieved prior to intestinal infarctio

Acute limb ischemia.

Although there is little information on the incidence of acute limb ischemia (ALI) in the general population, it is estimated to be 14 per 100,000 and to compose 10% to 16% of the vascular workload. Also, as surgical intervention has become an option for ALI, the numbers actually referred appear to be increasing. The two main causes of ALI are either an embolism or a thrombosis, and differentiation based on history and clinical examination alone may be clinically impossible in 10% to 15% of cases. However, with the advent of thrombolysis, the distinction between emboli and thrombotic occlusion

[Nonobstructive intestinal ischemia].

Acute or chronic intestinal ischaemia can be the consequence of either intrinsic vascular disease, systemic disease, drugs or surgical procedures. In one quarter of the patients with intestinal ischemia, no major vascular obstructions can be detected. Very rarely, the cause of ischaemia is splanchnic vasoconstriction due to cardiac arrhythmias or sepsis. The bowel becomes ischaemic as a result of underperfusion. The clinical picture resembles the one of occlusive intestinal ischaemia. From the pathological standpoint, the ischaemia is more intense on the antimesenteric border of the bowel and

Acute mesenteric ischemia.

The term acute mesenteric ischemia (AMI) is applied to a wide spectrum of bowel injury within the distribution of the superior mesenteric vessels, ranging from reversible alterations in bowel function to transmural necrosis of the bowel wall. Intensivists not only are called upon to manage this catastrophic disease but also may be faced with AMI as a consequence of other illnesses that they treat.

[Chronic mesenteric ischemia].

Thirty-four patients (twelve men, 22 women, mean age 53[16-71] years) with chronic mesenteric ischaemia were operated upon between 1979 and 1992. The most frequent symptom was loss of weight (50%) and postprandial pain (44%). The mean interval between onset of symptoms and diagnosis was 35 months. Angiography revealed disease of the coeliac trunk (CT) or the superior mesenteric artery (SMA) in 16 patients, of only the SMA in ten, and of only the CT in eight. Revascularisation was obtained with an autologous vein graft in 21 patients (on the TC in 12, AMS in nine), while transaortic endarterect

Chronic visceral ischemia.

Visceral ischemic syndromes constitute a spectrum of diseases that may, if not detected and treated, culminate in fatal intestinal gangrene. Symptomatic chronic visceral ischemia may run a prolonged course before diagnosis and effective surgical revascularization is employed. The internist or general practitioner plays an important role in the management of all patients with chronic visceral ischemia, usually being the first physician to evaluate the patient and often being able to direct the efforts to diagnose the problem effectively. An awareness of the disease and a high index of suspicion

Acute limb ischemia.

Acute ischemia of the extremity may be due to arterial occlusion from spontaneous thrombosis, embolus, arterial bypass graft thrombosis, trauma, or spasm. The presence of occlusion or stenosis can be determined noninvasively with the use of duplex Doppler ultrasonography. Most patients will require arteriography prior to thrombolytic or surgical therapy. New techniques, such as percutaneous aspiration thrombolectomy, expand the role of radiologic percutaneous therapy of the acutely ischemic limb. Prompt diagnosis and therapy are required to avoid limb loss or systemic metabolic complications f

Chronic labyrinthine ischemia.

The purpose of the present study was to describe the audiological and vestibular deficit in patients presumed to suffer from chronic impairment of the blood supply to the labyrinth. Thirty-nine subjects affected by various pathologies often impairing systemic blood circulation, which also presented labyrinthine damage of unclear origin, were evaluated for a possible relationship between these two conditions. At the time of this study 80 per cent of the subjects showed also a certain degree of increased blood viscosity. The most common finding in the group was slow and progressive bilateral sen
