Deaths associated with intrapartum asphyxia in Jamaica.

The Jamaican Perinatal Mortality Survey compared all 2069 perinatal deaths occurring during the 12 months between 1 September 1986 and 31 August 1987 with 10,086 survivors born in the 2 months of September and October 1986. The Wigglesworth classification identified 44% of the deaths as attributable to intrapartum asphyxia (IPA), and this grouping was largely confirmed by post-mortem examination where it had been carried out. About half of these babies weighted 2500 g+ and death should have been largely preventable. Comparison of the 813 IPA singleton deaths with 9919 singleton survivors using

A child-to-child programme in rural Jamaica.

A child-to-child programme was carried out in six rural schools in Jamaica. School children in grades 4 and 5 were taught concepts of nutrition, environmental and personal hygiene, and child development. The children were encouraged to take messages home to their parents and improve the care of their younger siblings. Four hundred and twenty-three children and 90 mothers and guardians from four of the project schools were evaluated and compared with 199 children and 47 parents from two control schools. The project children showed higher scores on a test of knowledge in the topics at the end of

Primary tumours of the small intestine in Jamaica.

A review of 60 patients with primary small bowel tumours seen at the University Hospital, Jamaica, during the 15 year period 1971-1985, revealed that adenocarcinoma was the commonest tumour (27%), followed by smooth muscle tumour (23%), and carcinoids (11%). There were 32 malignant and 28 benign tumours. The mean age at presentation was 56 years, with a range of 4 to 85 years. The most common clinical presentation was intestinal obstruction, followed by pain, weight loss, abdominal mass and intussusception. In the majority of patients the diagnosis was not made preoperatively, and 80% with ade

Incidence of skin disease in rural Jamaica.

The incidence of skin disease in people seeking health care in rural Jamaica during the period January to December 1985 has been studied. Results indicate that of 14,179 clinic visits reviewed, 6% involved a primary dermatologic diagnosis. Infectious and parasitic diseases predominated, with scabies, impetigo, and tinea being the most common diagnoses. Of these, scabies accounted for nearly one third of all dermatologic visits. Noninfectious skin disease accounted for less than 10% of dermatologic diagnoses, with unclassified eczema making up the majority. Undiagnosed skin conditions contribut

Cultural factors influencing teenage pregnancy in Jamaica.

A literature review was undertaken to identify key factors associated with teenage pregnancy in Jamaica. Using the PEN-3 cultural model, we first categorised these factors to develop a theoretical taxonomy that can be used to help health intervention planners to understand and address the phenomenon. Next, we examined the validity of this initial taxonomy by comparing it with cultural factors identified in semi-structured focus group discussions with Jamaican teenage mothers. Cultural factors identified in the initial literature review - such as parental interaction and control, the intergener

New Delhi metallo-β-lactamase in Jamaica.

INTRODUCTION: The global dissemination of the New Delhi metallo-beta-lactamase (NDM) gene among certain strains of bacteria has serious implications since the infections caused by such organisms pose a therapeutic challenge. Although the NDM gene has been detected in various parts of the world, this is the first report of its detection in the English-speaking Caribbean. The NDM producing Klebsiella pneumoniae was isolated from an Indian patient who had recently relocated to Jamaica. METHODOLOGY: Identification and susceptibility testing of the K. pneumoniae isolate was performed using the Vit

Balamuthia mandrillaris therapeutic mud bath in Jamaica.

Balamuthia mandrillaris is an emerging cause of encephalitis in humans. The transmission dynamics are poorly understood due to the high fatality rate and the sporadic nature of cases. Seventy-two soil samples were collected from beaches and the banks of lagoons, rivers, ponds, mineral springs and streams from across Jamaica and assayed for the presence of B. mandrillaris. Seventy-nine sites were sampled and the mitochondrial 16S rDNA gene of B. mandrillaris was amplified and sequenced to confirm the presence of the amoeba. One isolate of B. mandrillaris was recovered from soil from mineral spr

CKD Prevalence and Associated Factors in Jamaica.

INTRODUCTION: Jamaica has a high attributable burden of chronic kidney disease (CKD) but no population-based prevalence estimates. We aimed to estimate the prevalence of CKD and explore associated factors. METHODS: A secondary analysis of data from Jamaican residents aged ≥ 15 years from the nationally representative Jamaica Health and Lifestyle Survey-III was performed. CKD was defined as an estimated glomerular filtration rate (eGFR) < 60 ml/min per 1.73 m2, using the CKD Epidemiology Collaboration 2021 or Schwartz-Lyon equations, and/or urine albumin-to-creatinine ratio (UACR) ≥ 30 mg/g. Prevalence of Dirofilaria immitis in dogs in Jamaica.

Dirofilaria immitis is a zoonotic vector-borne parasite that causes heartworm disease most commonly in canines. Although present in the Caribbean, no published work exists from Jamaica. Therefore, the aim of our study was to evaluate the prevalence of D. immitis in Jamaica. From June to December 2022, whole blood samples from 1000 dogs were collected from all parishes and tested using the Bionote Canine Heartworm Antigen Test Kit 2.0. Dogs that were at least 6 months old, privately owned, residing in shelters, working in the security sector, or strays were recruited into the study. Details on

Endocrine disorders in a specialist hospital in Jamaica.

The objective of this study was to define the types of disease seen at the only endocrine clinic in Jamaica. The diagnoses of all patients attending the endocrinology clinic of the University Hospital of the West Indies over a 7-year period were analysed. Of the 1503 patients attending 263 weekly endocrinology clinics, 1251 patients had thyroid-related diseases, of which 684 patients had thyrotoxicosis and 233 patients had hypothyroidism. There were 17 pituitary tumours, eight cases of Cushing's syndrome and four of acromegaly. Cases of Sheehan's syndrome (n=11), hypopituitarism (n=17), hypoad

The development of primary health care in Jamaica.

The development of primary health care in Jamaica is explored, tracing its early roots to the abolition of the slave trade, the collapse of estate-based services after emancipation and the subsequent establishment of the Island Medical Services in 1875. Most development in the health sector occurred after World War I in response to the high infectious disease mortality rates. The Rockefeller Foundation was asked to assist with the control of hookworm, tuberculosis, malaria and yaws. Its recommendations led to the growth of public health programmes (e.g. environmental health, public health nurs

Familial study of prostate cancer in Jamaica.

OBJECTIVES: Rates of prostate cancer in Kingston, Jamaica are extremely high (occurring in more than 300 men out of 100,000 in 1989 to 1993). This article addresses the familial aggregation of prostate cancer in Jamaica. Early evidence for familial prostate cancer was found in the Utah Mormon population. Increased risk of prostate cancer in men with a family history of prostate cancer has been consistently observed in subsequent studies. There have been few studies, however, involving black men, who are known to have an overall higher risk of developing prostate cancer. METHODS: Two hundred s

Chemical burns as assault injuries in Jamaica.

A three-fold greater incidence of chemical burn injuries in Jamaican hospitals, compared to burn centres in other industrial countries, underscores the problem of the use of common chemicals for assault weapons in this country. With the increased availability of guns for personal use, many Jamaicans learned the value of carrying household chemicals such as sulphuric acid from batteries or sodium hydroxide obtained from cleaning supplies. Chemicals carried in a container, such as one might carry mace, afforded a means of defence among the lower socioeconomic groups who could not afford handguns
