The surgical management of an emerging clinical entity, namely disabling transient cerebral ischemic attacks, is described. A series of 19 patients treated in a 2-year period (12 with anterior circulation dysfunction and seven with posterior insufficiency) met the following criteria: 1) stereotypical recurrent episodes of transient neurological dysfunction related to the anterior or posterior circulation distribution; 2) failure of maximum medical therapy to control the transient neurological dysfunction; 3) four-vessel cerebral angiography demonstrating an isolated vascular territory correspo

INTRODUCTION: Antiplatelet therapy is key to prevent recurrences in patients with an acute or prior non-cardioembolic stroke or transient ischemic attack (TIA). The narrow balance between the risks of ischemic recurrence and major bleeding is a relevant clinical dilemma in this population.
AREAS COVERED: This review covers the current evidence on antiplatelet therapy for patients with non-cardioembolic stroke or TIA. Randomized controlled trials of antithrombotic strategies for patients with these conditions were searched in Pubmed/Medline from 1970 to 2022.
EXPERT OPINION: Numerous randomiz

OBJECTIVE: We aimed to estimate the status of risk factor control after ischemic stroke or transient ischemic attack (IS/TIA), and the influence on recurrent stroke in rural communities of northeastern China.
METHODS: This population-based, prospective cohort study enrolled adults aged ≥35 years residing in rural northeastern China. We conducted cardiovascular health examinations in 2012-2015 and followed up in 2018 to record any cardiovascular event. Control of risk factors after IS/TIA was determined through a baseline survey. The Cox proportional hazard model was used to evaluate the relat

BACKGROUND: The majority of TIAs last from 2 to 15 minutes, although some may be of long duration.
OBJECTIVE: We examined factors related to the duration of TIAs to identify the relationship to clinical characteristics.
METHODS: We performed brain imaging studies as well as angiographic and cardiac examinations in 81 consecutive patients (64 men and 17 women, age 65.8+/-9.9 years) with carotid TIAs. We evaluated risk factors (hypertension, diabetes mellitus, hyperlipidemia, alcohol consumption, and smoking), potential cardiac sources of emboli, and arterial stenosis > or =50% in diameter in

Sixty (29%) of 205 consecutive patients with transient ischemic attacks registered in a hospital stroke data base had headache within 72 hours of onset. Headache was significantly more common in nonsmokers (odds ratio = 2.8; 95% confidence interval = 6.7 to 1.2). Headache was infrequent in patients with amaurosis fugax, and was not significantly associated with any other particular clinical presentation of transient ischemic attack. Headache was more common in vertebrobasilar (33%) than in carotid distribution (24%) episodes, and was not rare in transient ischemic attacks presenting as lacunar

The evaluation of headache in patients with transient ischemic attacks (TIA) has various sources of difficulty, the definition of TIA being the most relevant. The classical definition needs to be supplemented with a normal CT scan if a misleading diagnostic statement is to be avoided. The clinical features of 90 patients suffering from TIA and who had contrast-enhanced CT scans are reported. Headache occurred in 30% of the patients. Headache prevailed in patients with vertebrobasilar TIA compared with carotid TIA (p less than 0.05). Headache prevailed as a preceding (24 h) and/or concomitant s

Several options exist for treating transient ischemic attacks (TIAs), although none can eliminate the risk of cerebral infarction. For carotid TIA, carotid endarterectomy is the preferred therapy in patients with an angiographically demonstrated sclerotic lesion, whereas extracranial-intracranial bypass may be beneficial in patients with carotid artery occlusion or intracranial arterial stenosis. For patients who are not surgical candidates, oral anticoagulants may be helpful, but hemorrhagic complications can occur. Control of risk factors (especially hypertension) and associated medical cond

Thirty-one patients with cerebral transient ischemic, attacks and ipsilateral internal carotid artery occlusion without contralateral internal carotid artery occlusion or stenosis were treated with a surgical anastomosis between a superficial temporal artery and a cortical branch of the middle cerebral artery of the symptomatic hemisphere. The anastomosis was successful in 28 patients. Recurrent transient ischemic attacks were abolished in 23 patients and reduced in three. Two patients, one with a patent anastomosis, had strokes during the follow-up period. Of seven patients who refused the op

BACKGROUND AND PURPOSE: Transient visual symptoms (TVS) are common complaints. They can be related to transient ischemic attacks, but the nature of the symptoms often remains uncertain, and data on prognosis are scarce. We studied the prevalence, presentation, and effect of different types of TVS, paying particular attention to the association with high-risk pathology of embolism.
METHODS: A total of 2398 patients with suspected transient ischemic attack admitted to the SOS-TIA clinic between January 2003 and December 2008 underwent immediate evaluation and treatment.
RESULTS: Eight hundred

A 57-year-old woman had transient right hemiplegia 2 days after physical stress. An electrocardiogram exhibited T-wave inversion with QT prolongation. An echocardiogram revealed akinesia of the entire left ventricle except for the basal region. Cranial computed tomography and transthoracic echocardiography revealed an apical thrombus that disappeared 1 month later. Coronary angiography was normal. After 3 months, left ventricular wall motion had completely normalized.

Background: Carotid endarterectomy (CEA) is a widely accepted treatment to mitigate stroke risk in patients with severe carotid stenosis. The timing of CEA, especially in cases of acute neurological symptoms like crescendo transient ischemic attack (TIA) and stroke-in-evolution, remains contentious. This study evaluates the effectiveness of urgent CEA (UCEA) within 6 hours of crescendo TIA onset compared to elective CEA (ECEA) performed within 2 weeks in preventing recurrent stroke. Patients and methods: This retrospective study analyzed 87 patients with crescendo TIA treated with UCEA and com

KEY CLINICAL MESSAGE: Isolated cardiac sarcoidosis may rarely present with TIA or stroke as an initial clinical manifestation. This case highlights the necessity of a broad differential and a high degree of suspicion for cardiac sarcoidosis in a patient with new neurologic symptoms and evidence of cardiac disease.
ABSTRACT: Cardiac sarcoidosis is a rare disease with a variety of clinical manifestations including heart failure and sudden death. Stroke as the earliest sign of disease has been described in rare cases. We present a case of a 54-year-old female with recurrent transient ischemic at

INTRODUCTION: Early diagnosis and stroke preventive treatment in patients with transient ischemic attack (TIA) are crucial, but hampered by delayed reporting of symptoms. Previous studies on causes of patient delay provided inconsistent results. We aimed to assess determinants of patient delay among patients with symptoms suggestive of TIA.
METHODS: We interviewed participants referred by their general practitioner to an outpatient TIA clinic within 72 h from symptom onset. We determined (i) the exact time from symptom onset to the first contact with a medical service (patient delay); (ii) de

BACKGROUND: Transient ischemic attack (TIA) is characterized by stroke-like neurologic signs and symptoms in the absence of demonstrable structural neuropathology. There is no test for TIA, with classification often reliant on subjective, retrospective report. Functional brain measures such as the electroencephalogram (EEG) may be helpful in objectively detecting and describing the pathophysiology of TIA, but this has not been adequately examined.
METHODS: EEG was obtained from a single electrode over the left frontal lobe during 3-minute resting-state and auditory oddball conditions administ

BACKGROUND: Clinical pathway (CP) of transient ischemic attack (TIA) is an interdisciplinary, comprehensive, standardized management model for medical care of TIA. It aims to standardize the procedure, reduce the cost, and improve the quality of medical care. However, its effect is still unclear.
OBJECTIVE: To evaluate the effect of TIA CP and provide evidence for control of medical care cost, optimize the structure of hospital cost, and make best use of medical resource.
METHODS: 112 patients in the CP group and 179 patients in the nonclinical pathway (NCP) group were included in this nonco

Transient ischemic attack is defined as transient neurologic symptoms without evidence of acute infarction. It is a common and important risk factor for future stroke, but is greatly underreported. Common symptoms are sudden and transient, and include unilateral paresis, speech disturbance, and monocular blindness. Correct and early diagnosis of transient ischemic attack versus mimicking conditions is important because early interventions can significantly reduce risk of future stroke. Nonspecific symptoms and gradual onset are more likely with mimics than with true transient ischemic attacks.

A 68-year-old woman with no prior medical history was diagnosed with probable glutamic acid decarboxylase 65 (GAD65) antibody-associated autoimmune encephalitis, which initially mimicked a transient ischemic attack (TIA) before progressing to super-refractory status epilepticus. She initially experienced a transient episode of right-sided weakness and dysarthria, leading to a presumed vascular diagnosis. Eight days later, she returned with right central facial palsy and conduction aphasia, which rapidly progressed to global aphasia and was followed by fever and seizures, prompting neurological
