Lower cranial nerve (LCN) schwannomas are rare, benign peripheral nerve sheath tumours arising from cranial nerves IX-XII. Though uncommon, they pose significant clinical challenges due to their complex anatomical location and potential for neurological deficits. Jugular foramen schwannomas (JFS) and hypoglossal schwannomas (HS) are the most prevalent subtypes. Clinical presentation varies by nerve involvement, ranging from hoarseness and dysphagia to tongue atrophy and paroxysmal cough. Diagnosis relies heavily on MRI and CT imaging, with differentiation from paragangliomas being critical. Cl

Jugular foramen (JF) metastasis is rare and often presents as JF syndrome. A 73-year-old male complained of left-sided mastoid pain that irradiated to the neck since the past 3 months. Onset of facial nerve (FN) palsy and persistence of the symptomatology despite corticosteroid therapy demanded radiologic evaluation. Computed tomography and magnetic resonance imaging showed a wide osteolytic lesion of the left JF with involvement of the third segment of the FN. The patient underwent transmastoid incisional biopsy. Histopathological examination showed an adenocarcinoma that was suggested to be

The present study was designed to examine the vagus nerve system, atrioventricular ganglion, and changes in cardiomyocytes in 45 cases of the fatal outcome of the disease among 20 patients who suffered a craniovertebral injury and 25 ones presenting with acute coronary insufficiency. Special attention was given to the central and peripheral parts of the vagus nerve system. The study revealed lesions in the jugular ganglia and their association with the previous craniovertebral injuries accompanied by hemorrhage in these ganglia, nerve trunks, and soft tissues of the jugular foramina. The well

Meningiomas are the second largest group of brain tumors after gliomas. They account for 13 to 18 per cent of all primary intracranial neoplasms. The majority occur in the cerebral chamber; only 8 to 9 per cent are located in the cerebellar chamber. Meningiomas are hamartomatous, not truly neoplastic tumors, which arise from dural fibroblasts and particularly from arachnoid cells that tend to cluster around the tips of the arachnoid villi. Their preferential sites correspond closely with the locations where arachnoid villi are most frequently encountered, namely, along the major venous sinuses

IntroductionIdiopathic intracranial hypertension is a condition characterized by elevated intracranial pressure in the absence of an intracranial mass lesion and is commonly associated with neuro-ophthalmic symptoms. Although venous outflow abnormalities, particularly transverse sinus stenosis, have been extensively investigated in idiopathic intracranial hypertension, the potential role of jugular foramen volume has not been previously studied. Considering that major venous structures pass through the jugular foramen, this study aimed to evaluate whether jugular foramen volume plays a role in

The jugular foramen (JF) is one of the most complex skull base apertures. Recent studies have expanded knowledge on JF morphology, yet rare ossified variants remain underreported. We describe a unique case observed in a dry skull, where two complete ossified septa subdivided the right JF into three distinct compartments. Morphometric analysis revealed a large posterior venous compartment and two anterior neural compartments separated by a thin bony plate, representing the coexistence of a well-differentiated ossified jugular process and an ossified dural septum. Neurosurgically, such ossificat

Facial nerve schwannoma (FNS) is a benign, slow-growing schwannoma that originates from Schwann cells. Facial nerve schwannoma is the most common tumor of the facial nerve but rare and only accounts for 0.15% to 0.8% of intracranial neurinomas. It may be manifested as asymmetric hearing loss, facial palsy, and hemifacial spasm. A 56-year-old woman was transferred to our department, because of pain behind the right ear and spasm of the right lateral muscle for more than 2 years and pulsatile tinnitus for half a year. Based on the preoperative medical history, physical signs, and auxiliary exami

Before the application of CT scan, the exact identification of extracranial extension of brain tumors was difficult. In the present paper, neuroradiological findings of eight brain tumors with extracranial extension were reviewed. These eight cases were composed of two sphenoidal meningiomas, one CP angle meningioma, one CP angle neurofibroma, two pituitary adenomas, and two jugular foramen chemodectomas. After the first application of CT scan on May, 1976, 841 cases of primary intracranial tumors have been experienced in our clinic. So the incidence of brain tumors with extracranial extension

Hypoglossal nerve palsy is most commonly associated with trauma, tumors, or vascular insults affecting the skull base, but isolated palsy due to jugular paraganglioma is extremely rare. We present the case of a woman in her 40s with progressive dysarthria and isolated left hypoglossal nerve palsy. MRI revealed a vascular lesion localized in the left jugular foramen with extension into the hypoglossal canal, sparing the adjacent lower cranial nerves. The absence of involvement of cranial nerves IX, X, and XI ruled out a complete Collet-Sicard syndrome. Imaging characteristics and anatomical cor

BACKGROUND AND PURPOSE: Jugular foraminal stenosis (JFS) or atresia (JFA) with collateral emissary veins (EV) has been documented in syndromic craniosynostosis. Disruption of EV during surgery can produce massive hemorrhage. Our purpose was to describe the prevalence of prominent basal emissary foramina (EF), which transmit enlarged EV, in syndromic craniosynostosis. Our findings were correlated with phenotypic and molecular diagnoses.
METHODS: We reviewed the medical records and imaging examinations of 33 patients with syndromic craniosynostosis and known fibroblast growth factor receptor (F

INTRODUCTION: The anatomical complexity of the jugular foramen makes surgical procedures in this region delicate and difficult. Due to the advances in surgical techniques, approaches to the jugular foramen became more frequent, requiring improvement of the knowledge of this region anatomy.
OBJECTIVE: To study the anatomy of the jugular foramen, internal jugular vein and glossopharyngeal, vagus and accessory nerves, and to identify the anatomical relationships among these structures in the jugular foramen region and lateral-pharyngeal space.
METHODS: A total of 60 sides of 30 non-embalmed cad

OBJECTIVE: To measure the size of jugular foramina in infants affected by external hydrocephalus (EH) and in a control group, to support the hypothesis that a jugular foramen (JF) stenosis may determine dural venous sinus alterations and increased venous outflow resistance as main pathophysiological factor.
METHODS: Minimum, maximum, and mean values of JF areas were measured in a series of phase-contrast magnetic resonance venous angiography (angio MRV PCA3D) performed on 81 infants affected by EH. Results were compared with a group of 54 controls.
RESULTS: Smaller JF area was significantly

The posterior inferior cerebellar artery (PICA) rarely arises from the cavernous segment of the internal carotid artery (ICA) and is called persistent trigeminal artery variant. The PICA also can arise from the cervical segment of the ICA, and it enters the posterior fossa via the hypoglossal canal, where it is called persistent hypoglossal artery variant. Using magnetic resonance angiography (MRA), we diagnosed a 79-year-old man with a PICA arising from the ascending pharyngeal artery and passing through the medial side of the jugular foremen pars vascularis. Only six cases of this variation

In achondroplasia, venous narrowing associated with a small skull base leads to elevated venous pressure which impairs cerebrospinal fluid (CSF) absorption, resulting in communicating hydrocephalus. We correlated venous narrowing at the jugular foramina and collateral circulation on magnetic resonance venography (MRV) with ventricular size in 17 patients. Patients were divided into three groups: Group I: progressive hydrocephalus; Group II: nonprogressive hydrocephalus; and Group III: normal ventricular size. The grades of venous narrowing and types of collaterals were well correlated with hyd

BACKGROUND: Meningiomas rarely extend out of their intracranial confines through skull foramina to present as cervical tumors, where they would be accessible to fine needle aspiration (FNA) and thereby create difficulties in cytodiagnosis by mimicking other, more commonly aspirated head and neck tumors.
CASE: A psammomatous meningioma arising intracranially and extending through the jugular foramen presented as a mass at the angle of the jaw clinically. On FNA cytology the diagnosis was suggested, but the cytomorphologic features overlapped with those of more commonly aspirated head and neck

BACKGROUND AND OBJECTIVES: Surgical resection of jugular foramen (JF) schwannomas with minimal neurological complications is challenging because of their difficult-to-access location and complex relationships with surrounding neurovascular structures, even for experienced neurosurgeons. In this article, we elucidate the membranous anatomy of JF schwannomas, with the aim of reducing iatrogenic injury to the lower cranial nerves (LCNs) during surgery.
METHODS: The clinical data of 31 consecutive patients with JF schwannomas were reviewed. The relationship between the tumor and the surrounding m

BACKGROUND: Multiple lower cranial nerve palsies have been attributed to occipital condyle fractures in older children and adults, but no clinical details of other possible mechanisms have been described in infants.
CASE REPORT: A 33-month-old boy suffered blunt head trauma. A bilateral skull base fracture was diagnosed, with favorable outcome during the first days after trauma. On the sixth day, the patient began to refuse drinking and developed hoarseness. Physical examination and additional investigations revealed paralysis of left VII, IX, X, and XI cranial nerves. A follow-up computed to
