Serum angiotensin-converting enzyme (ACE) was studied in 50 patients with sarcoidosis (39 active, 11 inactive) as well as in 50 control patients (34 with chronic nonspecific lung disease, 9 with Hodgkin and 7 with rheumatoid arthritis). There was a significant difference of ACE activity between sarcoidosis patients (28.6 +/- 11.4) and controls (14.8 +/- 4), and also between active (32.8 +/- 11) and the inactive (21.9 +/- 5.1) sarcoidosis (p less than 0.001). PURPOSE: The ophthalmic examination of patients with diagnosed systemic sarcoidosis.
MATERIAL AND METHODS: 33 patients (17 women, 16 men), aged 22-60 years had ophthalmic examination (visual acuity, anterior and posterior segment evaluation, applanation tonometry). In 8 patients repeated examination was performed (duration of observation: 2-31 months).

Seventeen cases of gastric sarcoidosis have been reported in Japanese literature. Of some 750 sarcoidosis patients, 27 were found to have involvement of their nasal mucosa. Most had multisystem disease, which was usually chronic. Interstitial lung disease developed in a 32-yr-old chemist after working 8 yr in a dusty atmosphere containing aluminum powders. Bronchoalveolar lavage disclosed a helper T-lymphocyte alveolitis, and transbronchial lung biopsies showed sarcoidlike epithelioid granulomas. Sarcoidosis is being increasingly recognized in India. Over the last 7 years we studied 40 patients. Older males out numbered young females. Thoracic (98%), constitutional (50%) and ocular (40%) involvement dominated the clinical profile. In addition, erythema nodosum (20%), facial palsy (13%), parotid enlargement (8%), lymphadenopathy (42%), hepatomegaly (37%) and splenomegaly (17%) were also observed. The authors present a personally studied series of 32 patients with sarcoidosis affecting the upper respiratory tract and nose. Twenty-five of these patients had lupus pernio (LP), which in 8 instances was accompanied by sarcoidosis of the upper respiratory tract (SURT). One hundred eighty-eight Caucasian patients with cutaneous sarcoid lesions were studied prospectively. Twenty-five had erythema nodosum, while 163 had infiltrative cutaneous lesions. A wide variety of exogenous or endogenous aggressors exhibit the capacity to induce pulmonary granulomatous lesions. This lesion is characterized by exacerbation of phagocytic function and tentative development of a specific immune response. In many clinical situations there is a well developed circulating antibody response with deficient cell-mediated immunity of variable degree. Finally, it is argued that the granulomatous lesion may be due to abnormal cooperation between T and B lymphocytes.
