Management of varus or valgus ankle deformity with ankle replacement.

Ankle replacements are probably here to stay. Improved designs and surgical technique led to much better mid-term and longer term outcomes than the first-generation replacements in the 1970s. Multiple recent papers also have discussed the many potential complications with total ankle replacement surgery. As we proceed in the future, one should be cognizant of all the pitfalls and know how to deal with the difficult ankles, especially varus and valgus deformities. There should be a clear understanding that the greater the varus or valgus, the harder the procedure and the less predictable the ou

Volkmann's ischemic contracture of the foot and ankle: evaluation and treatment of established deformity.

Fibrotic contracture of skeletal muscle can follow weeks or months after the severe ischemic insult of compartment syndrome. Commonly known as Volkmann's ischemic contracture, the affected limb often becomes dysfunctional and painful, and may lose sensibility. The pathogenesis of the muscle contracture includes prolonged ischemia, myonecrosis, fibroblastic proliferation, contraction of the cicatrix, and myotendinous adhesion formation. Resultant shortening or overpull of involved muscles leads to stiffness and deformity. Simultaneously, nerve injury from initial ischemia or subsequent soft tis

Management of varus ankle osteoarthritis with joint-preserving osteotomy.

The goal of osteotomy in the treatment of varus ankle arthritis is to shift the forces imparted to the ankle to a portion of the joint that is not involved in the degenerative process. The redistribution of loads and stresses seen by the tibiotalar joint can be approached either above or below the ankle with an osteotomy of the tibia or calcaneus. Evaluation of the deformity as being subtalar, supramalleolar, or a combination allows the surgeon to best address the increased joint stresses, thereby reducing the risk of failure of the osteotomy.

Reconstruction of digital deformities in rheumatoid arthritis.

Digital deformities result from rheumatoid synovitis. These deformities are easier to treat in the early stage, when the deformity is passively correctable. Treatment options become limited as the disease progresses and the deformity becomes fixed. Surgical treatment of digital deformities is last in the priority of surgical procedures for the rheumatoid hand and wrist. It is therefore important to understand the patient's needs and expectations for improvement and attempt to match them with the surgical options that can predictably improve the patient's function. A close collaboration with th

Osteotomy of the knee: is there a seat at the table?

Osteotomy is an alternative treatment for unicompartmental osteoarthritis of the knee. In genu varum, a high valgus closing wedge osteotomy is preferred, with overcorrection by 5 degrees. Complications and poor outcomes can be overcome by incomplete osteotomy with careful preoperative planning, precise use of cutting jigs, compression fixation, and early mobilization and weight bearing. Patella baja is related to postoperative immobilization, and unicompartmental arthroplasties are more difficult to revise because of bone loss. Supracondylar femoral osteotomy is preferred for genu valgum using

Stage IV posterior tibial tendon rupture.

Adult acquired flatfoot deformity progresses through well defined stages as set out by Johnson and Strom. Myerson modified this classification system with the addition of a fourth, more advanced stage of the disease. This stage describes the involvement of the tibiotalar joint in addition to the hindfoot malalignment seen in stages II and III. This most advanced stage is comprised of a hindfoot valgus deformity, resulting from degeneration of the posterior tibial tendon, with associated valgus tilting of the talus within the mortise. The deformity at the tibiotalar joint may or may not be rigi

The digital extensor mechanism of the hand.

In summary, there is a complex interplay between the extrinsic and intrinsic components of the digital extensor mechanism. Numerous examples of dynamic anatomy and pathology of the extensor mechanism in common clinical settings have been described. Treatment principles have been outlined. This review should provide the orthopaedic surgeon with a framework for management of common hand problems.

Triple arthrodesis for the adult-acquired flatfoot deformity.

Triple arthrodesis is often the procedure of choice for end-stage adult-acquired flatfoot. The benefits of triple arthrodesis include resolution of symptoms, hindfoot realignment, and stability. This article reviews the operative technique, realignment considerations, and postoperative management when performing triple arthrodesis. When taken into account, these recommendations provide a favorable outcome for both the patient and the surgeon.

Correcting flexion contractures: getting it straight.

Many factors play a role in causing a flexion contracture associated with TKA. The final goal of correction is to achieve and maintain a contracture that is < 15 degrees. Contractures between 10 degrees and 15 degrees may still be symptomatic, and those < or = 10 degrees are rarely a problem. Perioperative and intraoperative treatment measures exist to alleviate flexion contractures. Intraoperative correction determines the final result in most cases, except in inflammatory cases where the preoperative contracture is > 40 degrees. In these cases, under anesthesia the preoperative contracture n

Management of the rigid adult acquired flatfoot deformity.

The optimal management of the adult acquired flatfoot requires careful assessment of the deformity. Although it is essential to recognize the location and degree of malalignment, it is of equal importance to appreciate whether the deformity is flexible or rigid. For patients undergoing surgery for a flexible flatfoot, various soft tissue procedures and bony osteotomies may be used to restore alignment while preserving joint motion. However, for patients undergoing surgery for a rigid deformity, such procedures are indicated less often, and arthrodesis is usually necessary.

The difficult knee: severe varus and valgus.

To correct severe varus or valgus deformity, the surgeon must have clear knowledge of the location, orientation, and physical characteristics of the soft tissue structures that provide stability to the knee. Correct ligament balance restores proper load transfer across the knee and minimizes polyethylene wear. The failure to adequately release contracted ligaments or the overzealous release of ligaments will result in instability and secondary accelerated wear of a total knee implant. I describe several methods of selective ligament release of the contracted soft tissues that contribute to fix

Selective hindfoot arthrodesis for the treatment of adult acquired flatfoot deformity: an in vitro study.

An acquired flatfoot deformity with significant laxity at the transverse tarsal joint was created experimentally and the amount of correction that was obtained with selective hindfoot fusions was measured radiographically. Results showed that the talonavicular, double (talonavicular and calcaneocuboid), and triple arthrodeses were able to fully correct the deformity, including correction of hindfoot valgus with just a talonavicular fusion. Subtalar and calcaneocuboid fusions failed to completely correct the deformity. This study provides experimental evidence that although the triple joints ar

Uncovered posterolateral rotatory elbow instability with cubitus varus deformity correction.

The relationship between a longstanding cubitus varus deformity and the delayed onset of posterolateral rotatory instability has casually been reported as being caused by abnormal repetitive varus and external rotation torques with progressive failure of the lateral collateral ligament. This article presents a case of longstanding cubitus varus in a young man, following a supracondylar fracture sustained during childhood. Posterolateral rotatory instability with lateral collateral ligament deficiency was discovered after a corrective distal humeral realignment osteotomy. The scenario was worse

[Rheumatoid arthritis: surgery of the metacarpophalangeal joints of the long fingers].

The author reviews the pathophysiological mechanisms of rheumatoid deformations of the long fingers' MP joint. He considers the different possibilities of surgical treatments; then focusing on arthroplasty, he compares and comments on technical points (incision, resection of the MP head, section of intrinsic muscles, size of the implant, joint capsule and ligament reconstruction, dressing, postoperative care) and results (mobility, implant fracture, ulnar deviation and palmar dislocation) achieved by various authors. He concludes that the Swanson implant remains at present the best therapeutic
