Neurorrhaphy of the recurrent laryngeal nerve.

The authors have experimentally performed neurorrhaphy and its modifications and nerve grafting with end-to-end anastomosis of the recurrent laryngeal nerve in dogs. Recovery of a neurorrhaphy of the recurrent laryngeal nerve resulted in the bizarre and spasmodic movement of the vocal cords. These effects were recorded by means of electromyography and 16 mm cinematography to demonstrate restoration of nerve muscle function and vocal cord movements. From these results, it was concluded that these single modes were not adequate for restoration of laryngeal function, however, some specific attent

Paraganglioma of the Recurrent Laryngeal Nerve.

BACKGROUND/OBJECTIVE: Paragangliomas are rare neuroendocrine tumors that primarily arise in the adrenal gland. Head and neck paragangliomas comprise approximately 3% of all extra-adrenal paragangliomas, with a majority of those being found in the carotid body. Recurrent laryngeal nerve paragangliomas are exceedingly rare, with only 2 reported cases found in literature review. Here, we will present the third. CASE REPORT: The patient is a 46-year-old woman with a history of a right thyroid nodule that had been previously biopsied benign with "paucity of diagnostic material." Neck ultrasonograp

Recurrent Laryngeal Nerve Stimulator.

Electrical stimulation of the recurrent laryngeal nerve is a safe and promising therapeutic approach with the potentiality to overcome the shortcomings of conventional surgical glottal enlargement. Although aberrant or synkinetic reinnervation is commonly considered an unfavorable condition, particularly for recovery of vocal fold movement, its presence is essential to ensure the effective clinical performance of laryngeal pacemakers. Thus, the effective selection of patients who can profit from laryngeal pacemakers implantation demands the implementation of new diagnostic tools based on tests

Non-Recurrent Laryngeal Nerve.

UNLABELLED: The aim of the study was to assess the frequency of non-recurrent laryngeal nerves (Non-RLN). MATERIAL AND METHODS: A total of 6110 patients were operated in our hospital between 1 January 2005 and 31 December 2013 for various goiters (5429) and various types of hyperparathyroidism (618). Laryngeal nerve was exposed during operation in 1700 patients from superior aperture of the chest to superior aperture of the larynx. Identification process of RLN was started with dissecting inferior thyroid artery (ITA) and its junction with the nerve. Then main trunk of the nerve was exposed b

[Superior laryngeal nerve neuropathy].

Clinical examination of 28 patients with superior laryngeal nerve neuropathy along with X-ray and electromyographic studies has demonstrated that the disease was associated with cervical spine osteochondrosis. The latter condition was responsible for myofixation, conjugation of laryngeal ligaments and fascii that resulted in the compression of superior laryngeal nerve.

Neurorrhaphy of the recurrent laryngeal nerve.

BACKGROUND: It has been controversial to anastomose the severed recurrent laryngeal nerve after transection because when severed recurrent nerves are directly anastomosed, the vocal cords do not regain normal movements and are fixed in the median. The objective of this study was to learn whether direct neurorrhaphy is necessary if the recurrent laryngeal nerve is severed during thyroid or parathyroid surgery. STUDY DESIGN: From 1998 to 2001, 12 patients who had a complete recurrent laryngeal nerve injury during thyroid or parathyroid surgery were enrolled into this study. Eight had primary re

[Preservation of the recurrent laryngeal nerve].

A strategy to protect the inferior laryngeal nerve (ILN, recurrent and non-recurrent) as well as the superior laryngeal nerve (SLN) includes the reason for and extend of the operation to avoid recurrent goiter, selection of an experienced institution and operating team, a cautious intubation, adequate and anatomical operating technique, recognition of risk factors and postoperative treatment with a sufficient dosage of hormone and/or iodine. The ILN should be dissected routinely in view of its anatomical variation. The "capsular dissection technique" should be applied. The "laterialisation tec

The anatomy of the inferior laryngeal nerve.

Contradictory opinions are found in the literature concerning the precise anatomy and role of the inferior laryngeal nerve, the terminal portion of the recurrent laryngeal nerve. Moreover, operative damage to this nerve beyond the thyroid area is seldom described. Twenty-one human larynges were dissected to give a precise description of the inferior laryngeal nerve and to draw attention to the risks of injury to the nerve during specific laryngological operations. In contrast with the varied descriptions found in the literature, only small variations in the terminal branching of the nerve were

Electronic control of laryngeal spasm. I. Blockage of orthodromically induced action potentials in intact canine recurrent laryngeal nerves.

Spastic dysphonia is a central nervous system phenomenon of unknown etiology characterized by uncoordinated voice tremor with erratic patterns of laryngeal contraction. Standard treatments have not been entirely satisfactory. The authors propose to apply a concept of selective nerve activity blockage, which leaves normal contractions undisturbed, as the basis for suppression of laryngeal spasticity. Single pulses of constant duration and increasing amplitude were injected into specially designed blocking electrodes placed around six recurrent laryngeal nerves (three dogs). Vocal cord adduction

Effect of recurrent laryngeal nerve identification technique in thyroidectomy on recurrent laryngeal nerve paralysis and hypoparathyroidism.

OBJECTIVE: To investigate whether the recurrent laryngeal nerve (RLN) identification technique used in thyroidectomy affects RLN paralysis and hypoparathyroidism. DESIGN: Patients were allocated into 2 groups according to the thyroidectomy technique used to identify the RLN: (1) superior-inferior direction, exploring the nerve where it enters the larynx, followed by superior pedicle ligation; and (2) inferior-superior direction, following the inferior pedicle ligation and identifying the nerve in the tracheoesophageal groove. The first and second groups included 67 and 128 patients, respectiv

Innervation of the human cricopharyngeal muscle by the recurrent laryngeal nerve and external branch of the superior laryngeal nerve.

PURPOSE: The major component of the upper esophageal sphincter is the cricopharyngeal muscle (CPM). We assessed the contribution of the laryngeal nerves to motor innervation of the CPM. METHODS: We performed an intraoperative electromyographic study of 27 patients. The recurrent laryngeal nerve (RLN), vagus nerve, external branch of the superior laryngeal nerve (EBSLN), and pharyngeal plexus (PP) were stimulated. Responses were evaluated by visual observation of CPM contractions and electromyographic examination via insertion of needle electrodes into the CPM. RESULTS: In total, 46 CPMs (24

Laryngeal reinnervation with nerve-nerve anastomosis versus laryngeal framework surgery alone: a comparison of safety.

OBJECTIVE: To characterize any additional risks and rate of complication associated with laryngeal reinnervation in comparison to traditional laryngeal framework surgery. DESIGN: Retrospective chart review over a 5-year period. RESULTS: A total of 16 reinnervation procedures were performed to rehabilitate a paralyzed vocal fold. Of the 16 procedures, 12 (75%) were combined with arytenoid adduction. One reinnervation was aborted due to presence of scar and vascular malformation in the tracheal-esophageal groove. There were no major complications (airway obstruction or death). There were no ca

Effect of recurrent laryngeal nerve paralysis on superior laryngeal nerve afferents during evoked vocalization.

We tested the hypothesis that vocal fold paresis leads to a substantial reduction in activity from the internal branch of the superior laryngeal nerve (iSLN) during respiration and evoked vocalization. The iSLN afferent activity was measured before and after recurrent laryngeal nerve paresis by cold block in decerebrate cats during spontaneous respiration and electrically evoked vocalization. Response rate patterns of 33 iSLN single units from 11 cats were categorized into 5 groups based on responses to vocalization. Only 24% of single units during spontaneous respiration and 18% during evoked
