Infections in the joints of the hand and wrist carry the risk of significant morbidity. Common presenting symptoms include joint redness, swelling, and pseudoparalysis that occurs several days following a penetrating trauma. Diagnostic workup should be expedited, including a laboratory evaluation and arthrocentesis. Imaging, including radiographs, ultrasound, computed tomography, and/or MRI, are helpful tools in diagnosis. Once infection is identified, prompt surgical debridement and antibiotics are required. Once the infection has been managed, hand therapy is initiated to decrease the risk o

Extension of the fingers is a complex act. Boutonnière deformity is defined by flexion at the proximal inter-phalangeal (PIP) joint and hyperextension at the distal interphalangeal (DIP) joint due to disruption of the central slip of the extensor tendon. Swan neck deformity is defined by hyperextension at the PIP joint and flexion at the DIP joint, and the pathology is divided into intrinsic, extrinsic, and articular. The deformities are a result of imbalance of the tendons and ligaments in the fingers. Treatment is depending on the underlying cause and includes surgery and non-operative treat

Patients with musculoskeletal injuries to large joints often improve with physical rehabilitation. Injuries typically result from acute macrotrauma or chronic repetitive microtraumatic overuse. Rehabilitation may require therapeutic exercise techniques to regain range of motion and flexibility, increase local muscular strength and endurance, decrease swelling, and reduce pain. Rehabilitation specialists may use additional modalities, such as heat or ultrasound, to decrease acute pain or swelling, although evidence for these modalities is mixed. In addition to treatment for acute or chronic inj

Arthralgia is a rare but recognised complication of meningococcal septicaemia. We report a case of a 29-year-old man presenting with a 24 h history of fever, joint swelling and subsequent development of a non-blanching, petechial rash. He was treated for probable meningococcal septicaemia and the causative pathogen was later identified as Neisseria meningitidis. He was treated with ceftriaxone and after 10 days the pain and swelling in his joints improved.

Although simple characterization of discomfort as cramps, heaviness, shooting pains, and so forth can be misleading, history and examination are key to accurate diagnosis. Absence of both dorsalis pedis and posterior tibial pulses strongly suggests peripheral arterial disease (PAD), and the presence of either pulse makes PAD less likely. Hydroxymethylglutaryl coenzyme A reductase inhibitors (statins) are a common cause of lower extremity myalgias. Restless legs syndrome causes nocturnal discomfort but must be distinguished from confounding“mimics." Neurologic causes of leg symptoms include lum

OBJECTIVES: The objective of this study is to present two series of cases of pigmented villonodular disease of joints (PVNS) treated at different periods. The 215 cases from our hospital are current (1972-2009) and those 62 from the Jaffe collection from 1920 to 1968.
METHODS: By use of our computerized tumor data system we were able to define age, gender, site, treatment, complications, and outcome. Examination of the material from the Jaffe collection included all of the above primary data but none on treatment and outcome.
RESULTS: The data for the two systems studied were remarkably simi

BACKGROUND AND PURPOSE: Septic arthritis of finger joints is rare and its management not standardized. The outcome of all consecutive patients with finger joint arthritis was analyzed in terms of risk factors, surgical technique, antimicrobial therapy and hand therapy.
PATIENTS AND METHODS: Data of 31 patients with surgical treatment between 1993 and 2005 were screened and those from 29 patients with > 1 year of follow-up were retrospectively analyzed. Surgical techniques were debridement, primary, secondary arthrodesis or temporary joint distraction with external fixation. In addition, all p

A retrospective study of 45 patients hospitalized with blastomycosis of bones or joints revealed 41 cases of osteomyelitis and 12 cases of septic arthritis. The majority were men (35 [78%] patients) and non-Aboriginal (32 [71%] patients). Median time from the onset of symptoms to hospitalization was shorter in women than men (male, 48 d; female, 14 d; P < 0.02), and shorter for Aboriginals than non-Aboriginals (non-Aboriginal, 50 d; Aboriginal, 19 d; P < 0.04). Cutaneous disease was present in 33 (73%) patients, and lung involvement was present in 29 (64%) patients. The most common osseous sit

Para-articular cysts frequently are seen on routine imaging examinations. They may be clinically asymptomatic or may cause pain, swelling, or impaired joint function. They often are associated with underlying joint disorders, such as trauma, degeneration, or inflammation. This article discusses the cause, symptoms, and appearance of para-articular cysts on various imaging examinations including arthrography, ultrasound, CT scan, and MR imaging. Specific attention is focused on those cystic masses appearing around the hip, knee, ankle and foot, shoulder, elbow, wrist and hand, spine, temporoman

Facet joints have a variable anatomic orientation, and their imaging must be adapted to this variation. They have an important role in the stability and mobility of the spine at all levels. They are the sites of microtrauma leading to degenerative changes. Trauma may cause subluxation, dislocation, and fracture. As synovial joints, they manifest inflammatory processes and can present as part of a systemic disease. The facet joints are a component of a trijoint complex at each motion segment and cannot be separated biomechanically. Their visualization and analysis must be integrated with the ad

The authors applied clinico-genetic methods of analysis to work out the data concerning familial spread of hypermobility of the joints (567 inhabitants of Tbilisi, 302 children with congenital dislocation of the hip joint). A common genetic character of this pathology and congenital dislocation of the hip was established (88% of common genes). Approximate evaluations of relative contribution of the genetic component into the general dispersion (93%) and of the penetrance of the gene were obtained. A monogenous dominant type of inheritance was revealed.

With the exception of fracture dislocations following fractures of the ankle joint and talus, dislocations in the talus are very infrequent injuries. They pose a lot of management problems: soft-tissue damage in open and closed dislocations, imperfect reduction caused by osteochondral fragments, recurrent instability, and aseptic necrosis of the talus, which is greatly feared. Anatomical and prognostic criteria are included in a classification of mono-, bi- and triarticular dislocations of the talus. Based on six of our own cases treated in the last 12 months, we report the mechanisms of injur

Problems in the diagnosis, reduction, and maintenance of reduction of dislocations of the tarsometatarsal joints are discussed. Many different patterns may present due to the anatomy of the foot and ankle and the different mechanisms of injury. Closed reduction and maintenance of reduction may be difficult to achieve, and open reduction may be required. Fixation with multiple K wires is useful in maintaining reduction. The possibility of vascular compromise should be kept in mind. Although anatomical reduction should be the goal of treatment, this does not always assure a painless foot. Arthro

Twenty-six patients with suspected or known septic joints who had been studied by computed tomography were retrospectively reviewed. Eighteen patients (20 joints) proved to have septic arthritis. Computed tomography was advantageous in patients with acute septic arthritis of deep-seated axial joints, such as the hip and sacroiliac, where it demonstrated particular soft tissue swelling as early as 36 hours after the onset of symptoms. In three patients, this finding on computed tomography was helpful in accurately localizing the disease process and facilitating a diagnostic closed needle aspira

In 34 patients with different chronic skin diseases (including eczemas, lichen ruber, erythrodermia, pyodermia, pemphigus erythematosus, vasculitis, parapsoriasis varioliformis, scleroderma adultorum Buschke, prophyria cutanea tarda, rosacea, dermatomyositis, erythematodes chronicus discoides and indurative tuberculosis), X-ray studies revealed sacroiliac-changes consistent with a low-grade inflammation. This oteoarthritis slightly differs from the patterns disclosed earlier by the same authors in psoriatic patients, lacking any clinical bone or joint symptoms. So far, it has not been possible

The most common disease of the temporomandibular joint (TMJ) is osteoarthritis. Rheumatoid arthritis and psoriatic arthritis may also involve this joint. Other diseases that may occasionally affect the TMJ include familial Mediterranean fever, systemic lupus erythematosus, gout, Sjögren's syndrome and infectious arthritis. Many cases of TMJ syndrome are labeled as idiopathic facial pain syndrome, a category that probably represents a number of different entities. The role of dental malocclusion has been greatly overemphasized in the past.
