Posterior dislocation of total knee arthroplasty.

Posterior dislocation of the prosthesis after total knee arthroplasty is an infrequent but serious complication. Seven patients with this complication were treated from January 1985 until October 1989. Five of the seven dislocations occurred in primary total knee arthroplasties and two occurred after revision arthroplasty. Limb alignment before arthroplasty, when it could be determined, was valgus in all patients, averaging 25 degrees. In each case there was an identifiable problem with the knee extensor mechanism: five had patellar dislocations, one a patellar tendon rupture, and one a patell

Extensor Mechanism Disruption in Knee Dislocation.

Disruption of the knee extensor mechanism is a challenging injury with no clear consensus on optimal treatment. Although rare in the setting of knee dislocations, these injuries should not be overlooked. Acute, complete rupture of either the quadriceps or patellar tendon necessitates primary repair with or without augmentation. Surgical management may also be required in the setting of a partial tear if a significant extensor lag is present or nonoperative treatment has failed. Tendon augmentation is used during primary repair if the native tissue is inadequate or after a failed primary repair

Patellar dislocation following total knee replacement.

The reported incidence of patellar problems after total knee replacement has ranged from 5 to 30 per cent. Patellar dislocation is infrequent but can cause disabling symptoms. Between January 1974 and May 1982, eleven patients (twelve knees) with symptomatic lateral dislocation of the patella after total knee replacement were treated at The Hospital for Special Surgery by realignment of the extensor mechanism. All of the patients were women. Their average age was sixty-two years and average weight, eighty-seven kilograms. The diagnosis was osteoarthritis in seven knees and rheumatoid arthritis

Instability Testing for Congenital Hip Dislocation: Knee Extension Provokes Hip Dislocation.

The classic Ortolani and Barlow signs are routinely used to diagnose hip instability secondary to severe acetabular dysplasia in the newborn. However, eliciting a positive sign depends largely on the experience of the examiner and the subjective amount of manual pressure the examiner applies on the baby's hips. Furthermore, these signs do not give a clue for the selection of a maturation or immobilization device after reduction of an unstable hip: below-knee hip spica, above-knee hip spica or a Pavlik harness. The aim of this study is to describe a clinical sign that could be useful in detecti

Delayed presentation: dislocation of the proximal tibiofibular joint after knee dislocation.

Attention is brought to a unique case of an anterior dislocation of the proximal tibiofibular joint detected 1 month following closed reduction of a posterior knee dislocation. Open reduction and internal fixation were necessary to achieve a stable proximal tibiofibular joint. Additional attention should be paid to the proximal tibiofibular joint when evaluating acute or chronic knee dislocation.

Dislocation of the knee with lateral dislocation of the patella. A report of four cases.

In a group of 25 patients with traumatic dislocation of the knee, four, all of whom had similar ligament and medial soft-tissue injuries, also had associated lateral patellar dislocation. In all four reconstruction was delayed because of their other serious injuries. Having encountered the combination of knee dislocation and lateral patellar dislocation in 16% of our patients, we believe that it may be less rare than is commonly believed. We think that it is important to maintain a high index of suspicion of possible patellar dislocation when medial structures have been severely damaged. Early

Lateral patella dislocation associated with an irreducible posterolateral knee dislocation: literature review.

Posterolateral dislocations involve significant injury to the medial structures of the knee, therefore particular attention should be paid to repairing the medial patella stabilizers at the time of open reduction.

Total knee replacement for osteoarthritis of the knee with congenital dislocation of the patella.

We carried out three total knee replacements with proximal realignment in two patients with severe osteoarthritis of the knee and congenital dislocation of the patella. During the operation, the femur and the tibia were cut according to the recommendations of the manufacturer of the implant. The femoral component was placed in external rotation and the centre of the tibial component aligned in relation to the tibial tuberosity with regard to rotation and translation. After making the bone cuts, the iliotibial tract was detached from Gerdy's tubercle, the popliteus tendon divided, and the bicep

A Rare Case of Congenital Knee Dislocation.

Congenital knee dislocation (CKD) is a rare condition that can present as an isolated finding or in association with genetic syndromes. It is most commonly diagnosed postnatally and classified into three grades based on severity. While generally having a favorable prognosis, particularly for milder cases, prompt treatment is crucial. We present a case of CKD successfully treated with conservative measures, with serial casting and Pavlik harness, in an otherwise healthy infant. This report aims to increase awareness of the clinical presentation and management of CKD in general practitioners giv

Ipsilateral dislocation of knee, foot and ankle.

We present a case of ipsilateral dislocation of the knee, ankle and midfoot. No previous report of this combination of injuries was found in a review of the literature. In spite of the type of injury, no neurovascular compromise or complications turned up. Closed reduction of the dislocations was possible and performed under general anaesthesia. At the last examination, 4 years after injury, the alignment of the joints was preserved, and the range of motion of the three joints resembled that of the opposite side. Without fractures, the closed reduction and temporary immobilisation of all the a

Vascular and orthopedic complications of knee dislocation.

Experience with complete dislocation of the knee in 22 consecutive patients during a six year period was analyzed. Major vascular complications occurred in nine of 13 extremities with anterior dislocation, one of seven extremities with posterior dislocation and none of two extremities with lateral dislocation. Liberal use of trans-femoral ateriography for diagnosis disclosed significant arterial injuries in four of 15 limbs, despite postreduction pedal pulses which were apparently normal. Limb salvage was accomplished in 20 of 21 survivors and in eight of nine with associated vascular complica

Irreducible Posterolateral Knee Dislocation: A Case Report.

CASE: A 62-year-old male patient suffered an irreducible posterolateral knee dislocation after a horse fell on him. The left knee was slightly flexed with a medial dimple sign present. The medial retinaculum, medial patellofemoral ligament, posteromedial corner structures, and vastus medialis obliquus (VMO) muscle were incarcerated in the medial joint. An open reduction and the medial retinaculum and VMO gap were repaired, and the knee was stabilized in an external fixator for 4 weeks. At 32-month follow-up, the patient had almost full knee motion and good subjective outcomes with moderate res

Surgical outcomes after traumatic open knee dislocation.

The purpose of this study is to describe the types of injuries and surgical treatments associated with open knee dislocations and to present the functional outcomes of these patients. Between 2001 and 2005, the medical records of patients that sustained traumatic open knee dislocations at our Level 1 Trauma Center were retrospectively reviewed. Initial surgical intervention was performed in all patients including placement of spanning external fixator, repair of vascular injuries if necessary, and irrigation and debridement of the open wounds. Ligamentous reconstruction was delayed until after

Outcomes of the operatively treated knee dislocation.

Despite being a serious injury with early complications associated with a high degree of morbidity, the outcomes of knee dislocations have improved dramatically since the days of Sir Astley Cooper. Currently, most authors favor early surgical reconstruction of the injured ligamentous structures. With modern surgical techniques and rehabilitation protocols, most patients are able to achieve functional range of motion and stability necessary for everyday life. Despite improvements in treatment, stiffness and pain, rather than instability, can be a problem. Patients continue to have difficulty re

An irreducible knee dislocation: a case report.

An 8-year-old man presented after sustaining an injury during a fall. A closed reduction attempt failed, and after several tests, an open reduction was performed. With posterolateral dislocation of the knee, there can be anterior cruciate ligament, posterior cruciate ligament, and medial collateral ligament disruption. At the 6-month (final) follow-up, the patient had no subjective pain or instability. With this type of injury, the approach can be conservative monitoring or repair of all of the ligaments. Because of the age and activity level of our patient, we opted for repair of the medial c

Femoral-sided fracture-dislocation of the knee.

OBJECTIVE: To document a previously undescribed fracture-dislocation of the knee that involves a femoral condyle and associated ligaments yet spares the tibial joint surface. DESIGN: Retrospective. SETTING: University. PATIENTS: Four cases of fracture-dislocation of the knee that involve ligamentous injury and fracture of the femoral condyle(s). RESULTS: At a minimum two-year follow-up (average 28.8 months, range 26 to 37 months), overall functional outcome is only fair to good in this injury pattern. The Lysholm knee rating averaged 60 (range 39 to 74) and KT-1000 I-N testing averaged thr

Arthroscopic view of an irreducible knee dislocation.

Irreducible knee dislocation is a rare effection of the knee joint; only 23 cases have been reported in the literature. This is a case report of a 35-year-old man who injured his left knee in a motorcycle accident. Dislocation was documented on radiograph. His neurovascular status was intact. Attempts under sedation and anesthesia and even arthroscopy failed to reduce the dislocation. Eventually, the patient had open reduction of the dislocation. An arthroscopy view of the dislocation showed that the medial femoral condyle was buttonholed through the anteromedial capsule and retinaculum. Arthr

Acute Management of Traumatic Knee Dislocations.

Acute management of a knee dislocation requires prompt recognition. A high index of suspicion should exist in any trauma patient with knee pain or an effusion. A thorough clinical examination to identify vascular or neurologic injury is critical. Reduction must be performed if the knee presents dislocated and open reduction is warranted if the knee is irreducible. Stabilization can occur with a splint or knee immobilizer, but if reduction is unable to be maintained, an external fixator should be placed. Patients should be monitored postreduction for 24 to 48 hours with serial examinations to d
