Knee effusions.

There are many questions concerning knee effusions and the basic diagnostic approach to them. The case of a young female with gonococcal arthritis presenting to the Emergency Room with an isolated knee effusion is discussed. From the errors in diagnosis of this case, a basic outline in the approach and work-up of a knee effusion is detailed, including history and physical examination, and the use of synovial fluid analysis as a further diagnostic aid. Procedural instructions and contraindications for knee arthrocentesis are given, as well as data related to interpretation of pertinent laborato

Primary Total Knee Arthroplasty for Fractures Around the Knee.

Primary arthroplasty for fractures of the femoral neck is a very common procedure and even the gold standard for the geriatric patient. In contrast, primary arthroplasty for perigenicular fractures is a rare event. On the other hand, it can be a therapeutic option, especially for geriatric patients with prior osteoarthritis or complex fractures and severe osteoporosis. The operative procedure can be complex and requires experience in primary and revision arthroplasty as well as suitable implants and good preoperative planning. There are case series reporting primary arthroplasty in a total of ap

Predicting Knee Pain and Knee Osteoarthritis Among Overweight Women.

BACKGROUND: There is a need for prediction of knee osteoarthritis (KOA) in general practice to motivate subjects for preventive therapies and optimize preventive trials. AIM: To develop a prediction model, with questionnaire and physical examination variables, for incident frequent knee pain (FKP) and symptomatic KOA after 2.5 and/or 6.5 years among overweight and obese middle-aged women. DESIGN AND SETTING: Models were developed in the Prevention of Knee Osteoarthritis in Overweight Females study (age 50 to 60 years, body mass index [BMI] ≥ 27 kg/m2) (ISRCTN 42823086). FKP was defined as kn

Posterior Knee Loose Bodies: Treated by Posterior Knee Arthroscopy.

INTRODUCTION: The posterior portion of the knee joint, which includes the tibial attachment of the posterior cruciate ligament and the posterior horn of the menisci, has been called a "blind spot" because it is difficult to observe this area under arthroscopy through standard anterior portals. Posteromedial, posterolateral, and posterior transseptal portals have been developed for visualization and instrumentation of the posteromedial and posterolateral compartments of the knee joint. CASE REPORT: A 57-year-old man presented of persistent left posterior knee pain for 1 year. Radiographs and m

Knee Disarticulation for a Total Knee Arthroplasty Periprosthetic Fracture.

Total knee arthroplasty, although a very successful surgery, can present the patient with several potential complications, including the risk for periprosthetic fractures. Complications involving periprosthetic fractures after total knee arthroplasty can present a complex clinical scenario for orthopedic surgeons. To date, adequate literature exists to guide surgeons in approaching most periprosthetic fractures; however, standard operating procedure can fall short when confronted with more complex clinical scenarios. The authors present an alternative approach, falling outside the traditional

Knee flexion after total knee arthroplasty reduces blood loss.

PURPOSE: Extensive blood loss after total knee arthroplasty (TKA) may be a potential problem since it leads to anaemia, increased need for transfusion and prolonged hospitalization. Aim of this study was to investigate the effects of postoperative knee flexion after TKA on blood loss and the need for transfusion. METHODS: One hundred consecutive patients undergoing primary TKA from 2012 to 2013 were randomizely divided into two groups. In one group, the knee was extended for the first 6 h after surgery, whereas in the other was flexed at 90° for the same time. Two doses of endovenous tranexam

Postoperative numbness of the knee following total knee arthroplasty.

INTRODUCTION: The infrapatellar branch of the saphenous nerve is often injured during total knee arthroplasty (TKA), leading to numbness in its distribution distal to the incision. This is illustrated in a patient who suffered full-thickness burns to the lateral aspect of the knee from a hot water bottle. However, the proportion of patients who are informed of this phenomenon (as well as the effect of informing the patient of numbness) has not previously been studied according to the authors' knowledge. The aim of this study was to establish the proportion of patients with whom postoperative n

Total knee replacement in patients with below-knee amputation.

Total knee replacement (TKR) is reserved for patients with severe and disabling arthritis that is non-responsive to conservative measures. Based on existing data, total knee replacement is a safe and cost-effective treatment for alleviating pain and improving physical function in patients who do not respond to conservative therapy. Despite the large variation in health status of patients and types of prosthesis implanted, total knee replacement has proven to be a relatively low risk and successful operation. Each year in the United States surgeons perform approximately 300,000 TKR. Likewise, l

Role of total knee replacement in failed knee fusions.

The author describes his technique and the results of total knee replacement in four patients who already had fused knees. In two, the patella was present so a semiconstrained prosthesis was used, but in the other two a stabilized knee prosthesis was used because the patella was absent. In the first case, lack of experience with the technique led to failure of the prosthesis and revision was necessary, but in subsequent cases the patients have done well. Quadriceps control was not a problem. Range of movement improved slowly over the first year. The author concludes that disassembly of knee fu

Knee arthrodesis following total knee arthroplasty in rheumatoid arthritis.

Twenty-seven knees in 23 patients, all with seropositive rheumatoid arthritis and failed total knee arthroplasty, were treated by arthrodesis. Twenty of the 27 knees were solidly fused. A fusion aligned in 7 degrees +/- 5 degrees of valgus and knee flexion from zero to 30 degrees was associated with the highest rate of arthrodesis, the lowest rate of progression of disease in other joints, and the highest functional scores. Stable fixation using either internal or external fixation gave the most predictable rate of arthrodesis. Persistent sepsis and bone stock losses were associated with failu

Total Knee Arthroplasty With Megaprosthesis for Idiopathic Charcot Knee.

Charcot knee is a rare condition with few reported cases in the literature. Although the clinical presentation is highly variable, Charcot knee classically presents with painful arthropathy in addition to instability and decreased range of motion. Herein, we describe the case of a 72-year-old man with idiopathic Charcot knee. The diagnosis was established on the basis of history of neuropathic arthropathy of the knee, supported by knee radiographs and the exclusion of all known etiologies of Charcot knee. Because of a highly destructed knee joint, the patient underwent a total knee arthroplast

Medial unicompartmental knee arthroplasty in the ACL-deficient knee.

Symptomatic osteoarthritis (OA) of the knee develops often in association with anterior cruciate ligament (ACL) deficiency. Two distinct pathologies should be recognised while considering treatment options in patients with end-stage medial compartment OA and ACL deficiency. Patients with primary ACL deficiency (usually traumatic ACL rupture) can develop secondary OA (typically presenting with symptoms of instability and pain) and these patients are typically young and active. Patients with primary end stage medial compartment OA can develop secondary ACL deficiency (usually degenerate ACL rupt

Total knee replacement with the Kodama-Yamamoto knee prosthesis.

Since 1970, 170 knees have been operated upon using the Kodama-Yamamoto knee prosthesis. Of these 127 had the revised Mark-II prosthesis and of this group 91 cases have been followed for more than a year. Interim results suggest that the total knee score has improved. on the average, from 38 points preoperatively to 74 points after one year. A single early complication involved loosening of the prosthesis in a case of knee infection. Except for this case, no loosening and no radiolucent lines have been observed. Methacrylate bone cement is known to be harmful to bone and soft tissues, and our

Knee fusion versus above knee amputation as two options to deal with knee periprosthetic joint infection.

BACKGROUND: Periprosthetic joint infection (PJI) poses a significant challenge in total knee arthroplasty (TKA), with recurrence rates as high as 14-28%, leading to substantial morbidity and treatment costs. When conventional treatments fail, knee fusion and above-the-knee amputation (AKA) emerge as alternative options. Existing literature offers conflicting views on the efficacy and impact of knee fusion versus AKA with varied outcomes and limitations. METHODS: This retrospective national study spanning 2010-2022 investigates Knee Fusion and AKA as options for addressing Knee PJI. Utilizing

Radiographic assessment of knee-ankle alignment after total knee arthroplasty for varus and valgus knee osteoarthritis.

BACKGROUND: There are unanswered questions about knee-ankle alignment after total knee arthroplasty (TKA) for varus and valgus osteoarthritis (OA) of the knee. The aim of this retrospective study was to assess knee-ankle alignment after TKA. METHODS: The study consisted of 149 patients who had undergone TKA due to varus and valgus knee OA. The alignment and angles in the selected knees and ankles were measured on full-length standing anteroposterior radiographs, both pre-operatively and post-operatively. The paired t-test and Pearson's correlation tests were used for statistical analysis. RE
