Extracellular matrix changes in human corneas after radial keratotomy.

Extracellular matrix and basement membrane alterations were identified in human corneas after radial keratotomy. Ten normal and five radial keratotomy autopsy corneas (two at 6 months post surgery, and three at 3 years post surgery) were studied by immunofluorescence with antibodies to 28 extracellular matrix and basement membrane components. Outside of radial keratotomy scars, all studied components had a normal distribution. Of stromal extracellular matrix, only type III collagen accumulated around the scars. The basement membrane around epithelial plugs had a normal composition except for t

Bilateral Serratia marcescens keratitis after simultaneous bilateral radial keratotomy.

PURPOSE/METHODS: After bilateral simultaneous radial keratotomy, Serratia marcescens keratitis, which involved multiple incisions of both eyes, developed in a 46-year-old physician. The keratitis was treated with repeated wound debridement, fortified topical antibiotics, and topical povidone-iodine. RESULTS/CONCLUSIONS: Six months after radial keratotomy, uncorrected visual acuity was R.E.: 20/25 and L.E.: 20/60, both eyes correctable to 20/20. Health-care workers who undergo refractive surgery may be at increased risk of acquired postoperative infections because of their work environment. Al

Mycobacterium smegmatis keratitis after radial keratotomy--a case report.

A 42-year-old woman who developed a corneal ulcer in the left eye after radial keratotomy. The acid-fast stain showed that numerous clusters of acid-fast bacilli and Mycobacterium smegmatis were isolated on the culture. To the best of our knowledge, this is the first documented case of Mycobacterium smegmatis keratitis in the world literature. The sensitivity test showed that this organism resistant to all the anti-tuberculous agents and antibiotics we test. An initial response to amikacin and kanamycin was found, but relapse of infiltrate was noted one month later. The regimen was therefore c

Late development of ulcerative keratitis in radial keratotomy scars.

We describe three patients in whom ulcerative keratitis developed seven months to 2 1/2 years after uncomplicated radial keratotomy. All ulcers occurred along keratotomy scars within the interpalpebral fissure or inferiorly. At least one case developed spontaneously; another patient may have sustained an unrecognized corneal injury. The third patient was using an extended-wear soft contact lens for correction of residual myopia. Bacteria were isolated from two of the ulcers. With therapy, all ulcers healed without reducing visual acuity. We hypothesize that the altered structure of the slowly

Modification of corneal curvature following radial keratotomy in primates.

Radial keratotomy was performed on six stumptail monkey eyes (Macaca speciosa) and 22 owl monkey eyes (Aotus trivirgotus). Changes in keratometry, specular microscopy, and tonometry have been studied postoperatively for three to six months. Sixteen radial incisions in stumptail monkey eyes resulted in a significant mean corneal flattening of 2.75 diopters (P less than 0.005). This effect was stable with a mean 2.50 diopters flattening remaining six months after surgery. Sixteen radial incisions in ten owl monkey eyes resulted in a much greater short-term effect, with a mean 10.50 diopters of f

Intraocular lens calculation for cataract after previous radial keratotomy.

PURPOSE: The purpose of this study is to discuss the major reasons for the hyperopic shift after cataract surgery following radial keratotomy (RK) and to find potential methods to improve the prediction of intraocular lens power (IOLP). METHODS: The results for 18 cataract surgery eyes that had undergone RK were analysed retrospectively. The IOLP was calculated using Haigis, Hoffer Q, and SRK/II formulae with different keratometric measurements. Theoretical IOLP miscalculation ((delta)IOLP(K)) was assessed in order to define the optimal combination of different keratometric parameters and int

Corneal tensile strength in fully healed radial keratotomy wounds.

PURPOSE: To investigate the susceptibility to rupture of a human donor cornea with fully healed radial keratotomy (RK) incisions using biomechanical measurement methods and finite element analysis. METHODS: A human cornea 8 years after RK was cut into four strips and subjected to tensile testing until rupture occurred. The fellow cornea was pressurized on an artificial anterior chamber beyond the initiation of wound gape. The tensile strengths of nine strips from unincised corneas were measured as controls. To investigate the effect of epithelial plugs on the RK strip test results, a finite e

Differences between objective and subjective refractions after radial keratotomy.

BACKGROUND: In patients who are free of pathology, automatic refractions have shown close agreement with the subjective refractions. Clinical experience indicated that the normally strong relationship between objective and subjective refraction is significantly weakened as a result of radial keratotomy. METHODS: Seventy-two patients were refracted before and after surgery, objectively with a Humphrey Model #510 autorefractor and subjectively using a binocular refraction procedure without cycloplegia. All patients were free of ocular disease and had preoperative myopia ranging from -1.00 to -9

Tissue addition theory of radial keratotomy: a geometric model.

We examined the use of a geometric model to explain the effects of radial keratotomy (RK) on corneal topography based on wound gape of radial incisions. Histopathologic studies of healed human RK incisions revealed incision gape in Bowman's membrane and anterior stroma, with intervening epithelial ingrowth or fibrous scarring. For six human RK incisions examined centrally, mid-peripherally, and peripherally, the linear separation of the cut ends of Bowman's membrane averaged 0.012 mm, 0.018 mm, and 0.027 mm, respectively. The effect of postoperative corneal flattening of the "addition of tissu

Visual impairment following radial keratotomy. A cluster of cases.

Thirteen patients who underwent radial keratotomy developed complications leading to visual impairment. Three eyes were legally blind. Two groups of complications were seen: those common to surgical procedures involving the eye--optic atrophy, infections, cataract and retinal detachment, and those unique to radial keratotomy--complete failure of the procedure, marked undercorrection, marked overcorrection, and induced astigmatism. Symptoms due to anisometropia were prominent in the latter group who considered themselves visually disabled by the surgery. Radial keratotomy, like all surgical pro

[On the question of progressive hyperopia correction after radial keratotomy].

Progressive hyperopia development in the long-term period after radial keratotomy (RC) is associated with not only surgical effects, but also individual biomechanical features of the fibrous tunic of the eye, its age-related changes, and hydrodynamic disturbances. When choosing a method of correction, one should take into account a number of features characteristic of this induced refractive disorder: instability and irregularity of refraction, coexisting presbyopia, and, in some cases, anisometropia. Correction of progressive hyperopia after RC can be achieved with eyeglasses, or contacts, or

Radial keratotomy: background and how to manage these patients nowadays.

In this review, we presented the principles of radial keratotomy (RK), its evolution, enhancement, and complications, and strategies to manage the consequences of RK in the present day. It is essential to understand the RK procedure f, the theoretical background that supported this surgery, the current effect on the cornea, and how to approach patients needing vision improvement. These patients are developing cataracts that need to be handled well, from the IOL calculation to the surgical procedure. Guided keratorefractive surgery is the most accurate procedure to improve these patient's visio

Rare case of corneal keloid following radial keratotomy for myopia.

An adult male in his 50s presented with complaints of glare and gradual, painless, progressive diminution of vision in the right eye (RE). Visual acuity in RE was noted to be 2/60, and slit lamp biomicroscopy revealed a pearly grey-white elevated corneal opacity measuring 4 mm × 3 mm, obscuring the visual axis. There was no history of ocular trauma or infection. The patient had undergone bilateral radial keratotomy for myopia correction 25 years ago. Anterior segment optical coherence tomography imaging demonstrated increased corneal thickness of 1080 µm at the site of lesion and the height of

Corneal hysteresis in post-radial keratotomy primary open-angle glaucoma.

PURPOSE: Corneal hysteresis (CH) is a corneal biomechanical property measured by the ocular response analyzer (ORA). It is associated with primary open-angle glaucoma development, progression, and severity as well as intraocular pressure (IOP) measurement. Decreases in CH and changes in IOP measurements have been described for laser-assisted refractive surgery; however, patients with prior radial keratotomy (RK) have not been examined. We have performed a cohort study examining CH and intraocular pressure measurements (Goldmann applanation and ORA values including Goldmann-correlated and corne
