Therapeutic Ketosis for Heart Failure: A State-of-the-Art Review.

Heart failure is characterized by an energy-deprived heart, and in recent years it has been found that the failing heart increases ketone body oxidation to meet its energy demands. Accumulating evidence suggests that this metabolic adaptation is cardioprotective, suggesting that interventions that boost blood ketone levels could aid the failing heart. Indeed, multiple small clinical trials with short-term follow up have demonstrated that supplying the failing heart with exogenous ketone bodies may improve myocardial function across various manifestations of heart failure. As such, therapeutic

Salutary Effects of Nutritional Ketosis for the Diseased Human Heart.

PURPOSE OF REVIEW: We provide an overview of cardiac metabolism, ketone physiology and terminology, methods of elevating ketones and their effect on cardiac function and disease. We discuss future research directions and speculate what ketogenic strategies may yield optimal effects on the heart and cardiovascular disease. RECENT FINDINGS: Nutritional ketosis acutely elevates cardiac function (cardiac output, myocardial perfusion, etc.) in healthy people and those with cardiovascular disease in a dose-dependent manner between circulating ketones and cardiac function. Despite therapeutic potent

Prepartum feeding behavior is an early indicator of subclinical ketosis.

Cows diagnosed with subclinical ketosis (SCK) after calving are at increased risk of developing other diseases and compromised reproductive performance. The objective of this study was to determine whether changes in feeding and social behaviors during the transition period were associated with SCK during the week after calving. Feeding behaviors of 101 Holstein dairy cows were monitored from 3 wk before to 3 wk after calving. Ten otherwise healthy animals were identified as having SCK by serum beta-hydroxybutyrate levels >or=1,000 micromol/L taken during wk +1. These animals were matched by p

[Effect of ketosis on the reproductive process in high-yield cows].

Studies were carried out with a total of 40 ketosis-affected cows and 40 normal (control) cows. It was found that with the diseased animals the blood level of sugar was lowered by 45 per cent, that of total protein--by 24 per cent, that of carotene and vitamin A--by more than twice, and that of inorganic phosphorus--by 20 per cent as against the blood indices with the control cows. In the same period the content of ketone bodies in the blood of ths affected cows increased 5 times, and in the urine it rose 6 times. With cows that had ketosis the cases of dystocia and retained placenta were 5 ti

Carnitine and carnitine palmitoyltransferase in fatty acid oxidation and ketosis.

Carnitine is an essential factor in long-chain fatty acid oxidation. Carnitine acts as a carrier of fatty acyl groups from the cytoplasm to the mitochondrion. Long-chain acyl-CoA derivatives do not penetrate the mitochondrial inner membrane. Carnitine palmitoyltransferase A (CPT-A), located on the external surface of the inner membrane, catalyzes the conversion of cytoplasmic long-chain acyl-CoA and carnitine into acylcarnitine. The acylcarnitine is reconverted to intramitochondrial acyl-CoA by the action of carnitine palmitoyltransferase B located in the inner membrane. Now, the acyl-CoA is a

Ketosis and ketoacidosis in hospitalized patients receiving SGLT2 inhibitor therapy.

AIMS: The use and safety of sodium-glucose co-transporter 2 inhibitors (SGLT2i) in hospital are controversial due to the risk of ketoacidosis. We aimed to describe rates of ketosis, ketoacidosis, and acidosis in patients with type 2 diabetes treated with in-hospital SGLT2i use. MATERIALS AND METHODS: Single-centre retrospective cohort study of adult patients with type 2 diabetes admitted for >24 h. Rates of ketone testing, ketone levels, ketosis precipitants, and ketoacidosis and acidosis frequency were determined from examination of electronic medical records. Outcomes were described relativ

Exogenous ketosis suppresses diuresis and atrial natriuretic peptide during exercise.

We have previously demonstrated that exogenous ketosis reduces urine production during exercise. However, the underlying physiological mechanism of this antidiuretic effect remained unclear. Therefore, we investigated whether acute exogenous ketosis by oral ingestion of ketone ester (KE) during a simulated cycling race (RACE) affects the hormonal pathways implicated in fluid balance regulation during exercise. In a double-blind crossover design, 11 well-trained male cyclists participated in RACE consisting of a 3-h submaximal intermittent cycling (IMT180') bout followed by a 15-min time trial

Chronic Ketosis Modulates HIF1α-Mediated Inflammatory Response in Rat Brain.

Hypoxia inducible factor alpha (HIF1α) is associated with neuroprotection conferred by diet-induced ketosis, but the underlying mechanism remains unclear. In this study, we use a ketogenic diet in rodents to induce a metabolic state of chronic ketosis, as measured by elevated blood ketone bodies. Chronic ketosis correlates with neuroprotection in both aged and following focal cerebral ischemia and reperfusion (via middle cerebral artery occlusion, MCAO) in mouse and rat models. Ketone bodies are known to be used efficiently by the brain, and metabolism of ketone bodies is associated with incre

Ketosis and appetite-mediating nutrients and hormones after weight loss.

BACKGROUND/OBJECTIVES: Diet-induced weight loss is accompanied by compensatory changes, which increase appetite and encourage weight regain. There is some evidence that ketogenic diets suppress appetite. The objective is to examine the effect of ketosis on a number of circulating factors involved in appetite regulation, following diet-induced weight loss. SUBJECTS/METHODS: Of 50 non-diabetic overweight or obese subjects who began the study, 39 completed an 8-week ketogenic very-low-energy diet (VLED), followed by 2 weeks of reintroduction of foods. Following weight loss, circulating concentra

One third of cases of new-onset diabetic ketosis in adults are associated with ketosis-prone type 2 diabetes-A systematic review and meta-analysis.

AIMS: Ketosis-prone type 2 diabetes was defined by the World Health Organization in 2019. According to the literature, the diagnosis is based on the presence of ketosis, islet autoantibody negativity and preserved insulin secretion. Our meta-analysis assessed the prevalence and clinical characteristics of ketosis-prone type 2 diabetes among patients hospitalised with diabetic ketoacidosis (DKA) or ketosis. METHODS: The systematic search was performed in five main databases as of 15 October 2021 without restrictions. We calculated the pooled prevalence of ketosis-prone type 2 diabetes (exposed

Ketosis suppression and ageing (KetoSAge): the effect of suppressing ketosis on SHBG and sex hormone profiles in healthy premenopausal women, and its implications for cancer risk and therapy.

INTRODUCTION: Insulin resistance and hyperinsulinaemia significantly influence female hormone regulation and reproductive health. Despite increasing research, the complex pathways by which nutritional and metabolic signals regulate reproductive function remain poorly understood. Sex hormone-binding globulin (SHBG) is a key protein whose function is modulated by hyperinsulinaemia, liver function, and metabolic status, thereby influencing the active signalling of circulating sex steroids and intracellular signalling, which in turn, impacts endocrine and reproductive physiology. Consequently, SHB

Hyperglycaemia Presenting With Hemichorea: A Rare Case of Diabetic Striatopathy With Ketosis.

Acute movement disorder is a rare complication of diabetes mellitus. Uncontrolled diabetes mellitus can present with various movement disorders, of which chorea and ballism are the most common. Various nomenclatures have been used in the past for this condition. However, diabetic striatopathy is the most common and comprehensive terminology. It refers to a condition of hyperglycaemia and an acute-onset movement disorder with or without characteristic radiological findings on CT or MRI scan. It is commonly associated with non-ketotic hyperglycaemia in the background of type 2 diabetes mellitus.

Ketosis-prone Diabetes and Hypogonadism: A New Clinical Association to be Aware of ?

BACKGROUND: Ketosis-prone diabetes (KPD) is an emerging entity, sharing features of both type 1 diabetes mellitus and type 2 diabetes mellitus. Patients with KPD usually present with diabetic ketoacidosis without the classic phenotype of autoimmune type 1 diabetes. In most cases, they are Afro-American adults, who require insulin therapy for the management of acute decompensation, then usually encountering insulin-free remission for prolonged periods of time with diet or with non-insulin agents. Meanwhile, hypogonadism is a known condition that could be associated with higher risk of developin

Pathophysiological Abnormalities in Migraine Ameliorated by Ketosis: A Proof-of-Concept Review.

Migraine is a prevalent heterogeneous neurological disorder, enumerated as the eighth most disabling neurological disorder by the World Health Organization. The growing advancement in technology and investigation of various facets of cerebral metabolism in migraine has shed light to metabolic mechanisms in migraine pathophysiology. A growing number of clinical research postulates migraine as a reaction to oxidative stress levels that go beyond antioxidant capacity or cerebral energy deficiency. This has become an extremely attractive subject area and over the past years there has also been a s

Nutritional Ketosis in Parkinson's Disease - a Review of Remaining Questions and Insights.

Nutritional ketosis has promise for treating Parkinson's disease. Three previous studies explored the use of a ketogenic diet in cohorts with Parkinson's disease, and, while not conclusive, the data suggest non-motor symptom benefit. Before the ketogenic diet can be considered as a therapeutic option, it is important to establish with greater certainty that there is a reliable symptomatic benefit: which symptoms or groups of symptoms are impacted (if non-motor symptoms, which ones, and by which mechanism), what timescale is needed to obtain benefit, and how large an effect size can be achieved

Implications of Altered Ketone Metabolism and Therapeutic Ketosis in Heart Failure.

Despite existing therapy, patients with heart failure (HF) experience substantial morbidity and mortality, highlighting the urgent need to identify novel pathophysiological mechanisms and therapies, as well. Traditional models for pharmacological intervention have targeted neurohormonal axes and hemodynamic disturbances in HF. However, several studies have now highlighted the potential for ketone metabolic modulation as a promising treatment paradigm. During the pathophysiological progression of HF, the failing heart reduces fatty acid and glucose oxidation, with associated increases in ketone
