[On fronto-anterior reconstructive laryngectomy].

The paper describes improved fronto-anterior reconstructive laryngectomy using liberated specifically pediculated and lowered glottis to replace the removed anterior segment of the thyroid cartilage. The very technique as well as its advantages are discussed. The main advantage of the method is that it makes use of the pediculates, well vasculated glottal cartilage covered with cilliar epithelium preserving the physiological direction of the cilliar movement. The cartillage is significantly thicker than the free nasal septum cartilage transplant used until recently. Post-operative discomfort i

Results of near-total laryngectomy.

The clinical records of 225 patients undergoing primary or salvage near-total laryngectomy (NTL) for laryngeal and pyriform cancer were analyzed for local control and morbidity. If the primary cancer was laryngeal in origin, patients underwent a simple NTL; if it was pyriform, a minor modification called near-total laryngopharyngectomy (NTLP) was used. When NTLP was extended to include necessary portions of the tongue base or posterior pharyngeal wall, pharyngeal reconstructions were added. The principal outcomes studied were 1) 5-year local control of the primary cancer, 2) achievement of lun

Horizontovertical laryngectomy for supraglottic carcinoma.

Recent decades have seen the expansion of indications for partial laryngectomy. This study focused on the application of horizontovertical laryngectomy for T2 and selected T3 and T4 supraglottic carcinomas. An osteomuscular flap was designed to reconstruct the laryngeal defect to preserve all the essential laryngeal functions. Seventy-six patients treated between November 1979 and October 1990 were reviewed retrospectively. All but two were followed up for at least 5 years. There were 11 cases of T2, 57 cases of T3, and 8 cases of T4 tumors. The survival rates of 7 cases of stage II, 56 cases

Deglutition after supra-glottic laryngectomy.

This study of swallowing after a horizontal supra-glottic laryngectomy proposes to analyze the frequency of long-term post-operative complications, their mechanisms of onset, as well as their compensatory mechanisms. It rests on the study of three groups of patients: a retrospective series of 74 charts, a group of 14 patients studied at a distance from the intervention, and a group of 15 patients who have been entered in a prospective study concerning the post-operative care of partial laryngectomies. For these two groups, the swallowing work-up consisted of, besides a clinical evaluation, a v

Partial laryngectomy in transglottic carcinoma.

At the ENT Department of Zagreb University from 1973 to 1982 119 cases of transglottic carcinoma were treated. In the first group of 20 cases total laryngectomy was done. Five years later, ten of the patients treated had survived. Primary and secondary neck metastases were present in 6 cases, i.e. 30%. In the second group of 99 cases partial laryngectomy (vertical and combined) was done. Five years later 70 cases had survived, i.e. 70%. Primary and secondary neck metastases were present in 19 cases, i.e. 19%. In the partial laryngectomy the reconstruction was done by fascia sternohyoidea and p

[Phoniatric aspects of reconstructive laryngectomy].

Reconstructive laryngectomy has been performed at the E.N.T. Clinic in Ferrara for some time. After surgery the patients undergo speech therapy and phoniatric treatment for a period of time varying from 2 to 6 months. In order to better evaluate the vocal quality obtained after such reeducation the vocal emissions of 25 patients were examined and recorded. The recorded material, made up of prolonged vowels and 6 phonetically balanced sentences, was then evaluated by a panel of 7 "trained" listeners. The evaluation score-card proposed by Woiers in 1977 was used in taking the data. This not only

Deglutition following supraglottic horizontal laryngectomy.

In order to determine factors that may contribute to deglutition problems following supraglottic horizontal laryngectomy or its modified techniques, clinical records of 38 patients were studied. Contribution of the following factors was investigated: age; sex; tumor classification; radical neck dissection; extent of and symmetry in removal of the aryepiglottic folds, arytenoid cartilages, and false folds; removal of the base of the tongue, hyoid bone, and a part of the vocal folds; extent of removal of the epiglottis and thyroid cartilage; cricopharyngeal myotomy; and some complications and co

Epiglottic reconstruction after supraglottic laryngectomy.

It has been well established that supraglottic laryngectomy is an effective treatment of laryngeal cancer arising above the vocal cords with cure rates equaling total laryngectomy. Although there is preservation of a near normal voice after supraglottic laryngectomy, chronic aspiration occurs in some patients particularly after extended supraglottic laryngectomy or when there is associated compromised pulmonary function. During normal deglutition, the epiglottis serves to divert food to the pyriform fossae and partially covers the inlet to the airway. These important functions can be accomplis

Partial laryngectomy for transglottic cancers.

A reconstructive procedure was utilized after three-quarter laryngectomy in five patients with transglottic cancers. The physiologic reconstruction was successful in four of the five, and tumor control was 100% in all of the patients. It is concluded that with proper selection patients with transglottic cancers may survive after partial laryngeal surgery.

[Enlarged supraglottic laryngectomy (author's transl)].

Malignant tumour of the upper hypopharynx is the most often the continuation of the supraglottic cancer of the larynx or it develops from the beginning between the lingual site of the epiglottis and the base of the tongue. The enlarged supraglottic laryngectomy has the best results. The large defect between the partial resected base of the tongue and the border of the resected laryngeal cartilage was covered by the fascia of the sternohyoid muscle. From 22 cases operated from 1979 on this way one local relapse was found.

Limitations of horizontal partial laryngectomy.

The author considers successively: 1--The theoretical justifications for horizontal partial laryngectomy (anatomical considerations, indications as to the modalities of extension of supraglottic tumors, results concerning the cancer itself, functional results). 2--The indications of and contraindications for this type of surgery, with special reference to two types of cancers: median supraglottic carcinomas and antero-lateral epilarynx carcinomas. 3--Some specific problems, namely that of large supraglottic laryngectomy, of combined nodal surgery, and of preoperative radiotherapy. The limits o

Coblation-assisted transoral supraglottic laryngectomy.

BackgroundTransoral supraglottic laryngectomy (TSL) has been widely applied in the treatment of supraglottic cancers. The aim of this study is to evaluate a simplified technique for excising properly selected supraglottic tumors with a transoral coblation-assisted system.MethodsEight patients with T1-3N2M0 supraglottic cancer were treated with TSL with the coblation-assisted system.ResultsTumors in all patients were en bloc resected with negative surgical margins and the selective neck dissection was also performed. The mean operation time for TSL was 39 minutes (25-65 minutes). The average ti

Evaluation of hypoparathyroidism following laryngectomy.

BACKGROUND: Hypoparathyroidism is a recognised complication following laryngectomy; it is associated with significant short-and long-term morbidity. This study aimed to further characterise this condition, identify risk factors and describe preventative and management strategies in a large cohort. MATERIALS AND METHODS: This was a retrospective study at a tertiary referral centre for head and neck cancers. All consecutive patients who had total laryngectomy over an eight-year period were included. RESULTS: A total of 140 patients were included. Rates of transient and long-term hypoparathyroi

Total Laryngectomy-Still Cutting-Edge?

Surgical removal of the larynx (total laryngectomy) offers a curative approach to patients with advanced laryngeal and hypopharyngeal (squamous cell) cancer without distant metastases. Particularly in T4a carcinoma, laryngectomy seems prognostically superior to primary radio(chemo)therapy. Further relevant indications for laryngectomy include massive laryngeal dysfunction associated with aspiration and recurrence after radio(chemo)therapy, resulting in salvage surgery. The surgical procedure including neck dissection is highly standardised and safe. The resulting aphonia can be compensated by

Robotic-assisted salvage supraglottic laryngectomy.

Traditional open surgery has historically been the standard approach to treating many head and neck conditions. The introduction of the first robot into the surgical world in 1985 has been a keystone in the development of minimally invasive surgical (MIS) techniques. Transoral robotic surgery (TORS) is a minimally invasive technique used for the treatment of head and neck pathologies, including benign and malignant lesions. When performed in select patients, TORS offers low post-operative morbidity, along with very few functional and cosmetic compromises. Herein, we present the first TORS supr
