Kidney transplantation during autoimmune diseases.

Herein, we report the results of kidney transplantation in 9 of 376 patients who underwent kidney transplantation at our center between 1986 and 2007 because of chronic renal failure associated with autoimmune disease. Four of the 9 patients had systemic lupus erythematosus, 3 had Wegener granulomatosis, and 2 had Goodpasture syndrome. Six patients received organs from living donors, and 3 received cadaver organs. Infections were frequent and included cytomegalovirus and urinary tract infection in most cases. There was no difference in occurrence of metabolic and cardiovascular complications i

Polycystic liver and kidney diseases.

There have been remarkable advances in research on polycystic liver and kidney diseases recently, covering cloning of new genes, refining disease classifications, and advances in understanding more about the molecular pathology of these diseases. Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary disease affecting kidneys. It affects 1/400 to 1/1000 live births and accounts for 5% of the end stage renal disease in the United States and Europe, and is caused by gene defects in the PKD1 or PKD2 genes. Compared to ADPKD, polycystic liver disease (PCLD) is a milder …

[Skin manifestations in kidney diseases].

Skin irritations in conjunction with renal disorders are often specific. Therefore a close inspection of the integument may point to an internal organ pathology. A selection of hereditary syndromes and a summary table are shown including acquired diseases displaying symptoms of the skin and the kidneys. A special emphasis is laid on specific and unspecific skin alterations under chronic renal failure. Instead of a detailed list of therapeutic possibilities, priority is given to the exact description of macroscopic and microscopic skin alterations including pathophysiologic mechanisms.

[Pathogenesis of cystic kidney diseases].

Several gene mutations cause renal cysts in humans, including PKD1 and PKD2 (autosomal dominant polycystic kidney disease, ADPKD), PKHD1 (autosomal recessive polycystic kidney disease, ARPKD), TSC1 and TSC2 (Tuberous Sclerosis), NPHP1 (Nephronophthisis type I), and VHL (von-Hippel-Lindau syndrome). This raises the question, whether there are common denominators of cyst development. Both polycystin-1 and fibrocystin, the gene products of PKD1 and PKHD1, appear to act as adhesion molecules. Polycystin-2, encoded by PKD2, is a ion channel that requires polycystin-1 to translocate to the plasma me

[Hereditary diseases causing kidney calculi].

Urolithiasis and/or nephrocalcinosis due to hereditary diseases are a rare event which must be kept in mind of physicians who take care of children (10 to 40% of all causes of lithiases) as well as of adults (less than 15% of all causes of lithiases) since a specific management is usually required. The most frequent inborn disorders are idiopathic hypercalciuria, distal tubular acidosis, cystinuria and hyperoxaluria. Stone formation is always secondary to an increased urine concentration of promotors, i.e. calcium, oxalate, phosphate, cystine, xanthine. One of the most informative diagnosis in

Inherited diseases of the kidney.

It has long been known that a number of diseases affecting the kidney are the result of genetic defects passed on through the generations. Whereas some of these defects are rare, others, eg, the cystic diseases, are among the most common. Our understanding of the underlying pathobiology in these disorders based on physiologic and cell biologic studies is variable--we suspect that the V2 vasopressin receptor is defective in nephrogenic diabetes insipidus; we know that the glomerular basement membrane in Alport syndrome is abnormal; we suspect that a tumor suppressor gene is defective in Wilms t

[Ultrasonic diagnosis of kidney diseases].

Applied concomitantly with other methods ultrasonic testing is claimed to furnish reliable results in the diagnosis of renal disease. A total of 177 cases with radiologically confirmed urologic renal disease were tested by means of the ultrasonic method. Of the cases 128, [72.3%] were renal tumours and cysts. The echo-tests involves no risk, can be performed on outpatients, and repeated whenever necessary without special preparation.
